How many steps/day are enough? for children and adolescents

Worldwide, public health physical activity guidelines include special emphasis on populations of children (typically 6-11 years) and adolescents (typically 12-19 years). Existing guidelines are commonly expressed in terms of frequency, time, and intensity of behaviour. However, the simple step output from both accelerometers and pedometers is gaining increased credibility in research and practice as a reasonable approximation of daily ambulatory physical activity volume. Therefore, the purpose of this article is to review existing child and adolescent objectively monitored step-defined physical activity literature to provide researchers, practitioners, and lay people who use accelerometers and pedometers with evidence-based translations of these public health guidelines in terms of steps/day. In terms of normative data (i.e., expected values), the updated international literature indicates that we can expect 1) among children, boys to average 12,000 to 16,000 steps/day and girls to average 10,000 to 13,000 steps/day; and, 2) adolescents to steadily decrease steps/day until approximately 8,000-9,000 steps/day are observed in 18-year olds. Controlled studies of cadence show that continuous MVPA walking produces 3,300-3,500 steps in 30 minutes or 6,600-7,000 steps in 60 minutes in 10-15 year olds. Limited evidence suggests that a total daily physical activity volume of 10,000-14,000 steps/day is associated with 60-100 minutes of MVPA in preschool children (approximately 4-6 years of age). Across studies, 60 minutes of MVPA in primary/elementary school children appears to be achieved, on average, within a total volume of 13,000 to 15,000 steps/day in boys and 11,000 to 12,000 steps/day in girls. For adolescents (both boys and girls), 10,000 to 11,700 may be associated with 60 minutes of MVPA. Translations of time- and intensity-based guidelines may be higher than existing normative data (e.g., in adolescents) and therefore will be more difficult to achieve (but not impossible nor contraindicated). Recommendations are preliminary and further research is needed to confirm and extend values for measured cadences, associated speeds, and MET values in young people; continue to accumulate normative data (expected values) for both steps/day and MVPA across ages and populations; and, conduct longitudinal and intervention studies in children and adolescents required to inform the shape of step-defined physical activity dose-response curves associated with various health parameters.

Using Global Positioning Systems in Health Research A Practical Approach to Data Collection and Processing

The use of GPS devices in health research is increasingly popular. There are currently no best-practice guidelines for collecting, processing, and analyzing GPS data. The standardization of data collection and processing procedures will improve data quality, allow more-meaningful comparisons across studies and populations, and advance this field more rapidly. This paper aims to take researchers, who are considering using GPS devices in their research, through device-selection criteria, device settings, participant data collection, data cleaning, data processing, and integration of data into GIS. Recommendations are outlined for each stage of data collection and analysis and indicates challenges that should be considered. This paper highlights the benefits of collecting GPS data over traditional self-report or estimated exposure measures. Information presented here will allow researchers to make an informed decision about incorporating this readily available technology into their studies. This work reflects the state of the art in 2011.

GPS tracking in neighborhood and health studies: A step forward for environmental exposure assessment, a step backward for causal inference?

Recent studies have relied on GPS tracking to assess exposure to environmental characteristics over daily life schedules. Combining GPS and GIS allows for advances in environmental exposure assessment. However, biases related to selective daily mobility preclude assessment of environmental effects, to the extent that these studies may represent a step backward in terms of assessment of causal effects. A solution may be to integrate the Public health / Nutrition approach and the Transportation approach to GPS studies, so as to combine a GPS and accelerometer data collection with an electronic mobility survey. Correcting exposure measures and improving study designs with this approach may permit mitigating biases related to selective daily mobility.

Dynamic Accuracy of GPS Receivers for Use in Health Research: A Novel Method to Assess GPS Accuracy in Real-World Settings.

The emergence of portable global positioning system (GPS) receivers over the last 10 years has provided researchers with a means to objectively assess spatial position in free-living conditions. However, the use of GPS in free-living conditions is not without challenges and the aim of this study was to test the dynamic accuracy of a portable GPS device under real-world environmental conditions, for four modes of transport, and using three data collection intervals. We selected four routes on different bearings, passing through a variation of environmental conditions in the City of Copenhagen, Denmark, to test the dynamic accuracy of the Qstarz BT-Q1000XT GPS device. Each route consisted of a walk, bicycle, and vehicle lane in each direction. The actual width of each walking, cycling, and vehicle lane was digitized as accurately as possible using ultra-high-resolution aerial photographs as background. For each trip, we calculated the percentage that actually fell within the lane polygon, and within the 2.5, 5, and 10 m buffers respectively, as well as the mean and median error in meters. Our results showed that 49.6% of all ≈68,000 GPS points fell within 2.5 m of the expected location, 78.7% fell within 10 m and the median error was 2.9 m. The median error during walking trips was 3.9, 2.0 m for bicycle trips, 1.5 m for bus, and 0.5 m for car. The different area types showed considerable variation in the median error: 0.7 m in open areas, 2.6 m in half-open areas, and 5.2 m in urban canyons. The dynamic spatial accuracy of the tested device is not perfect, but we feel that it is within acceptable limits for larger population studies. Longer recording periods, for a larger population are likely to reduce the potentially negative effects of measurement inaccuracy. Furthermore, special care should be taken when the environment in which the study takes place could compromise the GPS signal.

