A 17‐year‐old woman presented with asymptomatic, multiple plaques around the right mandibular area of the face extending from the cheek to the neck. These lesions had gradually increased in size and number for 8 years, and then were stationary. When she came to us, there were a few skin‐colored or pale yellowish papules and plaques ranging from 0.2 to 1 cm in diameter (Fig. 1). The patient stated that initially the papules developed discretely, and then these papules were confluent and grouped slowly with new lesions. The most recent lesions were skin‐colored papules which were soft to the touch, the older lesions were yellowish plaques with firm consistency and a smooth shiny surface. The patients general condition had been good at the initial visit and the laboratory findings including blood cell counts, urinalysis, and routine blood chemistry tests were all within normal limits. A roentgenogram of the mandibular area did not reveal any significant findings. The presumptive clinical diagnosis of connective tissue nevus was made.

Hyperkeratotic spicules are a rare cutaneous disorder of unknown origin defined by the presence of multiple foci of circumscribed hyperkeratosis with a column of keratotic material protruding from the stratum comeum. 1 It has been described in association with multiple myeloma 2-1° and other underlying diseases, lt-16 We report an idiopathic case of follicular hyperkeratotic spicules with no evidence of an associated illness.

A 65-year-old male patient who has taken antituberculous drugs including cycloserine for 8 months showed multiple pruritic, violaceous, flat-topped papules and plaques on his back which had developed 5 months into therapy. Histopathologic findings were consistent with lichenoid drug eruption. Patch test with the drugs revealed a strongly positive reaction to cycloserine only. Four months after discontinuation of oral cycloserine, pruritus resolved and the lesions subsided without any new lesions. Cycloserine is one of the antituberculous drugs. Skin manifestations resulting from its administration are reported to be rare, but to our knowledge, this is the first case report of lichenoid drug eruption from its use.

We report a patient with pityriasis lichenoides et varioliformis acuta (PLEVA) with atypical clinical features, which was treated successfully with PUVA therapy. A 23-year-old man had discrete multiple erythematous scaly papules and patches on the trunk and extremities. New lesions developed abruptly within 1 month, forming generalized annular erythematous plaques with yellowish scales on the border. Skin biopsy specimens showed the typical histopathological findings of PLEVA. The skin lesions completely resolved with 8 months of PUVA therapy.
