The relationship between quality of life, sense of coherence and self-esteem in persons after coronary artery bypass graft surgery

Rosana Aparecida Spadoti Dantas; Sandra Adams Motzer; Marcia A. Ciol

We examined the contribution of Antonovskys sense of coherence in explaining the variance of quality of life (QOL) in 84 patients 1-2 years following coronary artery bypass graft surgery. The hypothesis was: after controlling for variables related to poor health vulnerability, perceived support, self-esteem, and chronic illness trajectory instability and work, the addition of sense of coherence will significantly add to the explained variance of quality of life. The first two variables explained 49% of the variance of the QOL scale. Adding perceived social support, self-esteem and sense of coherence increased explained variance to 64%, 69%, and 75%, respectively. These findings supported our hypothesis.

This cross-sectional study aimed to assess the quality of life of Brazilian HIV/AIDS carriers in a county in Sao Paulo and its association with socio-demographic variables and sexuality-related factors, using HIV/Aids Targeted Quality of Life (HATQoL). Of the 228 participants, 122 were men and 106 women, with an average age of 39 years. Different variables were associated to worse qualities of life, such as illiteracy, lower educational levels, and having no income or formal job. Concerning the variables related to sexuality, the worst QoL scores were associated to not having an active sexual life and having a partner with HIV/Aids. Living with HIV/Aids influences several domains of quality of life, requiring greater knowledge and comprehension of the different variables that can negatively influence the QoL of these individuals for a better care evaluation and delivery.

This study aimed to assess the cross-cultural adaptation of the Edmonton Frail Scale (EFS) and its validity in a Brazilian elderly sample. Translation and back-translation were performed, as well as discussion with professionals and elderly for conceptual equivalence, semantic validation and pre-test of the scale. The scale was applied to 137 elderly aged 65 years or older who lived in the community. In the know-groups validation of the frailty diagnosis between gender, age and cognitive deficit, elder elderly, female and with a cognitive deficit scored higher on the frailty diagnosis. A negative convergent correlation was found between the EFS and the Functional Independence Measure (FIM) (-0.53, p< 0.01) and the total score of the Mini-Mental State Examination (MMSE) (-0.60, p< 0.01). The watch test presented high sensitivity and low specificity levels. The Portuguese version of the EFS was considered valid in the study sample. BACKGROUND The Burns Specific Health Scale-Revised (BSHS-R) is of easy application, can be self-administered, and it is considered a good scale to evaluate various important life aspects of burn victims. OBJECTIVES To translate and culturally adapt the BSHS-R into the Brazilian-Portuguese language and to evaluate the internal consistency and convergent validity of the translated BSHS-R. METHODS The cultural adaptation of the BSHS-R included translation and back-translation, discussions with professionals and patients to ensure conceptual equivalence, semantic evaluation, and pre-test of the instrument. The Final Brazilian-Portuguese Version (FBPV) of the BSHS-R was tested on a group of 115 burn patients for internal consistency and validity of construct (using the Rosenberg Self-Esteem Scale (RSES) and the Beck Depression Inventory (BDI)). RESULTS All values of Cronbachs alpha were greater than .8, demonstrating that the internal consistency of the FBPV was very high. Self-esteem was highly correlated with affect and body image (r=.59, p<.001), and with interpersonal relationships (r=.51, p<.001). Correlations between the domains of the FBPV and the BDI were all negative but larger in magnitude than the correlations with RSES. Depression was highly correlated with affect and body image (r=-.77, p<.001), and with interpersonal relationships (r=-.67, p<.001). CONCLUSIONS The results showed that the adapted version of the BSHS-R into Brazilian-Portuguese fulfills the validity and reliability criteria required from an instrument of health status assessment for burn patients.

This study identified the nursing diagnoses of patients in the perioperative period of cardiac surgeries and verified the existence or not of agreement between the first author and other nurses in the identification of these diagnoses. Seventeen patients admitted for cardiac surgery were submitted to evaluation using instruments for the perioperative period. In the preoperative, out of the 9 diagnosis categories identified by the first author, three agreed with the nurses (example: Activity intolerance). In the trans-operative, seven categories showed agreement among the nurses and the first author (example: Risk for infection). In the post-operative, 11 categories showed agreement among the nurses and the first author (example: Risk for peripheral neurovascular dysfunction). This study identified the nursing diagnoses of patients in the perioperative period of cardiac surgeries and verified the existence or not of agreement between the first author and other nurses in the identification of these diagnoses. Psychometric properties of the Brazilian Portuguese versions of the 29‐ and 13‐item scales of the Antonovsky's Sense of Coherence (SOC‐29 and SOC‐13) evaluated in Brazilian cardiac patients

Rosana Aparecida Spadoti Dantas; Fernanda Souza Silva; Marcia A. Ciol

AIM AND OBJECTIVES To assess the construct validity and reliability of the Brazilian Portuguese versions of the 29- and 13-item scales of the Antonovskys Sense of Coherence Scale (SOC-29 and SOC-13). BACKGROUND Antonovskys Sense of Coherence Questionnaire was developed to measure the main construct of the salutogenic theory, the sense of coherence. The use of the sense of coherence by health professionals might help identifying specific coping strategies used by person with chronic illness. DESIGN Cross-sectional study. METHODS The SOC-29 has been culturally adapted to Brazilian Portuguese language. In this study, we assessed its psychometric properties (construct validity and internal reliability) tested on 203 cardiac patients. The SOC-13 was tested on another 100 cardiac patients hospitalised in the same hospital. The construct validity of both versions was investigated through Pearson correlation between the measures of sense of coherence and of correlated constructs (self-esteem, depression and age), analysis of the principal components and comparison of distinct groups (male vs. female). The internal reliability for both versions was evaluated by Cronbachs alpha. RESULTS For both versions, the construct validity assessments showed strong positive correlations between sense of coherence and self-esteem and strong negative correlations between sense of coherence and depression (convergent validity), and weak correlation between sense of coherence and age (divergent validity). Principal component analysis supported in part the presence of a single component (unidimensionality). Differences in sense of coherence were found by sex with SOC-29, but not with SOC-13. Cronbachs alpha coefficients showed satisfactory internal consistency in both versions. CONCLUSIONS The Brazilian Portuguese versions of SOC-29 and SOC-13 can be considered valid and reliable for the populations studied. More studies need to be performed to assess these proprieties in other Brazilian populations. RELEVANCE TO CLINICAL PRACTICE This scale can be used in research and clinical practice in Brazil to assess nursing interventions aimed at decreasing stress during cardiac rehabilitation.
