Estilo de vida e fatores de risco de pacientes com primeiro episódio de infarto agudo do miocárdio

The prevention of acute myocardial infarction (AMI) has been related to the identification and control of risk factors (RF) present in the life style of the individuals. The aim of this study was to know the profile of patients with a first episode of AMI and to identify RF for this disease. Seventy eight in patients from the Coronary Care Unity of a University Hospital were interviewed. The results showed that the life style of the studied group includes habits that collaborate to coronary artery disease maintenance or progress. These results will help to elaborate an educational program aiming to prevent reinfarction and to promote health.A prevencao do infarto agudo do miocardio (IAM) relaciona-se com a identificacao e controle dos fatores de risco (FR) presentes no estilo de vida dos individuos. Tal estudo propoe-se a conhecer o perfil dos pacientes com um primeiro episodio de IAM, identificando os FR para essa doenca. Foram estudados 78 pacientes, atendidos na Unidade Coronaria de um Hospital Universitario. Os resultados mostraram que o grupo estudado possui, em seu estilo de vida, habitos que propiciam a manutencao e/ou progressao de coronariopatia. Estes achados subsidiarao a elaboracao de um programa educacional, com vistas a prevencao de reinfarto e promocao de saude.

Clinical and psycossocial features of heart failure patients admitted for clinical decompensation

This study had the purpose to identify the sociodemographic and clinical profiles, history of hospitalizations due to Heart Failure (HF) and follow-ups (regular appointments, drug treatment, facilities and difficulties for follow-up) of patients admitted for clinical decompensation. Interviews were held with 61 patients, with average age of 58.1 (± 15.9) years, 3.5 (± 4.4) years of education and individual income of 1.3 (± 2.4) times the minimum wage. Most subjects were in functional classes III or IV of the New York Heart Association, having signs and symptoms of the congestive form of HF as the most frequent cause of hospitalization. Of all subjects, 75.4% reported clinical follow-ups, although they tended to be irregular. The use of drug therapy occurred in lower ratios than that recommended in the literature. Development of an instrument to measure beliefs and attitudes from heart valve disease patients

Foi objetivo deste estudo verificar a validade de conteudo e a confiabilidade do instrumento para mensuracao das crencas e atitudes dos pacientes valvopatas sobre sua doenca e tratamento (Instrumento-CAV). O instrumento foi analisado por tres juizes (utilizando criterios pre-determinados) e submetido ao pre-teste (n= 17 sujeitos). A maioria dos itens foi avaliada como pertinente, clara e de significância para a questao analisada. O pre-teste evidenciou a necessidade de pequenas modificacoes na construcao de algumas afirmativas, que otimizaram a compreensao do instrumento pelos pacientes. O instrumento re-estruturado foi aplicado em 46 pacientes para verificacao da consistencia interna. A totalidade do instrumento e a maioria de suas escalas apresentaram consistencia interna satisfatoria. Conclui-se que o instrumento e pertinente ao objeto de estudo e internamente consistente, ratificando a adequacao de sua aplicacao para mensuracao da forca de associacao entre variaveis pesquisadas.Fue objetivo de este estudio verificar la validez del contenido y la confiabilidad del instrumento para mensuracion de las creencias y actitudes de los pacientes con enfermedad de las valvulas cardiacas sobre su enfermedad y tratamiento (Instrumento-CAV). La mayoria de los itenes fue evaluada como pertinente, clara y significante para la cuestion analisada. Con prueba se evidencio la necesidad de pequenos cambios en la construccion de algumas afirmativas, que optimizaron la comprension del instrumento por los pacientes. El instrumento reestructurado fue aplicado en 46 pacientes para verificacion de la consistencia interna. La totalidad del instrumento y la mayoria de sus escalas presentaron consistencia interna satisfactoria. Se pudo concluir que el instrumento es pertinente al objecto de estudio y internamente consistente, ratificando la adecuacion de su aplicacion para mensuracion de la fuerza de asociacion entre las variables investigadas.

