Influence of immediate and permanent obturators on facial contours: a case series

IntroductionRehabilitation of patients after surgical removal of carcinomas in facial skeleton is one of the most difficult therapies of the stomatognathic system. Significant deformation of tissues, dysfunctions of the stomatognathic system with concurrent biological imbalance of the oral cavity environment frequently affect the treatment to become arduous. Scars and contraction of the oral crevice may cause serious psychological deficiencies that are another aspect of the treatment schedule.Case presentationThree Turkish patients ages 46 (male), 61 (male) and 24 (female) who experienced similar operations were rehabilitated with maxillary obturators. The situations was ideal for patient no 1. Patient no 2 could not receive an immediate obturator and patient no 3 rejected using permanent obturator. The paper describes the advantages of a surgical obturator which is constructed before operation and inserted immediately following partial maxillectomy and expresses long term complications when neglecting the use of definite obturator prosthesis, in the light of three cases.ConclusionThe primary objective of oral-maxillofacial and plastic surgeons and prosthodontists when treating tumors is to eliminate disease and to improve the quality of life including the facial contours which influences the psychological condition of patient. Neglecting immediate obturator construction may cause serious facial appearance problems due to soft tissue contracture. When permanent obturator is rejected, serious contracture of soft tissues and facial disharmony is inevitable.

Esthetic rehabilitation of crowded maxillary anterior teeth utilizing ceramic veneers: a case report

IntroductionIn attempting to provide a solution for cases that have been compromised by malpositioned anterior teeth, clinicians traditionally decide orthodontic approach. Nevertheless, in some cases changes in tooth morphology, tooth size, and shape is compulsory for optimal aesthetics. The advent in contemporary aesthetic materials and preparation techniques empowered the clinicians to deliver a promising result with minimal biologic cost. This article presents the clinical considerations that must be addressed when providing a prosthetic restoration for crowded teeth.Case presentationA 23 years old Turkish female patient who had malaligned anterior teeth was rehabilitated with ceramic veneers due to her rejection of orthodontic treatment. The paper describes ceramic veneers can be an alternative treatment to orthodontic therapy under variety of preparation forms when patient has anterior malalignment and rejects the orthodontic treatment.ConclusionThe new smile line was satisfactory for the patient and the esthetics was considered as excellent.

Fracture strengths of chair‑side‑generated veneers cemented with glass fibers

INTRODUCTION CAD/CAM (computer-aided design and computer-aided manufacturing) systems have refreshed the idea of chair-side production of restorations, but the fracture of ceramic veneers remains a problem. Cementation with glass fibers may improve the fracture strengths and affect the failure modes of CAD/CAM-generated ceramic veneers. Therefore, this study compared the fracture strengths of ceramic veneers produced at chair side and cemented with or without glass fibers with those of composite veneers. METHODOLOGY Thirty intact mandibular incisors were randomly divided into three groups ( n = 10) and treated with CAD/CAM-fabricated veneers cemented with dual-cure composite resin luting cement (CRLC; Group 1), CAD/CAM-fabricated veneers cemented with a glass fiber network (GFN) and dual-cure CRLC (Group 2), and a direct particulate filler composite veneer constructed utilizing fiber and a restorative composite resin (Group 3). The specimens were tested with a universal testing machine after thermal cycling treatment. RESULT The loads at the start of fracture were the lowest for traditionally fabricated composite veneers and higher for CAD/CAM-generated. Veneers cemented either without or with the GFN. The failure initiation loads (N) for the veneers were 798.92 for Group 1, 836.27 for Group 2, and 585.93 for Group 3. The predominant failure mode is adhesive failure between the laminates and teeth for Group 1, cohesive failure in the luting layer for Group 2, and cohesive laminate failure for Group 3, which showed chipping and small fractures. CONCLUSION Ceramic material is a reliable alternative for veneer construction at chair side. Fibers at the cementation interface may improve the clinical longevity and provide higher fracture strength values.

The esthetic rehabilitation of misplaced dental arch after fracture of anterior maxillae: a case report

IntroductionIn cases where the patient experiences trauma, the teeth may be fractured or a bone segment could be fractured causing misplaced teeth. Ceramic veneer restorations may be a solution for re-establishing the ideal position. The treatment of misplaced dental arch segment is presented and treatment options are discussed.Case presentationA 30-years-old female Turkish patient had a history of a dentoalveolar trauma and a surgical operation. Her main complaints were about the unpleasant appearance of her anterior teeth. The maxillary anterior teeth were positioned labially and had irregularities in vertical position inharmonious with each other and the horizontal plane. The misplaced sector treated with minimally invasive dentin bonded porcelain laminate veneers.ConclusionCeramic veneers can be a solution for patients with malpositioned anterior teeth even the situation is severe and excessive tooth reduction is needed.

Farklı Yapıştırma Simanlarında Marjinal Aralığın Değerlendirilmesi

Calismamizin amaci, 3 farkli metal altyapi uretim teknigi kullanilarak elde edilen 3 uyeli sabit metal-seramik koprulerin 3 farkli simanla yapistirilmasinin ardindan meydana gelen marjinal aciklik farklarinin karsilastirilip ve degerlendirilmesidir. Ornekler 3 farkli fabrikasyon teknigiyle (induksiyon dokum, milleme, lazer sinterleme) elde edilip 9 grupta degerlendirildi. Metal-seramik koprulerin marjinal aciklik degerleri simantasyon oncesi kaydedilip ardindan simantasyon sonrasi kayitlarla karsilastirildi. Degerlendirme icin her bir kron marjininin 40 farkli noktasindan kayitlar stereomikroskop kullanilarak alindi. Calismamizin sonuclarina gore induksiyon dokum teknigi ile elde edilen grup disindaki tum gruplarin aciklik degerleri klinik olarak kabul edilebilir sinirlar icindedir (p<0.05). Simantasyon yapilan orneklerin tamaminda marjinal araligin arttigi belirlendi; ancak bu degerler klinik olarak kabul edilebilir sinirlar icindedir.

Seramik Veneerler: Ağız İçi Tamir İçin Koruyucu Bir Seçenek

Seramik materyaller oral kavitede okluzal kuvvetler, isisal genlesme katsayilari arasindaki fark, hatali tasarim, materyaldeki mikrodefektler ve travma nedeni ile mekanik basarisizliga ugrayabilirler. Pahali ve zaman alan bir prosedur olmasi nedeni ile protezin tekrar yapilmasina alternatif olarak cesitli tamir teknikleri gelistirilmistir. Agiz ici tamir sistemleri, sabit bolumlu protezlerin hasta agzinda direk tamirini saglar. Cogunlukla, partikul dolduruculu kompozitlerin estetik ozellikleri ve uygulama kolayligi nedeniyle agiz ici tamirlerde tercih edilen materyaldir. Kompozitler, seramik materyaller ile kiyaslandiklarinda daha kolay asinmalari ve renk stabilitelerinin iyi olmamalari nedeniyle, uygulama alanlari daha kisitli olabilir. Bu olgu sunumunun amaci, 8 uyeli metal seramik sabit bolumlu protezin agiz ici tamirini sunmaktir. Sag maksiller lateral kesici insizal bolgesindeki seramik materyaldeki koheziv kirik, seramik veneer kullanilarak tamir edilmistir. Ayrica bu raporda tedavi alternatiflerinin avantajlari ve olasi kisitlamalari tartisilmaktadir. Agiz ici metal-seramik sabit bolumlu protezler dogrudan hasta agzinda restorasyona zarar vermeden estetik olarak seramik veenerler ile tamir edilebilir.
