Agnieszka Drosdzol; Violetta Skrzypulec; Ryszard Plinta

Aims. The aim of the study was to evaluate the influence of hirsutism on general quality of life, self-esteem and the prevalence of anxiety and depressive symptoms among adolescent girls. Methods. Fifty adolescent females with hirsutism, aged 13–18 years, were enrolled in the research group. The control group comprised 50 non-hirsute adolescents. A specific questionnaire was used as the research tool. It included self-evaluation inventories: Short Form-36 Health Survey Version 2, Hospital Anxiety and Depression Scale and Rosenberg Self-Esteem Scale. Results. Quality of life indices for hirsute girls scored lower than for the controls and statistically significantly so with regard to physical functioning (p = 0.04), general health (p = 0.002) and social functioning (p = 0.007). Anxiety was diagnosed in 26% in the group of hirsute girls as compared with 10% of the controls (p = 0.03). The study analysis revealed more clinically significant problems of low self-esteem in hirsute adolescents compared with non-hirsute girls (14% vs. 2%). Conclusions. Hirsutism is associated with a decreased quality of life, a higher prevalence of anxiety disorder and lower self-esteem in adolescent females. The mothers level of education is associated with the quality of life in adolescent girls.

Objectives To determine if there is a link between physical activity and sexual functioning in perimenopausal women, by comparing groups of women with low, moderate and high physical activity levels with regard to selected domains of their sexual functioning. Methods The research involved 400 healthy Polish women, aged 45–55 years. The final analysis encompassed 336 women who fulfilled all the inclusion criteria. The research tool was a two-part questionnaire. The first part covered the socio-economic status, patient history, and gynaecological-obstetric history. Part two consisted of an extended version of the International Physical Activity Questionnaire long form, evaluating the level of physical activity over the last seven days, and the Female Sexual Function Index. Results The mean age of the respondents was 50.56 ± 2.33 years. The analysis showed statistically significant differences between all the domains of physical activity and the FSFI of the studied women. In the group of women without sexual disorders, respondents with a low physical activity level in all questionnaire domains were the least numerous. Conclusions In perimenopausal women an association is seen between high levels of general physical activity and better sexual functioning.

INTRODUCTION The impact of premenstrual symptoms, such as the premenstrual syndrome (PMS) and the premenstrual dysphoric disorder (PMDD), on sexual satisfaction, sexual distress, and sexual behaviors has not yet been established. AIMS To assess the correlates and risk factors of sexual satisfaction and to evaluate sexual behaviors among Polish women with premenstrual symptoms. METHODS 2,500 females, aged 18 to 45 years, from the Upper Silesian region of Poland were eligible for the questionnaire-based, prospective population study. All the inclusion criteria were met by 1,540 women who constituted the final study group. The participants were further divided into two subgroups: PMS+ (749 females) and PMS- (791 healthy subjects). Two additional subgroups were created: PMDD+ encompassing 32 subjects diagnosed with PMDD, and PMDD- comprising 32 healthy women, matched to the PMDD+ females for age, marital status, education level, employment status, place of living, and body mass index. A multiple logistic regression analysis was performed to evaluate the influence of PMS on sexual satisfaction and adjust for potential confounders. MAIN OUTCOME MEASURE To evaluate risk factors for sexual dissatisfaction in a population of Polish females of reproductive age, diagnosed with PMS and PMDD. RESULTS Women from the PMS+ group were less sexually satisfied than PMS- (77.73% vs. 88.66%, P=0.001) and reported more sexual distress (28.65% vs. 15.24%, P=0.001). There were no significant differences in sexual satisfaction between PMDD- and PMDD+. Sexual satisfaction correlated positively with a higher frequency of sexual intercourses and a higher level of education. The presence of PMS correlated negatively with sexual satisfaction, even after adjusting for potential confounders in the multivariate logistic regression model (odds ratio=0.48; confidence interval: 0.26-0.89; P=0.02). CONCLUSIONS The presence of PMS is a risk factor for sexual dissatisfaction in Polish women of reproductive age.

