Adultos e idosos em hemodiálise: avaliação da qualidade de vida relacionada à saúde

Objectives: To categorize adults and elderly people in hemodialysis treatment residing in Ribeirao Preto, SP, Brazil. To evaluate and describe the differences in the Health-Related Quality of Life (HRQOL)of these patients. Methods: Cross-sectional and population study with 194 patients in hemodialysis in four dialyses services in the city. The instruments used were: Mini Exam of Mental State to characterize the population and Kidney Disease and Quality of Life-Short Form (KDQOL-SF TM ). Results: There were 132 adults and 62 elderly individuals in the study. Differences between the two groups’ average scores were found with statistical significance in the KDQOL-SF TM dimensions: physical functioning, role-physical, role-emotional, Burden of kidney disease and dialysis staff encouragement. Conclusion: Chronic renal failure and hemodialysis are related to the HRQOL of adults and elders. The results can support the health professionals’ actions in providing the care to the patients’ imminent needs, prevent complications and aim at a better HRQOL.

O idoso institucionalizado: avaliação da capacidade funcional e aptidão física

This study analyzed the relationship between physical fitness and functional capacity in 78 residents of long-stay institutions for low-income elderly located in five regions of Brazil. The majority of the sample consisted of women, and mean age was 77.4 years (SD = 7.9). Physical fitness was assessed with the AAHPERD test, adjusted for institutionalized elderly. The Katz scale was used for functional capacity. The five components of physical fitness rated fair for flexibility, coordination, agility, and aerobic endurance and good for strength. The mean general physical fitness (GPF) index was fair. According to the findings, the greater the degree of dependency in institutionalized elderly, the lesser their strength and GPF level; meanwhile, better coordination and agility are associated with greater independence for performing activities of daily living. The results can contribute to appropriate physical exercise programs for maintenance and/or recovery of functionality.

Idosos com insuficiência renal crônica: alterações do estado de saúde

This study aimed to describe health condition changes as experienced by elderly with Chronic Kidney Failure (CKF), under Continuous Ambulatory Peritoneal Dialysis (CAPD) treatment at the Dialysis and Kidney Transplantation Unit of a public hospital in Ribeirão Preto -- SP, Brazil. Participants were six elderly under CAPD treatment, aged 61 to 74 years. The theoretical and methodological framework was thematic oral history as proposed by Meihy. Data analysis led to the category of changes in elderlys health condition and disclosed the diagnosis of the disease, signs and symptoms of CKF, causes and irreversibility.Descrever as alteracoes do estado de saude vivenciadas por idosos portadores de Insuficiencia Renal Cronica (IRC), em tratamento de Dialise Peritonial Continua Ambulatorial (DPCA), atendidos na Unidade de Dialise e Transplante Renal de um Hospital Publico de Ribeirao Preto-SP foi o objetivo deste estudo. Participaram seis idosos, com idade entre 61 e 74 anos, em tratamento com DPCA. O referencial teorico metodologico utilizado foi a historia oral tematica, proposta por Meihy. Na analise, foi construida a categoria: alteracoes no estado de saude dos idosos e evidenciado o diagnostico da doenca, sinais e sintomas da IRC, causas e irreversibilidade.

Política nacional de atenção ao idoso e a contribuição da enfermagem

Este estudo objetivou descrever e avaliar a Politica Nacional de Atencao ao Idoso no Brasil e sua relacao com a enfermagem. Foi realizada uma revisao bibliografica do tema, o que permitiu a sua caracterizacao sob a forma de leis, decretos, textos e artigos em periodicos e livros. Os atores sociais envolvidos na Politica Nacional de Atencao ao Idoso foram os idosos, as familias, a comunidade e a equipe de saude, com destaque para a enfermagem. Apos analise da literatura consultada, concluiu-se que esta politica tem proporcionado, aos atores sociais envolvidos, conscientizacao para justica social e garantia plena dos direitos aos idosos. Concluiu-se, tambem, que a insercao da enfermagem na Politica Nacional de Atencao ao Idoso envolve o processo de cuidar no ciclo de vida e nos varios niveis de atencao a saude, onde se inclui a saude do idoso, assim como a participacao na elaboracao e implementacao dessa politica.

