Our understanding of dental plaque biofilm has evolved since the nonspecific plaque hypothesis that considered plaque as a nonspecific mass of native microorganisms that, because of lack of oral hygiene, builds up in proportions great enough to overcome the host resistance threshold and affect the tooth structure and tooth supporting tissues. A great diversity of microorganisms-over 700 species-was detected in the oral cavity, and evidence shows that the investigation of specific microorganisms or associations of microorganisms as etiological agents for periodontal diseases and caries is not a simplistic approach. Although clinical evidence shows that oral mechanical hygiene is fundamental to prevent and control caries and periodontal disease, it is important to highlight that optimal control is not achieved by most individuals. Thus the complementary use of chemotherapeutic agents has been investigated as a way to overcome the deficiencies of mechanical oral hygiene habits, insofar as they reduce both plaque formation and gingival inflammation, and represent a valid strategy to change the biofilm and maintain dental and periodontal health. The role of the dental professional is to monitor patients and offer them the best recommendations to preserve oral health throughout their life. With this in mind, chemical control should be indicated as part of daily oral hygiene, together with mechanical procedures, for all individuals who present supragingival and/or subgingival biofilm, taking into account age, physical and/or psychological limitations, allergies, and other factors.

EFFECT OF TWO MOUTHWASHES ON SALIVARY PH

To analyze the effect of two mouthwashes on salivary pH and correlate it with age, buffer capacity and saliva flow rate in healthy volunteers, a crossover phase IV clinical study involving three age-based groups was designed. Two commercial mouthwashes (MW), Cool Mint ListerineR (MWa) and Periobacter R (MWb) were used. The unstimulated saliva of each individual was first characterized by measuring flow rate, pH, and buffer capacity. Salivary pH was evaluated before rinsing with a given MW, immediately after rinsing, 5 minutes later, and then every 10 min (at 15, 25, 35 min) until the baseline pH was recovered. Paired t-test, ANOVA with a randomized block design, and Pearson correlation tests were used. Averages were 0.63 mL/min, 7.06, and 0.87 for flow rate, pH, and buffer capacity, respectively. An immediate significant increase in salivary pH was observed after rinsing, reaching average values of 7.24 (MWb) and 7.30 (MWa), which declined to an almost stable value 15 minutes. The great increase in salivary pH, after MW use shows that saliva is a dynamic system, and that the organism is capable of responding to a stimulus with changes in its composition. It is thus evident that pH of the external agent alone is not a good indicator for its erosive potential because biological systems tend to neutralize it. The results of this study enhance the importance of in vivo measurements and reinforce the concept of the protective action of saliva.

Mouthrinses containing alcohol and oral cancer: revision of epidemiological studies

The strong association between alcohol usage and the development of oral cancer (OC) has been reported in numerous papers. As some mouthrinses contain significant amounts of ethanol, a possible relationship to this pathology has been considered. The purpose of the present paper is to analyze several epidemiological studies which evaluated the association between commercial mouthrinses and the etiology of OC. Although some authors report an association, most of the studies are unclear and sometimes contradictory. The controversial aspects regarding the role of alcohol in OC may also make difficult to find a clear relationship between the use of mouthrinses containing alcohol and OC.

Nitric oxide synthase 2 (NOS2) expression in histologically normal margins of oral squamous cell carcinoma

The activity of Nitric Oxide Synthase 2 (NOS2) was found in oral squamous cell carcinomas (OSCC) but not in normal mucosa. Molecular changes associated to early carcinogenesis have been found in mucosa near carcinomas, which is considered a model to study field cancerization. The aim of the present study is to analyze NOS2 expression at the histologically normal margins of OSCC. Study Design: Eleven biopsy specimens of OSCC containing histologically normal margins (HNM) were analyzed. Ten biopsies of normal oral mucosa were used as controls. The activity of NOS2 was determined by immunohistochemistry. Salivary nitrate and nitrite as well as tobacco and alcohol consumption were also analyzed. The Chi-squared test was applied. Results: Six out of the eleven HNM from carcinoma samples showed positive NOS2 activity whereas all the control group samples yielded negative (p=0.005). No statistically significant association between enzyme expression and tobacco and/or alcohol consumption and salivary nitrate and nitrite was found. Conclusions: NOS2 expression would be an additional evidence of alterations that may occur in a state of field cancerization before the appearance of potentially malignant morphological changes. Oral Squamous Cell Carcinoma Clinical Aspects

Oral cancer (OC) is considered a serious public health problem that causes great morbidity and mortality in the population. While OC has a lower incidence than other malignant tumors, it is known to produce high mortality and serious disturbances or discomfort in the patient as a consequence of either the tumor itself or of the treatment. The Oral Squamous Cell Carcinoma is the most common malignant tumor of the lip, oral cavity and oropharynx (90% of the cases) while the remaining 10% of the cases are mainly melanomas, sarcomas, minor salivary gland carcinomas and metastatic cancers (Scully et al., 2006).