Modifiable risk factors for overweight and obesity in children and adolescents from São Paulo, Brazil.

BackgroundBrazil is currently experiencing a nutrition transition: the displacement of traditional diets with foods high in saturated fat, sodium, and cholesterol and an increase in sedentary lifestyles. Despite these trends, our understanding of child obesity in Brazil is limited. Thus, the aims of this study were (1) to investigate the current prevalence of overweight and obesity in a large sample of children and adolescents living in São Paulo, Brazil, and (2) to identify the lifestyle behaviors associated with an increased risk of obesity in young Brazilians.MethodsA total of 3,397 children and adolescents (1,596 male) aged 7-18 years were randomly selected from 22 schools in São Paulo, Brazil. Participants were classified as normal weight, overweight, or obese based on international age- and sex-specific body mass index thresholds. Selected sociodemographic, physical activity, and nutrition behaviors were assessed via questionnaire.ResultsOverall, 19.4% of boys and 16.1% of girls were overweight while 8.9% and 4.3% were obese. Two-way analysis of variance revealed that the prevalence of overweight and obesity was significantly higher in boys and in younger children when compared to girls and older children, respectively (P < 0.05 for both). Logistic regression analysis revealed that overweight was associated with more computer usage, parental encouragement to be active, and light soft drink consumption after controlling for differences in sex, age, and parental education (P < 0.05 for all). Conversely, overweight was associated with less active transport to school, eating before sleep, and consumption of breakfast, full-sugar soft drinks, fried food and confectionery (P < 0.05 for all).ConclusionsOur results show that obesity in São Paulo children and adolescents has reached a level equivalent to that seen in many developed countries. We have also identified three key modifiable factors related to obesity that may be appropriate targets for future intervention in Brazilian youth: transport mode to school, computer usage, and breakfast consumption.

Portable global positioning system receivers: static validity and environmental conditions

BACKGROUND GPS receivers are becoming increasingly common as an objective measure of spatiotemporal movement in free-living populations; however, research into the effects of the surrounding physical environment on the accuracy of off-the-shelf GPS receivers is limited. PURPOSE The goal of the current study was to (1) determine the static validity of seven portable GPS receiver models under diverse environmental conditions and (2) compare the battery life and signal acquisition times among the models. METHODS Seven GPS models (three units of each) were placed on six geodetic sites subject to a variety of environmental conditions (e.g., open sky, high-rise buildings) on three separate occasions. The observed signal acquisition time and battery life of each unit were compared to advertised specifications. Data were collected and analyzed in June 2012. RESULTS Substantial variation in positional error was observed among the seven GPS models, ranging from 12.1 ± 19.6 m to 58.8 ± 393.2 m when averaged across the three test periods and six geodetic sites. Further, mean error varied considerably among sites: the lowest error occurred at the site under open sky (7.3 ± 27.7 m), with the highest error at the site situated between high-rise buildings (59.2 ± 99.2 m). While observed signal acquisition times were generally longer than advertised, the differences between observed and advertised battery life were less pronounced. CONCLUSIONS Results indicate that portable GPS receivers are able to accurately monitor static spatial location in unobstructed but not obstructed conditions. It also was observed that signal acquisition times were generally underestimated in advertised specifications.

Active transportation and public transportation use to achieve physical activity recommendations? A combined GPS, accelerometer, and mobility survey study