The aims of this study were: to identify aged persons who were victims of Cerebrovascular Accident and received care at the Emergency Unit of a Public Hospital in Ribeirão Preto-SP, Brazil, and their respective families, as well as to identify the changes in family relations that occurred after the event. In order to identify these changes, the adapted Critical Incident Technique was used, while the analysis was based on the idea of Current of Thought. The sample consisted of 11 families, totaling 34 participants. The analysis of the consequences displayed the changes in family relations, which made up 13 subcategories, 5 of which were positive and 8 negative, totaling 58 alterations, 30 of which were positive and 28 negative. Sobrecarga dos cuidadores de idosos com acidente vascular cerebral.

The aim was to assess the burden on caregivers of elderly victims of cerebrovascular accident (CVA) and to correlate it with care hours, the elderly peoples age and functional independence. This cross-sectional study counted the participation of 62 elderly individuals with CVA and their caregivers. The instrument used contained socio-demographic and economic variables, the Mini-Mental Status Examination, the Functional Independence Measure (FIM) and the Zarit Scale. The possible correlations between the Zarit scale scores and the other variables were assessed using Pearsons Correlation Coefficient. Most caregivers were adults, children of the victims, married, and female. The mean Zarit score was 34.92(15.8). The FIM showed a negative correlation with caregiver burden, but no correlation with age and number of care hours. Fragilidade de idosos internados na clínica médica da unidade de emergência de um hospital geral terciário

Este estudo objetivou caracterizar os idosos internados na clinica medica de um hospital terciario, segundo variaveis sociodemograficas, e identificar a fragilidade nos mesmos. Estudo descritivo e transversal. A coleta de dados foi realizada no periodo de outubro de 2010 a marco de 2011, utilizando-se um instrumento contendo questionario sociodemografico e a EdmontonFrailScale. Participaram 84 idosos, media de idade 73, 8 anos; 60, 7% eram homens, 44, 0%, casados e 32, 1%, viuvos; 75, 0%, aposentados; 54, 8% estudaram entre um e quatro anos. Quanto a identificacao da fragilidade, 42, 9% apresentaram fragilidade severa, 33, 3%, leve e 19, 0%, moderada. Dentre os idosos que apresentaram fragilidade severa, houve predominio de mulheres (63, 6%), de idosos com 80 anos ou mais (63, 0%), dos que viviam sem companheiro(a) (53, 1%) e dos que descreveram sua saude como ruim (75, 8%). Acredita-se que identificar a fragilidade em idosos hospitalizados auxilia os profissionais da saude no planejamento e na implementacao da assistencia ao idoso.This descriptive and cross-sectional study was aimed at classifying elderly patients admitted to the medical clinic of a tertiary hospital, according to socio-demographic variables, and identifying their frailty. Data collection was carried out in the period between October 2010 and March 2011, through a questionnaire with socio-demographic data and the Edmonton Frail Scale. Eighty- four elderly patients with an average age of 73.8 participated in the study; 60.7% were male, 44.0% married and 32.1% widowers/ widows; 75.0% were retired; 54.8% possessed between one and four years of formal education. Regarding the identification of frailty, 42.9% presented severe, 33.3% mild and 19.0% moderate frailty. Among the elderly who presented severe frailty, 63.6% were women, 63.0% were over 80 years of age, 53.1% lived without a partner and 75.8% considered their health poor. It is believed that identifying the frailty of elderly patients helps healthcare professionals in planning and implementing care actions for the elderly. DESCRIPTORS: Aging. Hospitalization. Accidental falls in the elderly and their relation with functional capacity

