ABSTRACT Imipenem and meropenem Vitek 2 MICs were evaluated for a panel of 104 Enterobacteriaceae for identification of carbapenemase producers. The sensitivity and specificity values for the new CLSI interpretative criteria (CLSI document M100-S20-U, 2010) were 98% and 83% for imipenem and 76% and 83% for meropenem, respectively. We propose an algorithm that is highly sensitive (98%) and specific (94%) for carbapenemase screening based on the combined use of imipenem and meropenem MICs.

La Organizacion Mundial de la Salud ha puesto en marcha un programa para la vigilancia de la resistencia a los antibacterianos, denominado WHONET, que se desarrollo en Argentina mediante una red de 23 laboratorios de instituciones hospitalarias publicas y privadas someti-dos a programas nacionales e internacionales de control de calidad. Entre enero de 1995 y di-ciembre de 1996 se determino por el metodo de difusion en agar la sensibilidad a los antibacte-rianos de 16 073 aislados clinicos consecutivos, siguiendo las recomendaciones del Comite Nacional de Estandares para Laboratorios Clinicos (National Committee for Clinical Labo-ratory Standards:NCCLS) de los Estados Unidos de America. Mas de la mitad de los aisla-dos urinarios de Escherichia colifueron resistentes a la ampicilina, y mas de 30% a la trime-toprima-sulfametoxazol. Cuando se comparo la sensibilidad de los aislados urinarios de pacientes ambulatorios y hospitalizados, se observo una marcada diferencia en los perfiles de actividad (porcentaje de microorganismos resistentes aislados en pacientes hospitalizados frente a pacientes ambulatorios) de la gentamicina (8% frente a 2%), la norfloxacina (6% frente a 2%) y las cefalosporinas de tercera generacion (18% frente a 10%). Los aislados de Klebsiella pneumoniaerecuperados de hemocultivos presentaron resistencia a las cefalospo-rinas de tercera generacion y a la gentamicina en 71 y 60% de los casos, respectivamente. La proporcion de Staphylococcus aureusresistentes a la oxacilina fue de 39%. Cerca de la mitad de los aislados de Enterococcusspp. presentaron resistencia de alto nivel a los aminoglucosi-dos, pero no se detecto resistencia a los glicopeptidos. En nuestro medio, la ampicilina y la tri-metoprima-sulfametoxazol no fueron apropiadas para el tratamiento de las diarreas; Shigella flexneripresento mayor porcentaje de aislados resistentes a ambos farmacos (87 y 74%, res-pectivamente) que Shigella sonnei(47 y 71%, respectivamente). Se detecto resistencia a las cefalosporinas de tercera generacion en 40% de los aislados de Salmonella spp. recuperados en hospitales pediatricos. En casos de meningitis bacteriana, las tasas de resistencia de Strep-tococcus pneumoniaea la penicilina (18%) y de Haemophilus influenzaea la ampicilina (19%) se situaron en el rango intermedio de las descritas en paises americanos y europeos.

Streptococcus agalactiae is a significant worldwide cause of morbidity and mortality in pregnant women and their newborn infants. The objective of this work was to determine the usefulness of bioMerieux chromogenic medium chromID Strepto B (CR) for detecting S. agalactiae in pregnant women from the selective Todd-Hewitt broth (sel-THB ) against the methods proposed by the CDC . A total of 1924 swabs were analyzed, 962 from vaginal introitus and 962 rectal, belonging to 962 women in weeks 35-37 of pregnancy. The swabs were directly seeded in CR. Both swabs were later placed in sel-THB with 15 µg/ml supplement of nalidixic acid and 10 µg/ml colistin. After 24 h of incubation, subcultures in CR medium and agar containing 5% sheep blood (SBA) were performed. The prevalence found was 17.4%. Sensitivity, specificity, positive and negative predictive values of sel-THB subcultures with CR supplement and 48 h incubation were: 98.8, 100, 100 and 99.7%, respectively. The corresponding values of direct harvest of the sample were 57.8, 100, 100, and 90%, respectively. Sensitivity of sel-THB in SBA was 85%. Sel-THB subculture performance in CR was outstanding in comparison with the method proposed by the CDC.

This document contains the recommendations for antimicrobial susceptibility testing of the clinically relevant non-fermenting gram-negative bacilli (NFGNB), adopted after conforming those from international committees to the experience of the Antimicrobial Agents Subcommittee members and invited experts. This document includes an update on NFGNB classification and description, as well as some specific descriptions regarding natural or frequent antimicrobial resistance and a brief account of associated resistance mechanisms. These recommendations not only suggest the antimicrobial drugs to be evaluated in each case, but also provide an optimization of the disk diffusion layout and a selection of results to be reported. Finally, this document also includes a summary of the different methodological approaches that may be used for detection and confirmation of emerging b-lactamases, such as class A and B carbapenemases.
