Fasciotomy and Surgical Tenotomy for Recalcitrant Lateral Elbow Tendinopathy Early Clinical Experience With a Novel Device for Minimally Invasive Percutaneous Microresection

Background: The optimal choice for intervention for recalcitrant lateral elbow tendinopathy remains unclear as various treatment modalities have documented comparable results in the literature. Purpose: To explore the safety, tolerability, and early efficacy of a new minimally invasive mode of treatment that delivers focused, calibrated ultrasonic energy, effectively microresecting the pathological tendon and removing only pathological tissue. Study Design: Case series; Level of evidence, 4. Methods: Seven male and 13 female patients aged 33 to 65 years averaging 12.5 months (range, 4-48) of failed nonoperative therapy underwent the ultrasonic microresection procedure in an outpatient clinic setting. The procedure involved a sterile, ultrasound-guided percutaneous microresection with a proprietary device (TX1) performed through a stab incision under local anesthesia. The duration of the procedure and complications of the device or procedure were assessed. Outcome parameters included patient satisfaction; visual analog scale (VAS) pain scores; Disabilities of the Arm, Shoulder and Hand (DASH) scores at 1, 3, 6, and 12 months; and ultrasound assessment at 3 and 6 months. Results: The median duration for the sterile confirmatory ultrasound examination (phase 1) was 88.5 seconds (range, 39-211; SD, ±47.6), the median duration of the procedure proper (phase 2) was 10.1 minutes (range, 4.1-19.4; SD, ±3.7), and the median energy time (duration the TX1 device was activated) was 32.5 seconds (range, 18-58; SD, ±11.0). No complications were encountered. A significant improvement in VAS score (from 5.5 to 3.3; P < .001) occurred by 1 week, and significant improvements in both DASH-Compulsory (from 21.7 to 11.3; P = .001) and DASH-Work (from 25.0 to 6.3; P = .012) scores occurred by 1 month. The VAS scores further improved at 3, 6, and 12 months (from 2.0 to 1.0 to 0.50; P = .003 and .023). The DASH-Compulsory score improved significantly from 3 to 6 months (from 8.6 to 4.6; P = .003), and both the DASH-Compulsory and DASH-Work scores were sustained by 12 months. Sonographically reduced tendon thickness (19 patients), resolved or reduced hypervascularity (17 patients), and reduced hypoechoic area (18 patients) occurred by 6 months. Nineteen of the 20 patients (95%) expressed satisfaction with the procedure, with 9 patients being very satisfied with their overall experience at 6 months after the procedure, 10 patients somewhat satisfied, and 1 patient neutral. Conclusion: Ultrasonic microresection of diseased tissue with the TX1 device provides a focally directed, safe, specific, minimally invasive, and well-tolerated treatment for recalcitrant lateral elbow tendinopathy in an office-based or ambulatory surgical setting with good evidence of some level of efficacy in 19 of 20 patients (95%) that is sustained for at least 1 year.

Intravenous versus intra-articular tranexamic acid in total knee arthroplasty: A double-blinded randomised controlled noninferiority trial.

BACKGROUND Despite the proven efficacy of both intravenous (IV) and intra-articular (IA) tranexamic acid (TXA) in reducing blood loss during total knee arthroplasty (TKA), the ideal route of administration remained debatable. This study aimed to compare the effect of IV versus IA TXA on transfusion incidences, perioperative blood loss and postoperative lower limb swelling during TKA. METHODS One hundred patients were prospectively randomised into two groups: 1) IV TXA; and 2) IA TXA. In both groups, TXA was administered intraoperatively after cementing the prostheses. The perioperative blood loss was calculated using the haemoglobin balance method. The thigh, suprapatellar, and calf girths were measured preoperatively and on postoperative day (POD) 4. RESULTS Two patients in the IV group and one patient in the IA group required blood transfusion (p=0.500). The median and interquartile range (IQR) of perioperative blood loss on POD1 and POD4 was 530 (IQR 386,704) and 730 (IQR 523,925) ml for the IV group, compared with 613 (IQR 506,703) and 799 (IQR 563,1067) ml for the IA group (p=0.090 and p=0.232 respectively). The median increment in thigh, suprapatellar, and calf girths were 1.5 (IQR 0, 3.0), 2.0 (IQR 0.5, 4.0) and 0 (IQR 0, 1.0) cm for the IV group, compared to 2.0 (IQR 1.0, 4.0), 2.0 (IQR 0, 4.5) and 0 (IQR 0, 1.5) cm for the IA group (p=0.246, p=0.562, and p=0.937 respectively). CONCLUSIONS Both IV and IA TXA had comparable effect on transfusion incidences, perioperative blood loss, and postoperative lower limb swelling during TKA. IA TXA is an alternative to IV TXA. Intraoperative Morphometric Study of Gender Differences in Asian Femurs

