Validity of the Ages and Stages Questionnaire to detect risk of Developmental Coordination Disorder in preschoolers.

BACKGROUND A reliable and valid screening tool for detecting children at risk for Developmental Coordination Disorder (DCD) is needed. The purpose of this study was to evaluate the ability of the Ages and Stages Questionnaire-Third Edition (ASQ-3) motor scales to detect children at risk for DCD in a community-based sample of children aged 3.5-5.5 years. METHODS One hundred and sixty parent-child pairs were recruited from community-based organizations. Children were eligible if they spoke English and had no known physical impairments. Eligible parents were asked to fill out the ASQ-3, following which their childs fine motor and gross motor proficiency was assessed using the Movement Assessment Battery for Children-Second Edition (MABC-2). DCD risk was defined as those children scoring at or below the 16th percentile on the MABC-2. Sensitivity and specificity of the fine and gross motor areas of the ASQ-3 were examined and referenced against the DCD risk classification. RESULTS The ASQ-3 total motor score correlated moderately with overall standard score on the MABC-2 (r = 0.41; p < 0.001). Regardless of the ASQ-3 cut-off used, sensitivities of the ASQ-3 fine motor or gross motor scale to detect DCD risk were low at 21-47%, whereas specificities were high at 89-96%. CONCLUSION Early identification of motor skill delays is important in order to intervene and hopefully prevent the associated negative health consequences. However, because of the low sensitivity of the ASQ-3 motor scales, these results suggest that the ASQ-3 is not an appropriate screening tool to identify children at risk for DCD in the preschool population.

Tracking cardiorespiratory fitness and physical activity in children with and without motor coordination problems

OBJECTIVES Previous research has shown children with Developmental Coordination Disorder (DCD) have lower cardiorespiratory fitness (CRF) than typically developing (TD) children. This has been hypothesized to be due to an activity deficit, whereby poor motor functioning discourages children from participating in physical activities, but this hypothesis has not been directly tested. In this study, we use longitudinal data to measure the extent to which physical activity explains differences in CRF between children with and without motor coordination deficits. DESIGN Longitudinal observational study. METHODS The study sample is an open cohort of children, numbering 2278 at baseline (age 9-10), that was followed for up to 5 years (to age 13-14). Motor skills were assessed once over the study period. Children scoring at or below the 5th percentile (n=103) on the Bruininks-Oseretsky Test of Motor Proficiency-Short Form were considered to have possible DCD (pDCD). CRF (estimated peak VO2) was estimated from performance on the Léger 20m shuttle run test, and physical activity was measured with the Participation Questionnaire. Both fitness and physical activity were measured up to 7 times over the study period. RESULTS Children with pDCD had significantly lower CRF than their TD peers at each time point. CRF declined for both groups, but this decline was steeper for children with pDCD. Physical activity explained only a small part of the difference in CRF. CONCLUSIONS The activity deficit did not contribute to the persistent and gradually widening gap in CRF between children with and without possible DCD. Possible reasons for this and future directions are discussed.

Co-occurring motor, language and emotional–behavioral problems in children 3–6 years of age

PURPOSE Developmental Coordination Disorder (DCD) has been shown to co-occur with behavioral and language problems in school-aged children, but little is known as to when these problems begin to emerge, or if they are inherent in children with DCD. The purpose of this study was to determine if deficits in language and emotional-behavioral problems are apparent in preschool-aged children with movement difficulties. METHOD Two hundred and fourteen children (mean age 4years 11months, SD 9.8months, 103 male) performed the Movement Assessment Battery for Children 2nd Edition (MABC-2). Children falling at or below the 16th percentile were classified as being at risk for movement difficulties (MD risk). Auditory comprehension and expressive communication were examined using the Preschool Language Scales 4th Edition (PLS-4). Parent-reported emotional and behavioral problems were assessed using the Child Behavior Checklist (CBCL). RESULTS Preschool children with diminished motor coordination (n=37) were found to have lower language scores, higher externalizing behaviors in the form of increased aggression, as well as increased withdrawn and other behavior symptoms compared with their typically developing peers. CONCLUSIONS Motor coordination, language and emotional-behavioral difficulties tend to co-occur in young children aged 3-6years. These results highlight the need for early intervention.

