Antenatal prevention of atopic dermatitis associated with food allergies

To establish effect of probiotic Lactobacillus reuteri which is used in women in the last pregnancy trimester, prevalence of atopic dermatitis in children of the first 6 months of life. One group of women took Lactobacillus reuteri as chewing tablets of probiotic Lactobacillus reuteri, with dairy-limited diet excluding walnuts prior delivery. The second group of pregnant women took only proposed diet. Cumulative prevalence of atopic dermatitis was lower in group of children which mothers took Lactobacillus reuteri (6.7%) during pregnancy than in group of children of mothers from control group (24.2%). Analysis of the clinical features of disease showed that in children of the main group was observed mild atopic dermatitis (SCORAD amounted to 12,3+0.14 points). In children of the control group was middle and severe of atopic dermatitis (SCORAD 22-52 points, respectively). The use of Lactobacillus reuteri by women in the last pregnancy trimester was positive for general condition and status of gastrointestinal tract. Pregnant women which took Lactobacillus reuteri had increased metabolic activity of lactic flora and recovered balance between aerobic/anaerobic microorganisms. Disorders of intestinal microbial balance in pregnant women in control group were not recovered to the end of pregnancy.

P41 - Preexposure prophylaxis of infants’ food allergy

Methods We observed 100 “mother and child” pairs, which were divided into two groups. All children had AD, associated to cow milk protein (CMP) allergy. Lactating mothers from both groups were treated by diet therapy with the replacement of the cow milk to the New Zealand goat milk (1 group: 43 “mother and child” pairs) or non-dairy diet (2 group: 57 “mother and child” pairs). The effect of diet therapy was assessed by dynamics of clinical and immunological AD symptoms in infants after 1-3 months of treatment. Immunological effect of the therapy was assessed by the dynamics of levels of allergen-specific IgE and IgG to CMP, casein, b-lactoglobulin and goat milk protein which were measured by uncompetitive immunoenzyme assay on special test-systems from Allergopharma (Germany). The level of IFN-g, IL-2, IL-4, IL-5, IL-12, IL-13 was measured by immunoenzyme ELISA method.

P97 - Characteristic of protein sensibilisation in infants

Methods We examined 85 infants (39 girls, 46 boys), 1.5 – 18 months old, who were fed by artificial milk formulas. Gastrointestinal symptoms of food allergy were diagnosed in 45 (52.9%) children with diarrhea and in 40 (47.1%) children with constipation. The control group consisted of 25 healthy infants of the same age. The level of total IgE, cow and goat milk proteins IgE allergenspecific antibodys in coprofiltrates were measured by immunoenzymometric method using the spectrophotometer “Sunrise” (Belgium), with test-systems “Allergopharma” and “Dr. Fooke” (Germany).

P99 - Clinical effectiveness of the goat milk-based formula with prebiotics in infants with atopic dermatitis

Methods The examination included 50 formula-fed infants with AD aged 6-12 months old. The main group (n = 30) comprised infants, who received the goat milk-based formula with prebiotics. The reference group (n = 20) – infants, who received a formula based on soy protein isolates. All the patients received standard therapy for AD (antihistamine drugs, local anti-inflammatory therapy, therapeutic and cosmetic skin care, etc.) that did not differ in both groups. The effectiveness of the administered therapy was assessed on the basis of the dynamics of clinical symptoms of disease according to SCORAD score, and also by decrease of total IgE level in serum. The level of total IgE was measured by immune-enzymometric method using the spectrophotometer “Sunrise” (Belgium).

Algorithm of the choice of nutritional therapy in infants with food allergy

Methods We examined 50 formulae fed infants 1-12 months old with food allergy. The examination included a collection of allergy history, clinical evaluation and specific methods – the measurement of the coprofiltrate levels of total IgE and allergen-specific IgE to the cow milk protein and casein. The level of total IgE, cow and goat milk proteins IgE allergen-specific antibodies in coprofiltrates were measured by immunoenzyme method using the spectrophotometer “Sunrise” (Belgium), with testsystems “Allergopharma” and “Dr. Fooke” (Germany). The nutritional therapy of the food allergy in infants consists in replacement of cow milk protein formula on the formula based on the goat milk. The estimation of the efficiency of therapy based on dynamics of the clinical symptoms of the disease and decrease of the serum levels of total IgE and allergen-specific IgE to the cow milk proteins.

Characteristics of ante- and postnatal risk factors of the food allergy in infants

Background Risk factors which promote the development of allergy to the cow milk protein in infants include antenatal sensitization, bottle feeding of neonates, early start of bottle or mixed feeding, postnatal sensitization of mite and pollen allergy. The aim of the study was to analyze a family history of the allergic diseases, course of pregnancy and delivery in mothers, effect of concomitant pathology on the development and course of food allergy in infants.