BackgroundAccurate information is lacking on the extent of transportation as a source of physical activity, on the physical activity gains from public transportation use, and on the extent to which population shifts in the use of transportation modes could increase the percentage of people reaching official physical activity recommendations.MethodsIn 2012-2013, 234 participants of the RECORD GPS Study (French Paris region, median age = 58) wore a portable GPS receiver and an accelerometer for 7 consecutive days and completed a 7-day GPS-based mobility survey (participation rate = 57.1%). Information on transportation modes and accelerometry data aggregated at the trip level [number of steps taken, energy expended, moderate to vigorous physical activity (MVPA), and sedentary time] were available for 7,644 trips. Associations between transportation modes and accelerometer-derived physical activity were estimated at the trip level with multilevel linear models.ResultsParticipants spent a median of 1 h 58 min per day in transportation (8.2% of total time). Thirty-eight per-cent of steps taken, 31% of energy expended, and 33% of MVPA over 7 days were attributable to transportation. Walking and biking trips but also public transportation trips with all four transit modes examined were associated with greater steps, MVPA, and energy expenditure when compared to trips by personal motorized vehicle. Two simulated scenarios, implying a shift of approximately 14% and 33% of all motorized trips to public transportation or walking, were associated with a predicted 6 point and 13 point increase in the percentage of participants achieving the current physical activity recommendation.ConclusionsCollecting data with GPS receivers, accelerometers, and a GPS-based electronic mobility survey of activities and transportation modes allowed us to investigate relationships between transportation modes and physical activity at the trip level. Our findings suggest that an increase in active transportation participation and public transportation use may have substantial impacts on the percentage of people achieving physical activity recommendations.

Active commuting to school in New Zealand children (2004-2008): a quantitative analysis.

OBJECTIVE To describe trends in active commuting to school in children from Auckland region, New Zealand following implementation of the School Travel Plan (STP) program. The program included educational initiatives, enforcement activities and urban form changes around school environments. METHODS Active commuting to school was estimated for 57,096 students, aged 5-10 years, from 56 elementary schools. Data were drawn from surveys conducted between 2004 and 2008. RESULTS There was an overall increase in active commuting by the third year of STP implementation when compared to baseline levels (40.5% to 42.2%), [OR = 2.65, 95% CI = 1.75-4.02]. Students of high socio-economic background improved (38.9% to 39.1%) compared to those from mid [OR=0.66, 95% CI=0.82-1.01] and low [OR = 0.47, 95% CI = 0.32-0.68]. STP appealed to senior (43.6% to 44.3%), [OR = 1.33, 95% CI = 1.15-1.53] when compared to junior elementary students. The STP was equally effective in schools with small to medium student rolls compared to large schools (34.1% to 36.5%), [OR=1.33, 95% CI = 1.09-1.63]. Schools in Auckland city (47.2% to 48.1%), [OR = 1.43, 95% CI = 1.20-1.70] showed significant improvements when compared to schools from North Shore City. CONCLUSION The STP was associated with increased active commuting in children after three years of implementation.

A Review of Adolescent High-Intensity Interval Training

Despite the promising evidence supporting positive effects of high-intensity interval training (HIIT) on the metabolic profile in adults, there is limited research targeting adolescents. Given the rising burden of chronic disease, it is essential to implement strategies to improve the cardiometabolic health in adolescence, as this is a key stage in the development of healthy lifestyle behaviours. This narrative review summarises evidence of the relative efficacy of HIIT regarding the metabolic health of adolescents. Methodological inconsistencies confound our ability to draw conclusions; however, there is meaningful evidence supporting HIIT as a potentially efficacious exercise modality for use in the adolescent cohort. Future research must examine the effects of various HIIT protocols to determine the optimum strategy to deliver cardiometabolic health benefits. Researchers should explicitly show between-group differences for HIIT intervention and steady-state exercise or control groups, as the magnitude of difference between HIIT and other exercise modalities is of key interest to public health. There is scope for research to examine the palatability of HIIT as an exercise modality for adolescents through investigating perceived enjoyment during and after HIIT, and consequent long-term exercise adherence.

Neighborhood environments, mobility, and health: Towards a new generation of studies in environmental health research

While public policies seek to promote active transportation, there is a lack of information on the social and environmental factors associated with the adoption of active transportation modes. Moreover, despite the consensus on the importance of identifying obesogenic environmental factors, most published studies only take into account residential neighborhoods in the definition of exposures. There are at least three major reasons for incorporating daily mobility in public health research: (i) to identify specific population groups, including socially disadvantaged populations, who experience mobility or spatial accessibility deficits; (ii) to study the environmental determinants of transportation habits and investigate the complex relationships between transportation (as a source of physical activity, pollutants, and accidents) and physical activity and health; and (iii) to improve the assessment of spatial accessibility to resources and exposure to environmental hazards by accounting for daily trajectories for a better understanding of their health effects. There is urgent need to develop novel methods to better assess daily mobility. The RECORD Study relies on (i) an electronic survey of regular mobility to assess the chronic exposure to environmental conditions over a relatively long period, and (ii) Global Positioning System tracking to evaluate precisely acute environmental exposures over a much shorter period. The present article argues that future research should combine these two approaches. Gathering scientific evidence on the relationships between the environments, mobility/transportation, and health should allow public health and urban planning decision makers to better take into account the individual and environmental barriers to the adoption of active transportation and to define innovative intervention strategies addressing obesogenic environments to reduce disparities in excess weight.