Qualidade de vida em pacientes coronariopatas

Este estudo comparou as medidas de qualidade de vida (QV) entre 49 sujeitos com infarto do miocardio (IM) e 27 com angina pectoris (AP), atendidos em ambulatorio em um hospital universitario. Os dados foram obtidos por meio da aplicacao de um questionario para levantamento de dados sociodemograficos e clinicos e do instrumento SF-36 para avaliacao da QV. Constatou-se, em ambos os grupos, comprometimento da QV nas dimensoes: aspectos fisicos, dor, estado geral de saude, vitalidade, aspectos sociais e saude mental. Variaveis sociodemograficas e clinicas foram associadas com menores escores de QV em diferentes dimensoes. Estudos futuros devem ser conduzidos, visando a ampliacao do tamanho da amostra, e a compreensao da magnitude do efeito destas variaveis sobre a QV do coronariopata.

Correlação entre instrumentos de qualidade de vida relacionada à saúde e independência funcional em idosos com insuficiência cardíaca

The main purpose of this study was to identify the correlation between health-related quality of life and the Functional Independence Measure (FIM) in elderly patients with heart failure. 146 elderly outpatients completed the Minnesota Living with Heart Failure Questionnaire (LHFQ) and the FIM. Spearman correlation coefficient was used to identify correlations between health-related quality of life and FIM, and between them and New York Heart Association (NYHA) class. Statistically significant moderate correlations were found between physical and emotional LHFQ and total FIM and motor subscale. The results show that quality of life and functional independence are correlated concepts influenced by NYHA class and suggest that measures to promote functional independence (especially in the physical domain) can improve health-related quality of life in elderly patients with heart failure. Further research should include a larger sample and older patients with greater disease severity in order to verify the reproducibility of these findings.

Nurses' attitudinal and normative beliefs concerning hemodynamic assessement by pulmonary artery catheterization

The objective of this study was to identify, by using the Theory of Reasoned Action/Theory of Planned Behavior, the attitude and normative beliefs that influence the behavioral intention of the nurse to perform a hemodynamic assessment using the pulmonary artery catheterization. Data were collected through semi-structured interviews involving 23 nurses from three hospitals in the city of Campinas, São Paulo. The data were analyzed according to a qualitative methodology. Among the Attitude Beliefs, affective beliefs and those related to the advantages and disadvantages of performing the behavior stand out. Among the Normative Beliefs social referents were identified for the behavior, as well as the behavior-stimulating factors and the factors that discourage the performance of the behavior.

Caracterização da obesidade em pacientes com infarto do miocárdio

This study aimed to characterize obesity in infarcted myocardial patients. Data were collected from 43 patients at a University Hospital. It was observed that the Body Mass Index (BMI) was above the desired level for 69.7% (26.9 +/- 0.5) of the subjects; the body fat percentage had altered in 79% (20.0 +/- 1.0) of cases; the waist/hip (W/H) ratio was above normal in 72.5% (0.97 +/- 0.03) of cases and the waist circumference (WC) had altered in 64% (97.3 +/- 1.8) of the patients. Since the correlation for BMI, WC and fat percentage was significant, the use of BMI and WC was suggested for evaluating obesity in outpatients.This study aimed to characterize obesity in infarcted myocardial patients. Levantamento de subsídios para assistência de enfermagem ambulatorial a pacientes portadores de valvopatia aórtica

Este trabalho objetivou caracterizar o perfil do paciente ambulatorial com valvopatia aortica, identificar o grau de conhecimento sobre sua doenca, suas principais limitacoes no cotidiano e como ocorre o processo de enfrentamento nessa situacao. Os dados obtidos atraves de entrevista semi-estruturada aplicada a 12 pacientes e analisados atraves de metodologia de analise quantitativa e qualitativa, permitiram identificar como estes sujeitos representam para si a doenca e o tratamento, bem como suas implicacoes no cotidiano, ou seja, possibilitaram captar a realidade a partir da perspectiva do sujeito, um conhecimento imprescindivel para a elaboracao de uma proposta educacional.This study aimed at characterizing the profile of outpatients with aortic valve dysfunction, identifying their knowledge about the disease, their major limitations during everyday activities and their means of coping with their predicament. CRENÇAS ATITUDINAIS E NORMATIVAS DOS ENFERMEIROS SOBRE O ESTUDO HEMODINÂMICO POR MEIO DO CATETER DE ARTÉRIA PULMONAR
