Background Three years ago, the nonablative wrinkle reduction laser (a 585‐nm laser, Chromogenex V3; Chromogenex Light Technologies, Llanelli, U.K.) was developed, and there have already been several reports about its clinical effectiveness. The Chromogenex V3 laser has also been reported to be effective in treating acne and atopic dermatitis. These results suggest that the Chromogenex V3 laser has some immunological role. In this study, we investigated immunological changes elicited by laser irradiation at the ultrastructural level and by analysis of interleukin (IL)‐2 and IL‐4 mRNA in skin homing T lymphocytes.

Editor A 59-year-old Japanese male presented with two black skin lesions, which had developed concurrently on his right foot 2 years prior. The clinical features revealed a 14 × 12 mm sized irregular black macule on the tip of the right great toe (fig. 1a) and a 28 × 25 mm sized irregular black nodule with slight prominence on the lateral portion of the right mid-sole (fig. 1b). These lesions were both clinically diagnosed as acral lentiginous melanomas. Further examination revealed no additional significant findings throughout the whole body. The patient was scheduled for an operation, which would involve lymphatic mapping and sentinel lymphadenectomy. 99mTc-phytate was injected intradermally into the four quadrants around both lesions. Thirty minutes later, the gamma camera detected two lines, which were identified as lymphatic vessels in the lower extremity (fig. 2). A 2% patent blue dye was subsequently applied to each tumour. The lymphatic channel from the lesion on the tip of the great toe ascended via the dorsal aspect of the toe to the dorsalis pedis: another from the lateral portion of the mid-sole traversed medially to the anterior part of the non-weight-