Longitudinal examination of objectively-measured physical activity and sedentary time among children with and without significant movement impairments.

BACKGROUND Children with Developmental Coordination Disorder (DCD) tend to be less active than typically-developing (TD) children. Current evidence, however, is based on cross-sectional and self-reported activity, and little is known about sedentary time among children with significant movement impairments such as DCD. The current study examines the longitudinal patterns of objectively measured physical activity and sedentary time in children with and without possible DCD (pDCD). METHODS Data is from a longitudinal nested case-control study, with 103 participants (n=60 males ages=12 and 13 at baseline). Participants averaging ⩽16th percentile on the Movement Assessment Battery for Children were considered having significant movement impairments and pDCD (n=49). All participants wore accelerometers for seven days. RESULTS There were significant main effects for time (Estimate=-23.98, p<.01) and gender (Estimate=59.86, p<.05) on total physical activity, and time spent being sedentary (Estimate=15.58, p<.05). Significant main effects for pDCD (Estimate=-5.38, p<.05) and gender (Estimate=26.89, p<.01), and time by gender interaction (Estimate=-7.50, p<.05) were found for moderate-to-vigorous physical activity (MVPA). Sedentary time did not differ between children with and without DCD. CONCLUSIONS Results suggest children with pDCD engaged in less MVPA compared to TD children. Consistent patterns of MVPA over time, however, suggest that the divergence in MVPA occurs earlier in childhood. Further longitudinal research following a younger cohort is necessary to identify the specific point that differences in MVPA emerge.

Validity of field assessments to predict peak muscle power in preschoolers

Field-based fitness assessments are time- and cost-efficient. However, no studies to date have reported the predictive value of field-based musculoskeletal fitness assessments in preschoolers. The purpose of this study was to determine the validity of 2 field assessments to predict peak muscle power in preschool-aged children. Four-hundred and nineteen 3- to 5-year olds participated (208 girls, 211 boys; mean age: 4.5 ± 0.9 years). Peak power (PP) was evaluated using a modified 10-s Wingate protocol as the criterion standard. Standing long-jump was measured in inches to the back of the heel using a 2-footed takeoff and landing. Shuttle-run time was measured using a shuttle-run protocol, which required children to sprint 50 feet (15.2 m), pick up a small block, and sprint back, with time measured to the closest tenth of a second. Regression modelling was used to calculate the predictive power of each field-based measurement, adjusting for weight (kg), age, and sex. Both standing long-jump distance and shuttle-run time were significantly correlated with PP (r = 0.636, p < 0.001, and r = -0.684, p < 0.001, respectively). Linear regression analysis determined that a childs PP can be predicted from the childs weight, age, and sex and either standing long-jump or shuttle-run time (adjusted R2 = 0.79, p < 0.001, and 0.81, p < 0.001, respectively). The standing long-jump and the Bruininks Oseretsky Test of Motor Proficiency 2nd Edition shuttle-run are both significant predictors of peak muscle power in preschool children. Either measure can be used as a cost- and time-efficient estimate of musculoskeletal fitness in preschoolers.