Study of virulence factor of Candida species in oral lesions and its association with potentially malignant and malignant lesions

OBJECTIVE The aim of this study was to explore the association between malignant and premalignant lesions and the virulence factor profile of Candida spp. recovered from different oral lesions. DESIGN Candida spp. isolated from malignant lesions (squamous cell carcinoma, OC, n = 25), atypical lichen planus (AL, n = 11), chronic candidiasis (CC, n = 25), and asymptomatic carriers (WI, n = 15, control strains.) Isolates were identified in chromogenic medium, colony morphology and biochemical tests. The lipolytic and proteinase activity was determined on supplemented agar with olive oil and BSA, respectively. The biofilm formation with XTT reduction assay and cellular surface hydrophobicity (CSH) by water-hydrocarbon method were performed. RESULTS All isolates recovered from oral lesions produced the four virulence factors studied with significantly higher levels than in WI isolates. Interestingly, lipolytic activity was absent in WI isolates. The proteolytic activity was similar in AL and OC isolates. OC isolates showed significantly higher CSH values than other clinical isolates. Non-albicans species showed higher biofilm formation than C.albicans (P = 0.03.) There were no significant differences in virulence factors among species. A strong positive correlation was found between proteinase and lipase activity (r = 0.90, P < 0.0001), and between hydrophobicity and biofilm (R = 0.81, P < 0.0001.) CONCLUSIONS: Our results indicate that OC Candida isolates exhibited a significant higher attributes of virulence than other lesions fungus isolates, providing evidence about the association between Candida pathogenicity and lesions severity.