Sixty-two male and 290 female knees from the Singapore population were measured intraoperatively for anteroposterior (AP) height of both medial and lateral femoral condyles, mediolateral (ML) width, and AP/ML ratio (aspect ratio). Median AP/ML ratio for the medial condyle was 1.00 (range, 0.91-1.18) in men and 1.09 (range, 0.92-1.39) in women. Median AP/ML ratio for the lateral condyle was 0.98 (range, 0.84-1.14) in men and 1.06 (range, 0.89-1.36) in women. Both aspect ratios showed significant differences according to sex (P < .001). We conclude that women generally have narrower femurs than men for any given AP height. The aspect ratios of Asians also appear narrower than that reported in white populations.

Functional Outcome and Quality of Life after Patient-Specific Instrumentation in Total Knee Arthroplasty

Patient-specific instrumentation (PSI) surgery may represent the next advancement in total knee arthroplasty (TKA). In 2011, 60 patients were prospectively recruited and divided into two groups based on the patients choice: (1) PSI surgery; and (2) conventional TKA. At two years after surgery, the Knee Society Function Score, Oxford Knee Score and SF-36 scores were comparable between the two groups. Although the Knee Society Knee Score (KSKS) was 9 ± 3 points better in the PSI group (P=0.008), the two years improvement in KSKS was comparable between the two groups. None of the patients required revision surgery. These findings cannot justify the additional costs and waiting time incurred by the patients with PSI surgery in the practice of a high volume surgeon.

Two-year outcome of early deep MRSA infections after primary total knee arthroplasty: a joint registry review.

The aim of this study is to determine the success rate in eradication of early methicillin-resistant Staphylococcus aureus (MRSA) prosthetic joint infection. Rate of prosthesis retention and functional outcome between patients with prosthesis retention and prosthesis revision were compared. All patients who underwent primary total knee arthroplasty between May 1998 and September 2008 at our institution developing early deep MRSA infection were included. Patient demographics, time from infection to initial arthrotomy, successful eradication of infection and functional outcome of patients with a knee prosthesis at 2 years were studied. Open arthrotomy, debridement, and change of liner successfully treated 33.3% of infections. All remaining infections went onto treatment with 2-stage revision with a success rate of 88%. Overall 92% of patients had a well-functioning knee prosthesis at 2 years.

Prospective randomised trial comparing unlinked, modular bicompartmental knee arthroplasty and total knee arthroplasty: A five years follow-up

BACKGROUND A significant proportion of patients with knee osteoarthritis have articular degeneration that is limited to the medial and patellofemoral compartments. The objective of this study was to compare clinical outcomes of unlinked bicompartmental knee arthroplasty (BCA) and total knee arthroplasty (TKA) at 5years in this subset of patients. METHODS 48 patients were randomised into two groups: unlinked, modular bicompartmental arthroplasty and total knee arthroplasty. Data on demographics and clinical outcomes were collected (Bartlett Knee Score, Oxford Knee Score, Knee Society Score). Data on intra-operative blood loss in both groups were also recorded. RESULTS Out of the 48 patients, 26 underwent BCA and 22 had TKA. Both groups shared similar demographic profiles. At five years post surgery, there was significant improvement across all functional scores in both groups. However, there was no significant difference in outcome scores in the BCA group compared to the TKA group. The drop in serum haemoglobin levels postoperatively was 1.55 and 2.30g/dl for the BCA and TKA groups respectively (p<.001). The total amount of blood loss was 397 and 647ml respectively (p=.001). CONCLUSIONS Unlinked, modular BCA results in similar clinical and functional scores as TKA for medial and patellofemoral arthritis in the mid-term. Intra-operative blood loss was significantly lower in the BCA group compared to the TKA group. BCA is a viable option for a select group of young and active patients with the advantage of reduced intra-operative blood loss and equivalent functional outcomes as TKA.

