Neurobiology of Exercise

Voluntary physical activity and exercise training can favorably influence brain plasticity by facilitating neurogenerative, neuroadaptive, and neuroprotective processes. At least some of the processes are mediated by neurotrophic factors. Motor skill training and regular exercise enhance executive functions of cognition and some types of learning, including motor learning in the spinal cord. These adaptations in the central nervous system have implications for the prevention and treatment of obesity, cancer, depression, the decline in cognition associated with aging, and neurological disorders such as Parkinsons disease, Alzheimers dementia, ischemic stroke, and head and spinal cord injury. Chronic voluntary physical activity also attenuates neural responses to stress in brain circuits responsible for regulating peripheral sympathetic activity, suggesting constraint on sympathetic responses to stress that could plausibly contribute to reductions in clinical disorders such as hypertension, heart failure, oxidative stress, and suppression of immunity. Mechanisms explaining these adaptations are not as yet known, but metabolic and neurochemical pathways among skeletal muscle, the spinal cord, and the brain offer plausible, testable mechanisms that might help explain effects of physical activity and exercise on the central nervous system.

Increasing physical activity: a quantitative synthesis

National policy for increasing leisure physical activity in the United States is impeded by a poor understanding of interventions that can be implemented by community and clinical medicine. To clarify the literature in this area, we conducted a quantitative, meta analysis of 127 studies that examined the efficacy of interventions for increasing physical activity among approximately 131,000 subjects in community, worksite, school, home, and health care settings; 445 effects were expressed as a Pearson correlation coefficient (r) and examined as they varied according to moderating variables important for community and clinical intervention. The mean effect was moderately large, r = 0.34, approximating three-fourths of a standard deviation or an increase in binomial success rate from 50% to 67%. The estimated population effect weighted by sample size was larger, r = 0.75, approximating 2 standard deviations or increased success to 88%. Contrasts between levels of independent moderating variables indicated that effects weighted by sample size were larger when the interventions: 1) employed the principles of behavior modification, 2) used a mediated delivery, 3) targeted groups, 4) of combined ages, 5) sampled apparently healthy people, or 6) measured active leisure, of 7) low intensity, 8) by observation. Independently of sample size, effects were larger when interventions 1) used behavior modification, 2) employed a preor quasi-experimental design, or 3) were of short duration, regardless of features of the people, setting, or physical activity. Our results show that physical activity can be increased by intervention. The optimal ways for selecting intervention components, settings, and population segments to maintain increases in physical activity and the relative contributions by community and clinical medicine toward successful physical activity intervention require experimental confirmation, warranting accelerated attention in clinical trials.

Measuring enjoyment of physical activity in adolescent girls

BACKGROUND Enjoyment has been implicated as a determinant of physical activity among youth, but advances in understanding its importance have been limited by the use of measures that were not adequately validated. The present study examined: (1) the factorial validity of the Physical Activity Enjoyment Scale (PACES), and (2) the construct validity of PACES scores. METHODS Adolescent girls (N=1797), who were randomly assigned to calibration (n=899) and cross-validation (n=898) samples, completed the PACES and measures of factors influencing enjoyment of physical education, physical activity, and sport involvement. The factorial validity of the PACES and the measure of factors influencing enjoyment of physical education was tested using exploratory and confirmatory factor analysis. The hypothesized relationships among the measures were tested using structural equation modeling. RESULTS Unidimensional models fit the PACES and the measure of factors influencing enjoyment of physical education in the calibration and cross-validation samples. The hypothesized relationships between the PACES and the measures of factors influencing enjoyment of physical education, physical activity, and sport involvement were supported in the entire sample, were similar in African-American and Caucasian girls, and were independent of physical fitness. CONCLUSIONS Evidence of factorial validity and convergent evidence for construct validity indicate that the PACES is a valid measure of physical activity enjoyment among adolescent girls, suitable for use as a mediator variable in interventions designed to increase physical activity.