BackgroundPremenstrual dysphoric disorder has multiple determinants in the biological, psychological and socio-cultural domains. The aim of the study was to evaluate the risk factors for premenstrual dysphoric disorder in Polish women, considering their reproductive history, socio-economic factors, as well as lifestyle and health-related factors.Methods2,500 females, aged 18 to 45, from the Upper Silesian region of Poland were eligible for the prospective population study. The final study sample was 1,540 individuals. The research was based on a questionnaire containing socio-economic status, general health, lifestyle, medical and reproductive history, premenstrual symptoms based on the American Psychiatric Associations criteria for diagnosing premenstrual dysphoric disorder, and patient prospective daily ratings of symptoms. The Statistica 8.0 computer software was used for statistical analysis. The value of p < 0.05 was adopted as the level of statistical significance.ResultsThe mean age of the studied population was 31.9 +/- 7.3 years. The majority of the studied women were married (57.9%), lived in large cities (42.0%) and had tertiary education (43.2%). The results of the study indicated that the prevalence of premenstrual dysphoric disorder was 2.1%. The final statistical analysis revealed that only tertiary education decreased the risk of premenstrual dysphoric disorder (OR = 0.08; p < 0.05).ConclusionsOur research showed that women with tertiary education are less vulnerable to premenstrual dysphoric disorder than women with a lower level of education. Reproductive and lifestyle factors seem to be play a lesser role.

Objective: The aim of the study was to assess the effect of a 3-month pre-season preparatory period and short-term moderate aerobic exercise and intensive fitness and speed exercise on adipokine and ghrelin levels in young female handball and basketball players. Materials and methods: Fifty healthy young female professional basketball and handball players were enrolled into the study before the opening of the season (after a 2-month holiday with no training or dietary restrictions). Serum estradiol and plasma leptin, adiponectin, visfatin, and ghrelin levels were determined at the beginning and the end of a 3-month period of moderate aerobic training. Plasma adipokines and ghrelin levels were additionally assessed after 2 h of moderate aerobic exercise or after intensive fitness and speed exercise training. Results: Long-term moderate aerobic exercise was followed by a significant decrease in plasma ghrelin and leptin levels (921 ±300 vs 575±572 pg/ml. p<0.001 and 16.4±15.6 vs 11.8±16.3 ng/ml, p<0.01, respectively); plasma adiponectin and visfatin remained unchanged. No changes were observed in plasma ghrelin and leptin levels after short-term moderate aerobic exercise or after intensive fitness and speed exercise. Plasma visfatin concentration increased significantly after short-term moderate aerobic exercise (22.1 ±8.7 vs 27.6±9.0 ng/ml, p<0.001), whilst adiponectin increased after intensive fitness and speed exercise (16.7±7.8 vs 21.0±9.8 µg/ml, p<0.001). Conclusions: Regular moderate aerobic training in preparation for the match season is followed by a decline in circulating leptin and ghrelin levels even in athletes without body weight changes. Short-term intensive fitness and moderate aerobic exercise seem to modulate the production of different adipokines.

Introduction Obesity is one of the most common nutrition-related disorders worldwide and one of the major health problems in menopausal women, which significantly worsens the quality of life and can lead to severe diseases or even premature death. The aim of the study was to evaluate the relation between physical activity in different domains undertaken by menopausal women while performing daily activities and their body mass index (BMI). Material and methods The study was conducted on a random sample of 400 healthy Polish women aged 45-55. The questionnaire contained questions concerning the respondents’ socio-economic status, medical history and the long format of the International Physical Activity Questionnaire was used. Results Fifty-one percent (50.6%) of women had normal body mass, 43.75% of respondents were overweighed and 5.65% of women were obese. A moderate physical activity level was presented by most midlife women in domestic and garden domain, active transportation and leisure time. Pearsons correlation showed that such factors as age, education and physical activity level in different domains have a significant correlation with BMI. Conclusions Most respondents presented a moderate physical activity level in all domains except work, where the respondents had a high physical activity (PA) level. A high PA level especially at work (r = –0.5788) and during leisure time (r = –0.5175) strongly correlate with lower BMI.

