Aneurysms of the pancreaticoduodenal arteries in association with celiac axis occlusion.

Four patients with aneurysms of the inferior pancreaticoduodenal artery are described. All had occlusion of the celiac axis at its origin, with the inferior pancreaticoduodenal arcades serving as a collateral pathway. We propose that the association of the celiac axis occlusion and aneurysms in the collateral supply via the pancreatic arcades is more than coincidental. Awareness of this relationship may be of significance in planning therapeutic intervention

[The use of computerized tomography in the detection of intra-orbital foreign bodies].

Use of the CT scanner as an instrument for foreign body detection and localization was explored under experimental conditions. The results show that a sufficiently dense radiopaque foreign body as small as 0.7mm3 can be detected and localized by presently available scanners.

The Role of Arteriography in the Management of Renal Cell Carcinoma Associated with Von Hippel-Lindau Disease

AbstractRenal cell carcinoma was detected in 5 patients with von Hippel-Lindau disease most accurately by magnification arteriography. At an operation aided by arteriography 80 to 100 per cent of the lesions, some measuring only a few millimeters, could be removed. The patients were followed by serial arteriography every 6 to 12 months to assess the size of remaining lesions and the development of new lesions. Re-exploration was considered if there was rapid growth or an increase in size of more than 1cm.

Intestinal volvulus: angiographic findings.

Intestinal volvulus may be the underlying process in patients presenting with intestinal ischemia. In 3 patients, angiographic findings of volvulus were erroneously interpreted as indicating occlusive or nonocclusive intestinal ischemia. In a fourth, the correct preoperative diagnosis was suggested by angiographic findings. Volvulus should be suspected at angiography by the whirlpool arrangement of the mesenteric arterial branches, the delayed filling and emptying of the arteries, and the intense opacification of the bowel wall. Familiarity with the angiographic picture of intestinal volvulus is essential for early diagnosis and for prompt initiation of appropriate therapy.

Brachiocephalic atherosclerosis: a cause of amaurosis fugax.

Atherosclerosis of the brachiocephalic artery is not generally recognized as a cause of amaurosis fugax. Five such cases are reported. Three patients underwent surgery and remain asymptomatic. One was lost to follow-up; another was receiving oral medication and remained asymptomatic for the 12-month follow-up period.

Catheter dislodgement: a cause of failure of intraarterial vasopressin infusions to control gastrointestinal bleeding.

Catheter dislodgement is a major cause of technical failures in intraarterial vasopressin therapy for gastrointestinal bleeding. Ten such cases were observed in the past five years. In seven patients catheter dislodgement led to recurrent bleeding during vasopressin infusion. In one patient aortic infusion of vasopressin caused recurrent bleeding and reversible acrocyanosis of the feet, and in two patients vasopressin infusion into the left renal artery resulted in chest pain and hematuria. Catheter dislodgement should be suspected if bleeding that was initially controlled recurs during vasopressin infusion.

Tolazoline-Augmented Arteriography in the Evaluation of Bone and Soft-Tissue Tumors

Twelve patients with bone tumors were prospectively evaluated by arteriography both with and without tolazoline. In general, tolazoline improved definition of tumor extent; however, in 2 patients with highly vascular tumors and 2 patients with hypovascular tumors, tolazoline did not improve visualization. In 3 of the 12 patients, the tolazoline-augmented study was less accurate than the nontolazoline study in defining tumor extent.
