Chronic Arsenic Poisoning with Skin Cancer

Chronic arsenic poisoning is an alarming environmental health problem in Bangladesh. The toxicity of arsenic compounds depends on the amount, chemical and physical form of arsenic and the duration of exposure to arsenic. Skin manifestations are the prime and common manifestations of Arsenicosis. Patients are presenting with various complications relating to chronic arsenicosis, out of them we are reporting 2 cases of squamous cell carcinoma of skin. All were female, married, age ranging from 40-60 years, hailed from endemic zone of arsenicosis and the disease which were confirmed by histopathologically. Transfusion Transmissible Diseases Among Healthy Blood Donors Attended at Transfusion Medicine Department of DMCH in 2014

Background: This study was carried out to find out the seropositive of HBV, HCV, HIV, Treponema pallidum and Malarial parasite among the blood donors attended at transfusion medicine department of DMCH in 2014. Our objective is to prevent the spread of Transfusion transmissible disease through blood transfusion and awareness of mass population about TTI. Method: This study was carried out at transfusion medicine department of DMCH from 1st January 2014 to 31st December 2014.Total blood donors were 39,512.Blood donors of 18 - 60 years of both sexes were included in this study. Results: Among 39,512 blood donors relative blood donors were 32,778(82.95 %), voluntary blood donors were 6734.( 17.04% Among 39,512 blood donors , HBsAg positive 456 (1.154%), Anti HCV positive 33(.090%), Malarial Parasite positive 04 (.010), Treponema pallidum positive 87 (.220%) and positive for HIV 01(.0025%) Conclusion: Prevention of spread of TTIs should be the main goal at the current time. It is important to follow the WHO guidelines to tests every donor for HBV, HCV, HIV and Syphilis to decrease the transmission of TTIs. Clinical experience of docetaxel in combination with prednisolone and zoledronic acid for hormone-refractory prostate cancer (HRPC): A Bangladesh perspective.

e15049 Background:The patients with prostate cancer initially respond to androgen ablation therapy but most patients ultimately develop a hormone-refractory state. Most of the patients in our country present HRPC with bone metastasis. Earlier patients were treated with Mitoxantrone plus Prednisolone. Zoledronic acid was used for symptomatic relief of pain and increases the QoL. Docetaxel showed good response in case of HRPC. We conducted a study to evaluate the efficacy and toxicity of docetaxel in combination with zoledronic acid plus prednisolone in pain and PSA control. METHODS A total number of thirty patients were randomly assigned to receive zoledronic acid 4 mg/m2 in q3w with prednisolone 5 mg twice daily in combination with docetaxel 75 mg/m2 in patients with hormone refractory prostate cancer (HRPC) at our institution between July 2007 and June 2009. The primary endpoint was to evaluate the response to pain and PSA level. Secondary endpoint included overall survival and QoL. RESULTS A total of 28 patients were evaluable for the study. 43% of the pts had predefined reductions in pain. 54% had at least a 50% decrease in the serum PSA level. The overall response rate was 48.4% (13 patients), with partial responses in 24.24% (7 patients). The median duration of response was 20.2 weeks. 32% had improvement in the QoL. Median survival was 18.4 months. Adverse events related to Docetaxel included alopecia (60.7%), anorexia (50.1%) and fatigue (32.5%). Grade 3/4 neutropenia occurred in 48.4% of patients. CONCLUSIONS Earlier for bone Metastasis with HRPC, zoledronic acid, Mitoxantrone and prednisolone was the choice of treatment. But the result was not satisfactory. Combining docetaxel with zoledronic acid and prednisolone in Bangladeshi population has shown excellent response in case of pain and PSA reduction with tolerable toxicities. Though the survival rate was unchanged the combination of docetaxel with ZA and Prednisolone regimen is effective in reduction of pain and PSA level in Bangladeshi patient population.

Role of induction chemotherapy with docetaxel+cisplatin+5-FU (DCF) followed by radiotherapy for patients with inoperable head and neck cancer to observe local control

17027 Background: The combination of docetaxel (D) with cisplatin (C) has shown synergism in previous studies. In addition, docetaxel and 5-FU have shown activity in head and neck cancer. In Bangladesh we conducted a multicentre local study on DCF as neoadjuvant chemotherapy in inoperable and advanced head and neck cancer to assess the efficacy of local tumour control and tolerability. Methods: The treatment schedule consisted of docetaxel 60mg/m2 on day 1, cisplatin 25 mg/m2 and 5-FU 750 g/m2 on day 1–4, every three weeks for three cycles which was followed by radiotherapy. Radiotherapy was administered at a dose of 200 cGy/fraction for 33 fractions to a total of 6400–6600 cGy. Results: Between June 2003 and June 2006, a total of 30 patients were enrolled and assessable for response and toxicity analysis. Among them 25 were males and 5 were females with median age of 48 years (age range 30–66 years).
