Arthroscopic refixation of a combined fracture of the greater and lesser tuberosity of the proximal humerus

Combined fractures of the greater and the lesser tuberosity are very rare injuries. This is a case report on arthroscopic treatment using suture anchors and one distal fixation screw. Clinical and radiographic follow-up showed recovery of active motion and consolidation of the fragments in anatomic positions. Level of evidence V.

ZusammenfassungDie Schultergelenkinstabilität ist ein Phänomen mit einer Vielzahl klinischer Ausprägungsformen. Das Spektrum reicht von der schmerzhaften Hyperlaxität bis hin zur chronisch verhakten Schulterluxation. Die Diagnostik und Klassifikation stellen die Grundlage jeder weiteren therapeutischen Intervention dar. Richtig angewandt erlauben die verschiedenen klinischen Untersuchungsmethoden in Kombination mit der entsprechenden bildgebenden Diagnostik in der Mehrzahl der Fälle eine Einteilung in verschiedene Klassifikationssysteme und somit auch die Planung eines Therapieregimes.AbstractShoulder instability includes a vast spectrum of different manifestations ranging from painful hyperlaxity to chronic locked shoulder dislocations. A correct diagnosis and classification is essential to establish an adequate treatment strategy. The correct use of different clinical tests and signs in combination with the corresponding imaging diagnostics allows an explicit classification and therefore the correct choice of treatment regimes in the majority of cases.
