Abstract. When a person smiles and talks, everyone in his or her own specific way, it can be noticed that the tip of the nose drops and the upper lip shortens, making the nasolabial angle more acute. The importance of the upper lip in the dynamics of the nose makes necessary a more detailed study of the morphology and anatomical relations of the median part of the nasal septum depressor muscle, contributing in this way to new techniques in plastic surgery. The authors proposed to study the anatomy of the nasal septum depressor muscle and its relationship with the dermocartilaginous ligament of Pitanguy and the upper lip, important components of the nasal dynamics of facial expression. In this way they seek to contribute to the treatment of the nasal tip in aesthetic rhinoplasties.

Abstract. During surgical procedures on the epidermis, either aesthetic or reconstructive, the medical literature shows that certain technical standards and skin flap proportions should be followed, unless the risk of necrosis increases, in different degrees, directly related to the trauma. This study evaluates the evolution of skin flaps on the dorsum of rats treated with a vitamin pool (VP) (vitamins A, E, and C). Twenty-six rats, divided into two groups, were used: (A) the treated group (13 specimens), in which we performed a dorsal skin flap and applied the VP, and (B) the control group (13 specimens), in which we performed a flap on the dorsum without application of the VP. Our results demonstrate that, at the macroscopic level, the group treated with the VP had more tissue sufferance, whereas at the microscopic level, small statistical differences were observed between the groups and also between the vertex and the middle of the flap. Despite these small differences, better results were obtained in group A, showing that in this study the clinical aspect did not correspond to the histological features.

Abstract. The analysis of the abdominal skin on cadavers has made it possible for the authors to evaluate the skins regional characteristics, taking into consideration the age group to be lipoaspirated regardless of race or sex. In this way, norms are established which in the future may be used to comparatively analyze results related to the cutaneous retraction after plastic surgery lipoaspiration.

Liposuction is a safe method for the treatment of lipodystrophy. It gives good results in relation to body contours, especially when the superficial and deep layers of the superficial fascia are aspirated. The authors present clinical cases of female patients who underwent liposuction of the abdomen, flanks, and back in which superficial and deep liposuction was used. In the immediate postoperative period, these patients presented a skin pattern of marbled appearance, involving rosy-purplish stains intermingled with other whitish stains on the skin in the areas subjected to surgery and resembling the cutis marmorata described in the literature. Even 1 year after the operation, the stains had not receded. The literature mentions cases of cutaneous necrosis provoked by a temperature increase induced by liposuction cannulas. This trauma said to be the determining factor for local lesions of the subdermal plexus. However, no cases involving lesions of this plexus attributable to mechanical trauma from cannulas are cited. According to several authors, it is important during superficial liposuction to maintain a strip about 1 cm thick under the deep dermis for the preservation of the arterial plexus of the skin. This would avoid the formation of a skin pattern resembling cutis marmorata.

Abstract. Analysis of the dorsums skin and sacrum region allowed the authors to evaluate regional characteristics of cutaneous skin integument, taking into consideration the age group most subjected to liposuction. Thus, it was attempted to establish rules to facilitate, in a comparative form, the evaluation of results related to cutaneous retraction in plastic surgery.

BACKGROUND: Much has been discussed about the various techniques and access routes in rhinoplasty. All seek a predictable and reproducible associated with improved aesthetic and functional nose. This study evaluated the level of satisfaction of patients undergoing rhinoplasty in the past five years: by endonasal reductional rhinoplasty associated or not to rhino dynamic. METHODS: Through a retrospective study identified 201 patients who underwent endonasal rhinoplasty the period between 2005 and 2010. All procedures were performed with the same routine as surgical indication. Were evaluated in an objective manner about the surgery and subjectively about their lung function before and after surgery and the degree of satisfaction with the nasal aesthetics through a questionnaire. RESULTS: The age ranged between 14 and 68 years of age; predominant among Caucasians (96.75%). 83.87% of the patients changed their minds after surgery, indicating improvement in breathing pattern. Regarding the nasal aesthetic, 30 (32.26%) patients had exceeded their expectations, 42 (45.16%) felt very good, 18 (19.36%) looked as good and three (3.22%) evaluated with lower than expected. CONCLUSION: The high degree of satisfaction with the aesthetics espoused by patients as well as lead to an improvement of respiratory function in 90% notes that the procedure performed has great result so far.

Congenital or developmental deformities of the chest rarely cause severe functional problems in the manner of larger anomalies of the limbs. Poland syndrome is known as a variety of associated anomalies centered on the trunk and upper limbs. The authors report the case of a 28-year-old female with absence of the pectoralis major muscle and depression of the left anterior thoracic wall (sunken chest). She presented with hypoplasia of the papillary-areolar complex; her upper left limb was normal. Seven years after undergoing placement of a left-side breast prosthesis, she requested replacement of the implant because she was dissatisfied with its shape. Chest radiographs showed a chest deformity and deviation of the mediastinum to the right. After surgically removing the prosthesis, an area of costal reabsorption on the left side was seen at its site, with exposure of the parietal pericardium. The prosthesis was replaced with another of greater volume and the patient was referred to the thoracic surgery service. No cases of regional bone resorption secondary to the presence of a breast prosthesis are reported in the literature. However, it is known that compression results in bone absorption and subsequent bone remodeling. The authors therefore believe that greater attention to this possibility is needed in cases of breast prosthesis replacement among patients with Poland syndrome, by means of specific imaging studies such as computed tomography.

The objective of this report was to describe a variation in the origin of the lateral internal thoracic artery (LITA), a variable large-caliber artery in the thoracic wall. This report presents a case in which a trunk coming from the subclavian artery (SCA) bifurcates and gives origin to the LITA and internal thoracic artery (ITA). This case demonstrates an unusual bilateral origin for the LITA, which emerges together with the ITA rather than directly from the SCA, as could be expected. Although such presentation is uncommon, the possibility that it could be damaged during surgical interventions such as thoracotomy and pleural drainage justifies our report .
