The classification of clubfoot a practical approach

Abstract Clubfoot, a very common congenital deformity, remained controversial regarding its nomenclature, etiology, patho-anatomy classification, management and prognosis. A practical grading system, based only upon clinical assessments, has been tried on 1966 clubfeet. The observations of over 29 years have endorsed this system as being quickly performed, adequately helpful in planning the treatment and predicting the prognosis to a fairly good extent.

Pigmented villonodular synovitis with bone involvement

SummaryPigmented villonodular synovitis with bone involvements have been reported at various sites. The mode of bone involvement is still controversial. In the present report, out of 47 cases of pigmented villonodular synovitis there were six osseous involvements (at ankle, wrist, elbow, scapula, hip and knee) with bizarre manifestations. The lesions at ankle, wrist and elbow were peculiar, mimicking neoplasm. All cases were treated by operative methods.ZusammenfassungÜber pigmentäre villonoduläre Synovitis mit Knochenbeteiligung wurde verschiedentlich in der Literatur berichtet, wobei der Modus des Knochenbefalls noch unterschiedlich beurteilt wird. Im vorliegenden Bericht werden aus einem Krankengut von 47 Fällen mit pigmentierter villonoduldrer Synovitis 6 mit Knochenbeteiligung (Sprunggelenk, Handgelenk, Ellenbogengelenk, Scapula, Hüftgelenk und Kniegelenk) und einer ungewöhnlichen Manifestation beschrieben. Die Veränderungen am Sprung-, Hand- und Ellenbogengelenk waren durchaus mit einer bösartigen Geschwulst zu verwechseln. Alle geschilderten Fälle wurde operativ behandelt.

Soft tissue release in clubfoot by double incision.

A procedure for soft tissue release through double incisions in clubfoot is described. It has been practiced in 287 cases of severe, neglected, resistant, and relapsed clubfeet in the last 7 years. Generally, it has been found to be a useful approach without risking any skin necrosis.

Calcaneal osteotomy and tendon sling for the management of calcaneus deformity

A one-stage operation for severe post-poliomyelitic calcaneus deformity was done in seventy-seven feet of seventy-seven patients. The operation consisted of osteotomizing the calcaneus to increase the lever arm and to reshape the hind part of the foot and using a posterior sling formed from the peroneus brevis and tibialis posterior to provide plantar-flexion power. We were able to evaluate sixty-six patients more than three years after the operation. Most of the patients had improvement in the appearance and function of the foot as a result of the operation.

Another unique feature is the coverage of clinical problems of the tropics and developing countries. With advances in communication, borders and boundaries are little more than lines on maps. This book is, therefore, an important practical reference for students and all frontline workers in orthopaedics and traumatology.

Further experiences of Küntscher's nailing for tibial shaft fractures

SummaryWe present the experience of managing 429 fractures of the tibia fixed with straight Küntscher rods. The overall impression is quite encouraging because this method involves a short and easy operation with limited exposure and provides sound fixation. Complications have been negligible.
