Indicadores da institucionalização de idosos: estudo de casos e controles

OBJECTIVE To identify predictive indicators of institutionalization of the elderly. METHODS A case-control study was carried out with 991 elderly individuals in the city of Pelotas, (Southern Brazil), from 2007 to 2008. The cases of institutionalized elderly adults (n = 393) were detected using a census of all long-stay institutions for the elderly in the city. The population controls (n = 598) were randomly selected using a comprehensive health survey. Pearsons chi-square test and linear trends were used to compare groups in the crude analysis; and the binary logistic regression model of the adjusted analysis, with the effects expressed as odds ratios. RESULTS Institutionalization was more frequent in females (OR = 1.96, 95%CI 1.31, 2.95). Elderly with advanced age (OR = 3.23 and OR = 9.56 for age groups 70-79 and > 80 years, respectively), those who lived without a partner (single, divorced or widowed), and those who had no formal schooling or had a functional disability preventing them from performing basic activities for daily living were more likely to be institutionalized. An inverse trend between the incidence of elderly institutionalization and the level of physical activity was observed, where somewhat active and inactive subjects were more likely to be institutionalized (OR = 1.71 and OR = 4.73, respectively). CONCLUSIONS Of the factors examined, age > 80 years, living without a partner and being physically inactive were the indicators most strongly associated with institutionalization. The encouragement of informal care through cultural and educational activities focused on the role of the family in caring for the elderly can prevent the institutionalization of these individuals.OBJETIVO: Identificar indicadores de la institucionalizacion de ancianos. En la comparacion de los grupos, se emplearon las pruebas chi-cuadrado de Pearson y tendencia linear en el analisis bruto y el modelo de regresion logistico binario en el analisis ajustado, con medidas de efecto expresadas en odds ratio (OR). RESULTS: La institucionalizacion fue mas frecuente en el sexo femenino (OR=1,96; IC95% 1,31;2,95). Ancianos con edad avanzada (OR=3,23 y OR=9,56) para grupos etareos de 70-79 y > 80 anos, respectivamente), que vivian sin companero (solteros, separados y viudos), no poseian escolaridad formal y presentaban incapacidad funcional para actividades basicas de la vida diaria, presentaron mayor probabilidad de institucionalizacion. Se observo tendencia inversa entre la ocurrencia de institucionalizacion del anciano y el nivel de actividad fisica, donde sujetos poco activos e inactivos presentaron mayores probabilidades de institucionalizacion (OR=1,71 y OR=4,73, respectivamente). CONCLUSIONS: Entre todas las caracteristicas investigadas en los ancianos, edad > 80 anos, vivir sin companero y ser fisicamente inactivo fueron los indicadores mas fuertemente asociados a la ocurrencia de institucionalizacion. El incentivo al cuidado informal, a partir de acciones educativas y culturales enfocadas en el papel de la familia para el anciano puede impedir la institucionalizacion de esos individuos.

Lifestyle, self-reported morbidities, and poor sleep quality among Brazilian workers

OBJECTIVE To examine perceived poor sleep quality and its association with unhealthy lifestyle and self-reported morbidities among Brazilian workers. METHODS This study was part of a national survey involving 47,477 workers who received a questionnaire about lifestyle. Chi-square tests and logistic regression (OR=Odds Ratio) were conducted. RESULTS Poor sleep quality was reported by 21% of workers and was more prevalent among women (p<0.01). After being adjusted for socio-demographic indicators, negative perception of health, well-being, stress, self-reported morbidities, physical inactivity during leisure-time (OR: 1.18) in both genders, and inadequate fruit and vegetable intake (OR(men): 1.15; OR(women): 1.17), as well as excessive alcohol consumption (OR: 1.15) in men remained associated with poor sleep quality. With respect to morbidities, only diabetes in women (OR: 1.46) and hypertension in men (OR: 1.18) were associated with poor sleep quality. CONCLUSIONS Our findings indicate that certain unhealthy behaviors and self-reported morbidities were associated with poor sleep quality. Efficient initiatives should be implemented to promote a healthier lifestyle among workers, which could reduce or control glucose levels and blood pressure in addition to improving perceived sleep quality.

Deslocamento para o trabalho e fatores associados em industriários do sul do Brasil

Cross-sectional study that aimed to estimate the prevalence of forms of commuting to and from work and to identify factors associated among industrial workers in the State of Rio Grande do Sul, southern Brazil. A total of 2,265 workers completed a questionnaire on the forms of commuting to and from work (walking/biking, bus or car/motorcycle). Multinomial logistic regression was used to estimate the association between the outcome and sociodemographic, occupational and behavioral variables. The main form of commuting to and from work was by bus (45.7%). Workers with higher socioeconomic condition were more likely to engage in passive commuting.Estudo transversal visando estimar a prevalencia das formas de deslocamento para o trabalho e identificar fatores associados em trabalhadores do Estado do Rio Grande do Sul. Um total de 2.265 trabalhadores responderam questionario sobre a forma de deslocamento utilizado para ir ao trabalho: caminhada/bicicleta, onibus ou de carro/moto. Para estimar a associacao entre o desfecho e variaveis sociodemograficas, ocupacionais e comportamental foi utilizada a regressao logistica multinomial. O principal meio de deslocamento para o trabalho foi o onibus (45,7%). Trabalhadores com maior condicao socioeconomica tiveram maior probabilidade de deslocamento passivo.