AIM This study aimed to determinate the prevalence of falls in the elderly and its relationship with the functional capacity. METHOD This is an epidemiological and cross-sectional study; a two-stage cluster sample of 240 male and female subjects aged over 60 years was used. Data were collected from November 2010 to February 2011. The following questionnaires were used: socio-demographic profile, assessment of falls, Functional Independence Measure, Lawton and Brody Scale. Significance was set at 0.05. To identify the occurrence of falls and their relation with functional capacity, the prevalence ratio and prevalence odds ratios were used, as well as multiple logistic regression. RESULTS Average age was 73.5 years (±8.4); 25% 80 years or more, with preponderance of female gender; 48.8% attended school between 1-4 years. The average was 1.33 falls (±0.472), with prevalence in women and elderly between 60 and 79 years old; the most frequently sites were the backyard and bathroom. Strong correlation between the level of functional independence and instrumental activities and age was found, but no relation between elderly victims of falls and the gender and age variables. CONCLUSION Women who suffered falls related to functional independence were predominant, which can be prevented through elderly health promotion strategies, a policy that serves to offer living conditions to people in the aging process.OBJETIVO: determinar a prevalencia de quedas em idosos e sua relacao com a capacidade funcional. METODO: trata-se de estudo epidemiologico transversal de base populacional, com uma amostra por conglomerado de duplo estagio de 240 sujeitos, com idade acima de 60 anos, de ambos os sexos, residentes em Ribeirao Preto, SP. Os dados foram coletados entre novembro de 2010 e fevereiro de 2011 e utilizaram-se os questionarios: perfil social, avaliacao de quedas, Medida de Independencia Funcional e Escala de Lawton e Brody. Foi adotado o nivel de significância de 0,05. Para a identificacao da ocorrencia das quedas e sua relacao com a capacidade funcional, foram utilizadas razao de prevalencia e de chances de prevalencia e regressao logistica multipla. RESULTADOS: a media de idade foi de 73,5 anos (±8,4), 25% com 80 anos ou mais, predominio do sexo feminino; 48,8% estudaram de 1 a 4 anos. Media de 1,33 quedas (±0,472); com maior prevalencia em mulheres e idosos mais jovens; o local mais frequente foi o quintal e o banheiro. Houve forte correlacao entre o nivel de independencia funcional e as atividades instrumentais com a idade, e nao houve relacao entre os idosos que sofreram queda e as variaveis sexo e idade. CONCLUSAO: houve predominio de mulheres que sofreram quedas relacionadas a independencia funcional, podendo-se prevenir com estrategias de promocao a saude ao idoso, politica essa para oferecer condicao de vida a pessoa no processo de envelhecer.

Functional independence in patients with chronic kidney disease being treated with haemodialysis

PURPOSE This study has described and analysed the functional independence of the patients served in the haemodialysis services of a countryside town in the State of São Paulo, Brazil, using the Functional Independence Measure (FIM). METHOD The population considered was that of 214 patients being treated with haemodialysis, assessed in 2011, by means of a social, demographic and clinical report, a Mini-Mental State Examination (MMSE) and also the FIM. RESULTS The mean age of the population under study was 58.01 years, while the mean FIM point score was 118.38 points, showing a level of complete or modified independence within this population. Even though the level of dependence found has been low, this can be highlighted, within the locomotion domain, in the activity of going up and down stairs (10.28%). Age, complications arising from haemodialysis, and comorbidities show a negative correlation with FIM. CONCLUSION Awareness of the level of functional independence of the patients being subjected to treatment with haemodialysis is essential in order to back up intervention for the improvement of nursing assistance provided to this population.OBJETIVO: este estudo teve como objetivo descrever e analisar a independencia funcional dos pacientes atendidos nos servicos de hemodialise, de uma cidade do interior paulista, segundo a Medida de Independencia Funcional (MIF). METODO: a populacao foi de 214 pacientes em tratamento hemodialitico, avaliados em 2011, por meio de um questionario sociodemografico e clinico, Miniexame do Estado Mental e MIF. RESULTADOS: a idade media da populacao do estudo foi de 58,01 anos e a media da MIF foi de 118,38 pontos, evidenciando nivel de independencia completa ou modificada dessa populacao. Mesmo tendo sido baixo o nivel de dependencia encontrado, essa dependencia destacou-se no dominio locomocao, na atividade subir e descer escadas (10,28%). A idade, complicacoes relacionadas a hemodialise e comorbidades apresentaram correlacao negativa com a MIF. CONCLUSAO: conhecer o nivel de independencia funcional dos pacientes em tratamento hemodialitico e primordial para subsidiar intervencoes para a melhoria da assistencia de enfermagem prestada a essa populacao.
