Initiatives to improve prescribing efficiency for drugs to treat Parkinson’s disease in Croatia: influence and future directions

Parkinson’s disease (PD) is the second most common neurological disease affecting older adults. Consequently, this disease should be a focus among payers, with increasing utilization of newer premium-priced patent-protected add-on therapies to stabilize or even improve motor function over time. However, expenditure can be moderated by reforms. Consequently, there is a need to assess the influence of these reforms on the prescribing efficiency for drugs to treat PD in Croatia before proposing additional measures. Prescribing efficiency is defined as increasing the use of add-on therapies for similar expenditure. An observational retrospective study of the Croatian Institute for Health Insurance database of drugs to treat patients with PD in Croatia from 2000 to 2010 was carried out, with utilization measured in defined daily doses (defined as the average maintenance dose of a drug when used in its major indication in adults). The study years were chosen to reflect recent reforms. Only reimbursed expenditure is measured from a health insurance perspective. Utilization of drugs to treat PD increased by 218% between 2000 and 2010. Reimbursed expenditure increased by 360%, principally driven by increasing utilization of premium-priced patent-protected add-on therapies, including ropinirole and pramipexole. However, following recent reforms, reducing expenditure/defined daily dose for the different drugs, as well as overall expenditure, stabilized reimbursed expenditure between 2005 and 2010. Treatment of PD is complex, and add-on therapies are needed to improve care. Reimbursed expenditure should now fall following stabilization, despite increasing volumes, as successive add-on therapies lose their patents, further increasing prescribing efficiency.

Patients Practice and Knowledge of Pharmaceuticals Disposal: Example from Some Rural Areas of Croatia

Consumption of the pharmaceutical products for both human and veterinary use is constantly increasing. As the consequence considerable amount of pharmaceuticals and its metabolites is released into the environment at therapeutic concentrations. The aim of this article is to describe patient practice and knowledge of unused drugs disposal in some rural areas of Croatia. Results showed that mostly older population live in these areas with considerable co morbidity of various chronic diseases, using considerable number of pharmaceuticals. Educational level and therapy compliance is low, as well as knowledge on how to dispose unused pharmaceuticals. On the other hand, due to the relative geographical isolation there is tendency to stock various pharmaceuticals as well as self-medication. Unused drugs end up improperly disposed in the ecologically delicate environment rich in drinking water supply. Strong support from primary health care is needed as well from pharmacies for returning of unused pharmaceutical when possible or clear advice how to properly dispose them if they cannot be returned.

CLINICAL CHARACTERISTICS AND COMORBIDITY OF PEDIATRIC TRICHOTILLOMANIA: THE STUDY OF 38 CASES IN CROATIA

BACKGROUND The main goal of this study was to analyse and show clinical characteristics and psychiatric comorbidity in 38 participants aged between 10 and 17 with DSM-IV diagnoses of Trichotillomania (TTM) that we were treating at Childrens Hospital Zagreb from 2008 to 2017. SUBJECTS AND METHODS We analyzed the data obtained from semi-structured interviews by the criteria of DSM-IV, Youth Self Report (YSR) (Achenbach & Rescorla 2001) and survey that we created. RESULTS From 38 participants 21 were girls. The activities during which the participants state that they mostly pull hairs are as follows: doing homework and learning, working on PC, in the toilet, watching TV etc. The most common sites on the body from which participants pulled hair were scalp and among nonscalp sites eyebrows and eyelashes. We found nail biting in more than a half of participants. In 22 participants one or more comorbid disorder has been found, of which ADHD (n=6) and tics (n=5) are most co-occurring disorders. The internalized and externalized problems were nearly evenly represented. Trichophagia was reported by two participants. The results indicate that more than two thirds of participants isolate themselves during hair pulling and half of them try to hide consequences. Median time from the first occurrence of the symptoms to the first visit to a child psychiatrist caused by TTM problem was 9 months (min 5; max 24) what we consider a very long period of time that increased the probability of complications. CONCLUSIONS Knowledge about this disorder and cooperation among pediatric experts is extremely important for recognizing it at an early stage and starting the treatment especially considering habit-forming mechanism, the burden of an emotional distress and frequent comorbidity. Further research is needed.

Is salt intake hidden risk for rural population: case study Village of Sjeverovac, County Sisacko-moslavacka?

Nutritional habits between urban and rural population differ as much as life style and socioeconomic standards of these populations. In Croatia, rural populations are mostly live stock producers. Their nutrition frequently depends on their own production, which includes high quantity of cured meat products, eggs and dairy products. Data were obtained from longitudinal the Croatian Adult health Survey and pilot cross-sectional survey in three villages in Sisacko-moslavacka county. Our findings show that there is no great difference between urban and rural population regarding salt intake. The difference is in the type of food and food preparation, as well as life style. We propose combination of more culturally and socially sensitive questionnaires along with analytical methods that include biological matrix, such as 24-hour urine collection.