Tracking of physical activity and fitness during the early years

The early years are characterized by rapid physical growth and the development of behaviours such as physical activity. The objectives of this study were to assess the 12-month changes in and the tracking of physical activity and fitness in 400 preschoolers (201 boys, 4.5 ± 0.9 years of age). Physical activity data, expressed as minutes per day and as the percentage of time spent at various intensities while wearing an accelerometer, were collected in 3-s epochs for 7 days. Short-term muscle power, assessed with a 10-s modified Wingate Anaerobic Test, was expressed as absolute (W) and relative (W/kg) peak power (PP) and mean power (MP). Aerobic fitness, assessed with the Bruce Protocol progressive treadmill test, was expressed as maximal treadmill time and heart rate recovery (HRR). Light physical activity decreased by 3.2 min/day (p < 0.05), whereas vigorous physical activity increased by 3.7 min/day (p < 0.001), from year 1 to year 2. Physical activity exhibited moderate tracking on the basis of Spearman correlations (r = 0.45-0.59, p < 0.001) and fair tracking on the basis of κ statistics (κ = 0.26-0.38). PP and MP increased from year 1 (PP, 94.1 ± 37.3 W; MP, 84.1 ± 30.9 W) to year 2 (PP, 125.6 ± 36.2 W; MP, 112.3 ± 32.2 W) (p < 0.001) and tracked moderately to substantially (PP, r = 0.89, κ = 0.61; MP, r = 0.86, κ = 0.56). Time to exhaustion on the treadmill increased from 9.4 ± 2.3 min to 11.8 ± 2.3 min (p < 0.001) and tracked strongly (r = 0.82, κ = 0.56). HRR was unchanged at 65 ± 14 beats/min (p = 0.297) and tracked fairly (r = 0.52, κ = 0.23). The findings indicate that fitness tracks better than physical activity over a 12-month period during the early years.

The longitudinal relationship between generalized self-efficacy and physical activity in school-aged children

Abstract Our understanding of the longitudinal relationship between generalized self-efficacy (GSE) and physical activity in children and youth is limited. The purpose of this study was to investigate the effect of GSE towards physical activity on sedentary behaviours and physical activity in school-aged children over time. A total of 2278 nine-year-old children (1120 girls and 1158 boys) were recruited at baseline and followed for seven waves of data collection from 2005 to 2008. All children completed questionnaires at each wave assessing their GSE (adequacy, predilection, and enjoyment), sedentary behaviours, free play, and organized activity. Mixed-effects models were used to estimate changes in physical activity and GSE within individuals over time, controlling for gender and motor ability. The results showed that participation in free play significantly increased over time, whereas organized activity significantly decreased over the same period. Children with high perceived adequacy and predilection had higher free play and organized activity participation relative to other children over time. However, the effect of perceived adequacy diminished over time, while the gaps between groups with different levels of predilection widened over time. While sedentary behaviours were lower over time in children with high predilection, these behaviours were consistently higher in children with high enjoyment. The differences in sedentary behaviours between groups increased over time for both predilection and enjoyment. This study highlights the importance of different components of GSE on physical activity participation. In addition, interventions targeting the enhancement of predilection may facilitate physical activity and reduce sedentary behaviours.

MovingU: A prospective cohort study to understand behavioural and environmental contexts influencing physical activity during the transition into emerging adulthood

BackgroundChildren and youth are often considered the most active segment of the population, however, research indicates that physical activity (PA) tends to peak during the adolescent years, declining thereafter with age. In particular, the acute transition out of high school is a period for which individuals appear to be at high-risk for becoming less active. Relatively few studies have investigated the factors influencing the changes in PA during this transition period. Therefore the purpose of the MovingU study is to gain a comprehensive understanding of the behavioural patterns and the socio-ecological factors related to the changes in PA during the transition out of high school.Methods/DesignMovingU is comprised of two phases. Phase I is a prospective cohort design and aims to follow 120 students in their last year of high school through to their first year out of high school. Students will be asked to complete questionnaires measuring various psychosocial and socio-environmental variables (e.g., self-efficacy and distress) four times throughout this transition period. Students will also be given a wrist-worn accelerometer to wear for 7-days at each of the four assessments. Phase II is a cross-sectional study involving 100 first-year university students. Students will be asked to complete the same questionnaire from phase I, wear a wrist-worn accelerometer for 5-days, and complete ecological momentary assessments (EMA) using their smartphones at randomly selected times throughout the day for 5-days. EMA items will capture information regarding contextual and momentary correlates of PA.DiscussionThe MovingU study represents the first to evaluate the social and environmental influences of PA behaviour changes, including the use of intensive real-time data capture strategies during the transition out of high school. This information will be critical in the development of interventions aimed to prevent or attenuate such drastic declines in PA during emerging adulthood period.
