Osteosyntheseverfahren bei dislozierten proximalen Humerusfrakturen

ZusammenfassungZwischen April 2000 und März 2002 wurden 102 dislozierte proximale Oberarmfrakturen, davon 61 vom Typ Neer IV, V und VI, mit einer neuen winkelstabilen T-Platte osteosynthetisch versorgt. Implantat, Operationstechnik und erste klinische Erfahrungen werden vorgestellt. Lediglich in 6 von 102 Fällen wurden sekundäre Dislokationen des Kopffragments beobachtet. In 95 von 102 (93,1%) Fällen wurde eine Übungsstabilität erreicht, die eine uneingeschränkte, unmittelbar postoperative krankengymnastische Übungsbehandlung erlaubte. Auch bei schwieriger Frakturmorphologie wurde frühzeitig eine gute Funktion des Schultergelenks erreicht. Die 1-Jahres-Nachuntersuchung von bisher 45 Patienten erbrachte einen relativen Constant-Score von 80,5% (71,1/88,3 Punkte). Teilnekrosen des Humeruskopfs wurden in 6,6% der Fälle festgestellt und gingen mit einem signifikant schlechteren Ergebnis (45,1%) einher.AbstractFrom April 2000 to March 2002 we treated 102 dislocated proximal humeral fractures, 61 of which were classified as Neer types IV, ¶V and VI, with a new T-plate that is fixed by means of locking screws and incorporates angular stability. The design of the implant, the operative procedure, and our own initial experience are presented. X-ray follow-up has shown secondary dislocation of the humeral head fragment in only 6 of the 102 cases. A rigid osteosynthesis was achieved in 95 (93.1%) cases, allowing unrestricted physical therapy immediately after the operation. Even patients with more complicated fractures and a poorer bone quality attained good shoulder joint function within a short time. The 1-year follow-up in 45 cases has shown relatively constant scores of 80.5% (71.1/88.3 points ). Partial necrosis of the humeral head occured in 6.6% of cases, in which the functional outcome was significantly worse (45.1%) than in the others.