Simultaneous versus staged bilateral unicompartmental knee replacement

We prospectively followed 171 patients who underwent bilateral unicompartmental knee replacement (UKR) over a period of two years. Of these, 124 (72.5%) underwent a simultaneous bilateral procedure and 47 (27.5%) underwent a staged procedure. The mean cumulative operating time and length of hospital stay were both shorter in the simultaneous group, by 22.5 minutes (p < 0.001) and three days (p < 0.001), respectively. The mean reduction in haemoglobin level post-operatively was greater by 0.15 g/dl in the simultaneous group (p = 0.023), but this did not translate into a significant increase in the number of patients requiring blood transfusion (p = 1.000). Association of surgeon factors with outcome scores after total knee arthroplasty

There was no significant difference in the rate of complications between the groups, and at two-year follow-up there was no difference in the outcomes between the two groups. We conclude that simultaneous bilateral UKR can be recommended as an appropriate treatment for patients with bilateral medial compartment osteoarthritis of the knee.

Intra-articular tranexamic acid wash during bilateral total knee arthroplasty.

Purpose. To identify preoperative factors (including surgeon factors) associated with outcome scores after total knee arthroplasty (TKA). Methods. Medical records of 2848 patients (3458 knees) who underwent primary TKA by 27 orthopaedic specialists were retrieved. Three specialty knee surgeons who had one-year fellowship in TKA performed 1930 TKAs, and 24 general orthopaedic surgeons performed 1528 TKAs. Four of them (including all 3 specialty knee surgeons) were ultra-high-volume (⩾100 TKAs a year), and 21 of them were senior consultants (⩾5 years post residency). At 2 years, 2922 (85%) of knees had complete follow-up data. Oxford Knee Score, Knee Society knee and function scores, and SF-36 quality-of-life score were assessed by independent physiotherapists before and after surgery. Outcomes were compared in terms of dichotomised specialty, seniority, and surgical volume of surgeons. Results. Comparing ultra-high-volume (⩾100 TKAs per year) specialty knee surgeons with general orthopaedic surgeons, the former achieved better outcomes in terms of the Oxford Knee Score at 6 months, Knee Society knee and function scores at 2 years, and SF-36 scores at 6 months and 2 years. Comparing lower-volume (±100 TKAs per year) specialty knee surgeons with general orthopaedic surgeons, the former still achieved better outcome and quality-of-life scores, except for SF-36 Mental Component Score at 2 years. Conclusion. Specialty training and clinical research in TKA improved outcome and quality-of-life scores.

Minimally Invasive Computer-Assisted Total Knee Arthroplasty Compared With Conventional Total Knee Arthroplasty: A Prospective 9-Year Follow-Up

Purpose. To compare the outcome after simultaneous bilateral total knee arthroplasty (TKA) with or without an intra-articular tranexamic acid (TXA) wash in terms of blood loss, haemoglobin change, and transfusion requirement. Methods. 35 women and 10 men (mean age, 67.5 years) who underwent primary simultaneous bilateral TKA by a single senior surgeon were compared with 45 matched controls. In the TXA group, 1500 mg of TXA diluted in 100 ml of 0.9% sodium chloride was administered as a wash after cementing of implant and before closure of the retinaculum. At least 5 minutes of contact time was allowed before wound closure and tourniquet deflation. No drain was used. Results. No patients had thromboembolic complication. Compared with controls, the TXA group had lower perioperative blood loss (920 vs. 657 ml, p=0.001), total blood loss (997 vs. 679 ml, p<0.001), blood transfusion rate (60% vs. 37.8%, p=0.035), percentage of patients requiring more than one blood unit (24.4% vs. 8.9%, p=0.048), and length of hospitalisation (6 vs. 4 days, p<0.001). Nonetheless, the 2 groups were comparable in blood units and volume transfused. Conclusion. An intra-articular TXA wash during simultaneous bilateral TXA reduced total blood loss and resulted in a difference of 22.2% in blood transfusion rate and a 2-day reduction in the length of hospital stay.
