A man in late middle age presented with fever, systemic symptoms, raised inflammatory markers and anaemia of chronic disease. Despite two months of investigation, the diagnosis of a haematological malignancy was made only at autopsy.

1 year prior to diagnosis was negatively associated with OAC OR (95% CI) 0.56 (0.340.94) p=0.026 and total consumption both 1 and 10 years prior, 0.40 (0.22-0.74) p=0.004 and 0.42 (0.23-0.77) p=0.005 respectively. Total weekly potato consumption 1 year prior was associated with OAC 2.04 (1.07-3.88) p=0.03. Fruit, but not fruit juice, consumption was negatively associated with OAC for both time points (p<0.0001 unless stated): frequency 0.33 (0.21-0.52) 1 year and 0.44 (0.29-0.68) 10 years prior; weekly consumption 0.30 (0.16-0.54) 1 year and 0.34 (0.17-0.69) p=0.003 10 years prior. Red meat was associated with OAC 1 year prior to diagnosis: frequency 2.13 (1.22-3.73) p=0.008 and weekly consumption 2.78 (1.46-5.37) p=0.002. White meat and fish consumption showed no association with OAC. Tea, but not coffee, consumption (cups per day) was associated with OAC development: 2.07 (1.11-3.85) p=0.021 1 year and 2.38 (1.34-4.21) p=0.003 10 years prior to diagnosis. Discussion: Vegetable and more strikingly fruit consumption were negatively associated with OAC. Greater consumption of red meat and tea were positively associated with OAC. Dietary modification may offer opportunities to impact upon the increasing incidence of OAC
