Optic gliomas in children

A. M. Chutorian; James F. Schwartz; Robert A. Evans; Sidney Carter

SINCE the first clear description of a patient with optic glioma by Wishartl in 1833, numerous publications concerned with this subject have appeared, including several detailed reports and excellent reviews of the Considerable controversy exists with respect to the fundamental histopathology of the lesion, its natural course, and treatment. There are many descriptions of patients who have had remarkably protracted and benign courses, after only partial extirpation of optic gliomas or even without benefit of therapy,5Jn-1G making the evaluation of therapy exceedingly difficult. Taveras and associates8 in 1956 reviewed the experience at the Columbia-Presbyterian Medical Center with 34 patients of all ages, including 21 children, who had optic gliomas. This review was the first report in which enthusiastic statements were made about the efficacy of radiation therapy in a group of significant size. The present study provides the opportunity for a considerable increase in the duration of follow-up of many of the patients studied by Taveras and his associates, and also includes a much larger population of children. An attempt was made in the present study to relate the diagnostic and therapeutic data to the prognosis for vision and survival in this group of children with glioma of the optic nerve or chiasm, or both.