Thirty human teeth (patient age range 13-25 years, third molars and first premolars) fixed in neutral forrnalina pour 48 hs. and histologyc technique. Cavity carving and filling with amalgama. The sample were sectioned (VL) with carborundum disk. Observed by microscopy lateral incidence. Results: Revelated sub-superficial decalcification PAS + , 40 to 80 micrometers undept and extension 200 to 400 micrometers was not included in cavity preparation, corroborated by MEBSe estudiaron 27 pacientes con sindrome de obstruccion respiratoria. Por medio de telerradiografias laterales se evaluaron dimensiones de nasofaringe midiendo sobre las mismas hipertrofia de cornetes y adenoides. Otros estudios fueron realizados sobre telerradiografias frontales y rinomanometrias. Se realizaron cefalomgramas Ricketts y McNamara. Se obtuvo como resultado que el area libre en respiradores bucales por hipertrofia de cornetes y adenoides disminuye significativamente dificultando el pasaje aereo normal y que el valor medio de H.C. es de 8,2 mm. En el estudio de estructuras profundas el plano palatino, el mandibular, profundidad facial y altura facial inferior fueron los mas significativamente alterados (AU)Se analizan las circunstancias y caracteristicas del terreno oseo en irnplantologia, para llegar a un diagnostico del momento en que se aplicaran los implantes. Segun este analisis se los clasifica en inmediatos y diferidos. Los inmediatos tienen ventajas al reducir el tiempo operatorio y conservacion del proceso alveolar. En esta forma de implantar se pueden emplear recursos complementarios: dilatacion alveolar, injertos oseos autologos recubiertos con periostio, materiales osteotroficos u oseinico mineral, que evita la lisis del coagulo y favorece la coagulacion sanguinea y materiales para reforzar o de refuerzo corno es laesponja de titanio. Los diferidos respecto a su instalacion, pueden tener interrogantes en su pronostico por la existencia de areas osteoliticas que comprometen la instalacion de los implantes. Se evaluaron variables binarias que se incorporaron a un modelo DAG con estructura de causa suficiente para cada patologia.Se realizo el estudio citoquimico y ultraestructural de las glandulas linguales del embrion de pollo con el objeto de analizar el desarrollo morfogenetico y los glicoconjugados elaborados por las mismas para inferir sus posibles significancias histofisiologicas con referencia a las glandulas salivales de esta especie. Se emplearon lengua de embriones de pollo de 7 a 19 dias de incubacion las que se procesaron para su estudio ultraestructural y citoquimico (Rojo de Rutenio, PAS, Alcian blue, Azul de Toluidina). A los 11 dias aparecian esbozos y cordones celulares canalizados cuyas celulas se relacionaban mediante complejos de union. No se detecto reaccion positiva para mucinas. A partir de los 15 dias los acinos estaban bien diferenciados, con granulos secretorios PAS positivos, alcianofilicos y metacromaticos. Poseian organelas tipicas de la sintesis proteica y secrecion luminal Rojo de Rutenio positiva, alcianofilica, metacromatica y PAS positiva. A los 19 dias se detecto un marcado incremento de las mucinas. Estas observaciones demuestran que las glandulas linguales estan citoquimica y ultraestructuralmente diferenciadas desde los 15 dias del desarrollo y contribuiran por medio de los glicoconjugados como ya ha sido establecido a la lubricacion de los alimentos. Ademas podrian iniciar el proceso digestivo de los mismos y protegerian a la mucosa bucal, evitando su deshidratacion y la proliferacion de germenes patogenosEn las pasadas decadas se ha observado un interes creciente en el estudio de las predisposiciones geneticas a las enfermedades complejas, esto ha desembocado en una enorme cantidad de publicaciones epidemiologicas sobre asociaciones entre gen y enfermedad. Sin embargo, no siempre la magnitud de la asociacion entre la presencia de un determinado genotipo y la enfermedad esta determinada, por lo cual es necesario identificar las verdaderas asociaciones geneticas entre la presencia de una enfermedad compleja y los genotipos de las personas. Uno de los puntos en contra de estos estudios son los tamanos de muestra con los que se realizan los estudios y las variantes genotipicas que deben estudiarse por lo cual las revisiones sistematicas y meta-analisis resultan herramientas valiosas para establecer certeramente las asociaciones entre genes y enfermedades. To gather the data the epidemiological card to register malocclusions of qualitative range was used, made by the University of Dentistry of Montevideo, Uruguay, and this enabled us to determine the morphofunctional characteristics that prevail in temporary, mixed and permanent dentition. The results indicate a prevalence of abnormal lip closure and the presence of habits. Metodos: Se planifico y desarrollo una capacitacion semi-presencial titulada “Viaje al mundo microscopico: Aportes de la microscopia del siglo XXI”, en base a la demanda de los docentes de escuela primaria en relacion a la tematica de la microscopia. Resultados: Esta intervencion extensionista, de caracter interdisciplinario e integrado, tuvo un impacto muy positivo en los actores involucrados -profesores primarios y secundarios, formadores de futuros docentes y docentes universitarios- enriqueciendo el conocimiento de problematicas educativas y estrategias para afrontarlas. Conclusiones: Se considera que los ninos seran los principales beneficiarios de esta intervencion extensionista, la cual ha contribuido a promover la equidad y la calidad educativa. Se destaca asimismo que a traves de esta experiencia se genero una valiosa capacidad instalada -sostenible a traves del tiempo- de recursos de estudio y actividades de aplicacion virtuales, que permiten aprovechar el potencial pedagogico de las TIC.Las ciencias de la salud son, por su propia naturaleza intrinseca, un quehacer moral. Glands were processed by conventional histology (hematoxilin-eosin and Masson trichromic) and histochemical techniques including PAS, PAS/neuraminidase, alcian blue, toluidine blue and PNA lectin. The obtained results ha ve indicated the presence of numerous glands located anteroposterior and lateral !O the lingual cartila~e. In the anterior region, gla73ndular lo bes showed tubular structures formed by basophilic cuboidal cells.- In the posterior region a scarce number of mucous adenomers and intraepithelial glands were found. The histochemical reactions indicated the presence of glycoconjugate compounds in the glandular secretion. These glycoconjugates had a high number of terminal sialic acid residues because it was observed a decrease in PAS and alcian blue reactions after neuraminidase digestion. In addition, the affinity by PNA lectin was increased with enzymatic digestion. lt was also detected a strong alician.;philic and metachromatic sulphated groups. These findings showed that the glandular lingual secretion in these bires, present the same types of glycoconjugates as !hose described in mammals.