Conditions for preparations for the 2008 Beijing Paralympic Games in the opinion of the Polish national team Study aim: To evaluate the process of preparing Polish athletes with disabilities for the 2008 Beijing Summer Paralympic Games. Material and methods: The study included 31 women and 58 men representing 11 Paralympic disciplines, accounting for 97.8% of the Polish team taking part in the games. The average age was 32 years; the average amount of experience with sports training, 12 years. The method of diagnostic survey applied in the study was a questionnaire by J. Kłodecka-Różalska that was adapted to the needs of athletes with disabilities. The interpretation of the results was based on the arithmetic mean of individual evaluations. After checking the confidence interval, the factors were classified into one of three categories of conditions: satisfactory, sufficient, or unsatisfactory. Results: Data analysis revealed different levels of care and conditions created for the Polish national team during their preparations for the Beijing Paralympic Games. The athletes were fully satisfied with the conditions of the central camps and consultation (food, accommodation, sports equipment) and social relations (atmosphere of cooperation between the athletes and cooperation between the athletes and national team coaches of various disciplines). Issues requiring more professional attention are the health of athletes nominated to participate in the Paralympic Games (medical care and individual orthopaedic equipment) and contact with the media, which shapes the image of sport of the disabled people. Conclusions: In preparation for Paralympic Games, these athletes need to be provided with the constant and highlevel assistance of a psychologist and a nutritionist. In the perspective of future athletic goals set for the representatives of our country, a high standard of sports facilities, individual sports equipment, transport to the central camps and consultation, as well as professional care in the field of wellness, physiotherapy and massage, should be taken into account for Paralympic Game preparations.

Abstract The main purpose of this study was to determine sexuality of disabled athletes depending on the form of locomotion. The study included 170 disabled athletes, aged between 18 and 45. The entire population was divided into 3 research groups depending on the form of locomotion: moving on wheelchairs (n=52), on crutches (n=29) and unaided (n=89). The research tool was a questionnaire voluntarily and anonymously completed by the respondents of the research groups. The questionnaire was composed of a general part concerning the socio-demographic conditions, medical history, health problems, a part dedicated to physical disability as well as the Polish version of the International Index of Erectile Function (IIEF) and the Female Sexual Function Index (FSFI) evaluating sexual life. STATISTICA 10.0 for Windows was used in the statistical analysis. Subjects moving on crutches were significantly older than ones moving on wheelchairs and unaided (34.41 ±11.00 vs. 30.49 ±10.44 and 27.99 ±10.51 years, respectively) (p=0.018). Clinically significant erectile dysfunctions were most often diagnosed in athletes moving on wheelchairs (70.27%), followed by athletes moving on crutches and moving unaided (60% and 35.42%, respectively; p=0.048). Clinical sexual dysfunctions were diagnosed on a similar level among all female athletes. It was concluded that the form of locomotion may determine sexuality of disabled men. Males on wheelchair revealed the worst sexual functioning. Female athletes moving on wheelchairs, on crutches and moving unaided were comparable in the aspect of their sexual life.

Background Recent studies have reported many possibilities for the treatment of idiopathic toe walking (ITW); however, none of them have been sufficiently documented. The purpose of this case study was to document the evolution of the gait pattern of a child with severe ITW using the Gillette Gait Index before and after the third and sixth weeks, a nonsurgical treatment program and then every 3 months to 1 year from the start of the treatment. This is significant because the case study shows that a nonsurgical treatment program can be an alternative treatment method for children with severe ITW. Case description The case study involved a 5-year-old boy diagnosed with severe ITW. An orthopedist recommended a surgical treatment, but his parents refused to provide consent. Intervention The subject participated in a 12-week nonsurgical treatment program that used tone-inhibiting casts (TICs) combined with physiotherapy based on neurodevelopmental treatment principles. The treatment protocol included the following: 1) precast preparation; 2) TICs with treatment; and 3) post-cast treatment to improve the gait pattern. Outcomes After treatment with TICs, the range of motion of ankle dorsiflexion during stance had increased, resulting in an almost normalized gait. The patient stopped toe walking for at least 1 year. Discussion This study demonstrates that nonsurgical treatment should be considered first, with surgical options reserved for resistant cases; however, further research is required given the current lack of knowledge about treatment outcomes using TICs and the wide use of this treatment modality in children with ITW.