Worksite physical activity interventions

BACKGROUND National objectives for public health have targeted worksite as important settings for interventions to increase physical activity. However, expert reviews reveal no scientific consensus about the effectiveness of worksite interventions for increasing physical activity or fitness. METHODS We judged the quantity and quality of existing evidence against scientific standards for the internal and external validity of the research design and the validity of measurements. Meta-analytic methods were used to quantify the size of effects expressed as Pearson correlation coefficients (r). Variation in effect was examined in relation to several features of the studies deemed important for implementing successful worksite interventions. Pre-experimental cohort studies were excluded because they are sensitive to secular trends in physical activity. RESULTS Twenty-six studies involving nearly 9,000 subjects yielded 45 effects. The mean effect was heterogeneous and small, r = 0.11 (95% CI, -0.20 to 0.40), approximating 1/4 S.D., or an increase in binomial success rate from 50% to 56%. Although effects varied slightly according to some of the study features we examined, effects were heterogeneous within levels of these features. Hence, the moderating variables examined did not explain variation in the effects (P > 0.05). The exception was that effects were smaller in randomized studies compared with studies using quasi-experimental designs (P < 0.05). CONCLUSIONS Our results indicate that the typical worksite intervention has yet to demonstrate a statistically significant increase in physical activity or fitness. The few studies that have used an exemplary sample, research design, and outcome measure have also yielded small or no effects. The generally poor scientific quality of the literature on this topic precludes the judgment that interventions at worksites cannot increase physical activity or fitness, but such an increase remains to be demonstrated by studies using valid research designs and measures.

Enjoyment mediates effects of a school-based physical-activity intervention.

PURPOSE The study evaluated whether targeted changes in factors influencing enjoyment of physical education (PE), physical activity enjoyment, and self-efficacy beliefs about participating in physical activity mediated the effect of the Lifestyle Education for Activity Program (LEAP) intervention on participation in physical activity. METHODS High schools (N=24) paired on enrollment size, racial composition, urban or rural location, and class structure were randomized into control (N=12) or experimental (N=12) groups. Of the 4044 girls enrolled and eligible, 2087 (51.6%) participated in the measurement component of the study. There were 1038 girls in the control group and 1049 girls in the experimental group. INTERVENTION LEAP was a comprehensive school-based intervention emphasizing changes in instruction and school environment designed to increase physical activity among black and white adolescent girls. It was organized according to the Coordinated School Health Program and included a PE component with core objectives of promoting enjoyment of PE, physical activity enjoyment, and self-efficacy. RESULTS Latent variable structural equation modeling indicated that: 1) the intervention had direct, positive effects on physical activity and factors influencing enjoyment of PE, which subsequently explained the effects of increased physical activity enjoyment and self-efficacy on increased physical activity; and 2) an additional, indirect effect of physical activity enjoyment on physical activity operated by an influence on self-efficacy. CONCLUSIONS Increases in enjoyment partially mediated the positive effect of the LEAP intervention. To our knowledge, we have provided the first experimental evidence from a randomized controlled trial linking increased enjoyment with increased physical activity among black and white adolescent girls.

Heart rate variability, trait anxiety, and perceived stress among physically fit men and women.

BACKGROUND It is unclear from prior reports whether the relationships between self-ratings of anxiety or emotional stress and parasympathetic nervous system components of heart rate variability are independent of personality and cardiorespiratory fitness. We examined those relationships in a clinical setting prior to a standardized exercise test. METHODS AND RESULTS Heart rate variability (HRV) was measured during 5 min of supine rest among 92 healthy men (N=52) and women (N=40) who had above-average cardiorespiratory fitness as indicated by peak oxygen uptake measured during grade-incremented treadmill exercise. HRV datasets were decomposed into low-frequency (LF; 0.05-0.15 Hz) and high-frequency (HF; 0.15-0.5 Hz) components using spectral analysis. Self-ratings of trait anxiety and perceived emotional stress during the past week were also assessed. CONCLUSIONS There was an inverse relationship between perceived emotional stress during the past week and the normalized HF component of HRV (P=0.038). This indicates a lower cardiac vagal component of HRV among men and women who perceived more stress. That relationship was independent of age, gender, trait anxiety, and cardiorespiratory fitness. It was also independent of heart rate; mean arterial blood pressure; and respiration rate, factors which can influence HRV and might be elevated among people reporting anxiety and perceived stress. We conclude that vagal modulation of heart period appears to be sensitive to the recent experience of persistent emotional stress, regardless of a persons level of physical fitness and disposition toward experiencing anxiety.

