Plasmacytoma of the orbit involving lacrimal gland with secondary transformation into multiple myeloma: case report

The authors present a rare case of plasmacytoma of the orbit involving lacrimal gland with secondary transformation into multiple myeloma in a 42-year-old woman. The lesion was surgically removed and analyzed. Histopathological examination with immunostaining revealed it to be positive for immunoglobulin G and Kappa chains, demonstrating monoclonality. However, no abnormality was observed on serum electrophoresis, skeletal survey and bone marrow aspiration. Therefore, the tumor was diagnosed solitary plasmacytoma of bone. The patient was treated with external beam radiotherapy and has remained disease free for 5 years and 6 months until 2007, when she presented a pathological fracture due to multiple myeloma. Extensive medical work-up to rule out multiple myeloma or other malignant lymphoproliferative conditions involving orbit or ocular adnexa is needed when the diagnosis of solitary plasmacytoma of bone is suspected because treatment and prognosis are very different.

Tumor neuroectodérmico primitivo periférico primário da órbita: relato de caso

The authors present a case of primary peripheral primitive neuroectodermal tumor of the orbit in a 10-month-old girl, which is the first case in the Brazilian literature. Clinical, radiologic and histopathologic features are discussed. The diagnosis was confirmed by immunohistochemical analysis which is essential to the diagnosis of small round cell tumors in the orbit.

Canaliculite crônica supurativa - aspectos clínicos e terapêuticos: relato de 3 casos

Lacrimal canaliculitis is a rare disease caused mainly by Actinomyces israelii. It should be regarded as a differential diagnosis of recurrent chronic conjunctivitis. The purpose of this study was to report 3 cases of chronic suppurative canaliculitis and different treatment options. The first patient presented with an upper left canaliculitis and was treated with a canaliculotomy. Nevertheless, he had an ipsilateral lower canaliculitis after 6 months and underwent intracanalicular injections of fortified cefazolin with complete remission. The second patient presented with a lower left canaliculitis and underwent a canaliculotomy. The third patient had a lower left canaliculitis and underwent intracanalicular injections of fortified cefazolin. Both achieved complete remission. The present article demonstrated that intracanalicular irrigation of fortified cefazolin may be a helpful treatment of chronic suppurative canaliculitis with mild symptoms and signs. The most important benefit of this approach is to avoid injury to the lacrimal canaliculus.

Dacrioadenite bilateral causada pelo vírus Epstein-Barr: relato de caso

UNLABELLED The authors present a rare case of acute bilateral dacryoadenitis in association with infectious mononucleosis. A 27 year-old woman had acute bilateral lacrimal gland enlargement with inflammatory signs. The CT scan findings and laboratorial investigations confirmed the diagnosis of infectious mononucleosis. The systemic treatment with intravenous steroids was initiated leading to complete remission of the signs and symptoms. COMMENTS Dacryoadenitis is an uncommon manifestation of infectious mononucleosis and may minimize other signs of the disease. Mononucleosis has to be considered in acute dacryoadenitis cases. Systemic steroids contribute to the fast regression of inflammatory condition.

Myiasis is the invasion of human tissues by Diptera larvae. Ocular involvement is rare. The objective of this paper is to report a case of myiasis in the topography of the lacrimal sac and discuss current treatments for this disease.

Microscopic alterations in silicone tubes surface after application of ophthalmological lubricants

Objective: To identify microscopic morphological alterations in the surface of silicone tubes used for intubation of the lachrymal system after exposure to ophthalmological lubricants. Methods: Experimental, descriptive and longitudinal study consisted of the application of ophthalmological lubricants in silicone tubes. The tubes were divided in: Group 1 (Cylocort®), 2 (Epitezan®), 3 (Labcaina®), 4 (Liposic®), 5 (Maxinom®) and 6 (Vista Gel®). One tube was not exposed to any lubricant, used as control. The tubes were observed and photographed after 2 hours, 30 days, 45 days before and after cleaning the surface and lumen. The following aspects were observed: surface (regularity, transparency, quantity, size and shape of the substances) and lumen (obstruction). Results: Control: irregular surface with pores after 2 hours: Group 1 – irregular surface with presence of film; Groups 2, 3 and 5 – abundant and irregular quantity of ointment at the surface; Group 4 – discrete modification at the surface; Group 6 – growth of pigmented (brownish) structures with filaments in the lumen, with discrete film in the surface. 30 Days: Groups 1, 4 and 5 – increase of the irregular superficial film; Group 2 – crust with notorious horizontal lines; Group 3 – diminution of the superficial film; Group 4 – crust less evident. Group 6 – increase of the structure seen with 2 hours of exposition, arboriform aspect. Forty-five days pre cleaning: Group 4 – diminution of the surface crust; Group 6 – expansion of the arboriform structure; unaltered findings in other groups. 45 days after cleaning: Groups 1 and 5 – light diminution of the surface crust; Groups 2, 3 and 4 – kept the modifications; Group 6 – the structure inside the lumen was not identified, clear surface, without evidence of film. Conclusions: Microscopic morphological alterations in the surface and lumen of silicone tubes can occur when those remain in contact with determined ophthalmological lubricants.

Resultados da conjuntivodacriocistorrinostomia com implante de prótese lacrimal de polietileno de baixa densidade

Objetivos: Estabelecer quais sao as complicacoes pos-operatorias da conjuntivodacriocistorrinostomia (CDCR) com implante de protese lacrimal de polietileno de baixa densidade e relacionar a etiologia da obstrucao e o aparecimento destas complicacoes e, consequentemente, o sucesso pos-operatorio. Metodos: Foram analisadas 35 vias lacrimais submetidas a conjuntivodacriocistorrinostomia com implante de protese lacrimal de polietileno. Com a finalidade de estudar as possiveis relacoes entre as variaveis encontradas (etiologia x complicacoes e etiologia x sucesso) realizou-se o teste exato de Fisher. Resultados: As etiologias de obstrucao mais frequentes foram a dacriocistite cronica e pos-dacriocistorrinostomia que, em conjunto, foram responsaveis por 53,9% dos casos. Os traumatismos ocasionaram 16,3% das obstrucoes. A taxa de complicacoes encontrada foi 74,3%. A conjuntivodacriocistorrinostomia apresentou sucesso em uma unica intervencao cirurgica em 9 vias lacrimais (25,7%). Apos duas ou mais intervencoes cirurgicas, 17 proteses lacrimais tornaram-se pervias e bem localizadas (48,6%). O insucesso ocorreu em 9 vias lacrimais (25,7%). Nao houve relacao estatisticamente significante entre etiologia e ocorrencia de complicacoes nem entre etiologia e sucesso pos-operatorio. Conclusoes: As complicacoes encontradas sao similares as descritas na literatura e nao tiveram relacao com a etiologia da obstrucao canalicular. O mesmo pode ser considerado no que se refere ao sucesso da conjuntivodacriocistorrinostomia. As vantagens da protese lacrimal de polietileno de baixa densidade sao a grande disponibilidade, a facil confeccao no per-operatorio de acordo com o comprimento e o diâmetro do colarete desejados e pode ser substituida pela protese lacrimal de vidro de borosilicato assim que o edema regrida.