Promotion of Physical Activity Among High-School Girls: A Randomized Controlled Trial

OBJECTIVES Many adolescent girls fail to meet national guidelines for physical activity, and the prevalence of obesity is increasing among this group. Our study examined the effects of a comprehensive school-based intervention on physical activity among high-school girls. METHODS A group-randomized controlled field trial was conducted at 24 high schools. A school-based sample of 2744 girls (48.7% African American, 46.7% White) participated in a measurement protocol when they were in eighth and then ninth grade. A comprehensive physical activity intervention was designed to change the instructional program and the school environment to increase support for physical activity among girls. RESULTS At follow-up, 45% of girls in the intervention schools and 36% of girls in the control schools reported vigorous physical activity during an average of 1 or more 30-minute time blocks per day over a 3-day period. CONCLUSIONS A comprehensive school-based intervention can increase regular participation in vigorous physical activity among high-school girls.

Brain monoamines, exercise, and behavioral stress: animal models.

This paper summarizes our studies examining whether changes in levels of brain monoamines after chronic exercise are associated with altered behavioral and endocrine responses to stressors other than exercise. The focus is on using animal models relevant for understanding reports by humans that regular physical activity reduces depression and anxiety. We studied the effects of chronic activity wheel running or treadmill exercise training on levels of norepinephrine (NE) measured in brain cell bodies and terminal regions at rest and after behavioral stress. We also measured brain levels of serotonin, i.e., 5-hydroxytryptamine (5-HT), dopamine (DA), and gamma aminobutyric acid (GABA), which function as both antagonists and synergists with NE. In general, we found that chronic activity wheel running increased NE levels in the pons medulla at rest and protected against NE depletion in locus coeruleus cell bodies after footshock; the concomitant reduction in escape-latency was consistent with an antidepressant effect. Wheel running also decreased the density of GABAA receptors in the corpus striatum while increasing open-field locomotion, consistent with an anxiolytic effect, but had no effect on hypothalamic-pituitary-adrenal cortical response to footshock measured by plasma levels of adrenocorticotropic hormone (ACTH), corticosterone, and prolactin. In contrast, treadmill exercise training increased the metabolism of NE in brain ascending terminal areas for NE, increased the secretion of ACTH after footshock and immobilization stress and had no effect on GABAA receptor density or open field locomotion. The validity of animal models for studying depression and anxiety after forced versus voluntary exercise is discussed. Recommendations are offered for improving the methods used in this area of research.

Increasing and maintaining exercise and physical activity

The efficacy of exercise and physical activity for promoting health, coupled with the high prevalence of sedentariness in most industrialized nations, has implications for behavior therapy. Exercise and physical activity are frequently used as adjunctive or primary interventions in behavior therapy. However, adherence is usually poor in exercise programs. To clarify past research and facilitate future research and applications in behavior therapy, 56 studies that used behavior modification or cognitive-behavior modification to increase and maintain exercise and physical activity are critically reviewed. These studies show that increases in self-reported frequency of physical activity and attendance in exercise programs accompany all the interventions used. Other conclusions cannot be drawn, because most studies have limited internal or external validity due to their non- or quasi-experimental designs, or their use of indirect estimates of exercise, physical activity and fitness, or their failure to link increased exercise or activity with fitness or therapeutic outcomes. Thus, the effectiveness and generalizability of behavior change interventions with exercise and physical activity remain unclear. To illustrate theoretical approaches to changing exercise and physical activity, the Relapse Prevention Model and the Transtheoretical Model are discussed. Personal attributes, environments, and features of physical activity associated with the adoption and maintenance of exercise and physical activity are also discussed, because they can influence behavior therapy interventions. The timeliness of studying exercise and physical activity interventions in behavior therapy is reinforced by their designation by the U.S. Public Health Service as research priorities for the year 2000.
