Sleep quality in rheumatoid arthritis: relationship between the disease severity, depression, functional status and the quality of life.

Background The aim of this study was to evaluate sleep quality and the related variables in patients with rheumatoid arthritis (RA). Methods Ninety-four patients diagnosed with RA and fifty two healthy controls were enrolled in the study. Disease activity was assessed through the Disease Activity Score (DAS) 28 scale. All patients were assessed using the Rheumatoid Arthritis Quality of Life and Health Assessment Questionnaire scales, together with the Beck Depression Inventory. Radiological damage was calculated with the modified Larsen method. The Pittsburgh Sleep Quality Index (PSQI) was used for the evaluation of the sleep disturbance. Results The patients with RA had significantly higher scores in the subjective sleep quality, sleep latency, habitual sleep efficiency, sleep disturbance domains and the total PSQI score compared to the healthy control group. According to the results of Spearman’s analysis, there was a significantly correlation between the age, disease activity, CRP, pain, fatigue, depression, functional disability, quality of life, radiological damage, menopause status, duration of morning stiffness, ESR levels and the sleep disturbance. The logistic regression analysis indicated that depression and DAS 28 scores were predictors for poor sleep quality. Conclusion The sleep quality is disturbed in patients with RA. The poor sleep quality is especially associated with the disease activity and depression.

The Relationship of Metabolic Syndrome With Disease Activity and the Functional Status in Patients With Rheumatoid Arthritis

Background The aim of this study is to investigate the frequency of metabolic syndrome (MS) in patients with rheumatoid arthritis (RA) and to determine the relationship between the clinical and laboratory parameters of RA and the components of the metabolic syndrome (MS). Methods Fifty-four patients with RA and 52 healthy individuals were enrolled in this study. The diagnosis of rheumatoid arthritis was based on the American College of Rheumatology criteria and the diagnosis of the metabolic syndrome was made according to the criteria set out in the American Heart Association/National Heart, Lung, and Blood Institute (AHA/NHLBI). The functional status and disease activity were also recorded in patients with RA. Results MS was diagnosed in 42.6% of the patients with RA and in 9.6% of the healthy controls. The systolic and diastolic blood pressure values were observed to be significantly higher in the patients with RA in comparison to the controls. Also, the frequency of MS was higher in the inferior functional group in relation to the higher functional group. A positive correlation was observed between the DAS28 scores and hypertension in patients with RA. Conclusions In this study, MS was more frequently detected in the patients with RA compared to the control group. Also, an inferior functional status in RA was also found to be associated with the presence of MS. Thus, the presence of MS in patients with RA may be associated with a higher cardiovascular risk.

High Frequency of Fibromyalgia in Patients With Acne Vulgaris

Objectives This study aims to investigate the frequency of fibromyalgia syndrome and to specify fibromyalgia syndrome-associated clinical symptoms in patients with acne vulgaris. Patients and methods Eighty-eight patients (28 males, 60 females; mean age 23.2±5.1 years; range 18 to 40 years) with acne vulgaris and age, sex- and body mass index-similar 76 healthy controls (14 males, 62 females; mean age 24.5±2.9 years; range 18 to 35 years) were included. Acne vulgaris was evaluated by using the Global Acne Scale, while Hospital Anxiety and Depression Scale was used to evaluate anxiety. Results Fibromyalgia-associated pain, sleep disturbance, anxiety, and menstrual cycle disturbance were significantly more frequent in patients with acne vulgaris than controls. Also, the severity of anxiety and the number of tender points were significantly higher in the acne vulgaris patients than controls. Conclusion This study indicates that patients with acne vulgaris have increased frequency of fibromyalgia syndrome than healthy controls (21.6% versus 5.3%, respectively).

Novel FGF10 mutation in autosomal dominant aplasia of lacrimal and salivary glands.

ObjectiveAplasia of lacrimal and salivary glands (ALSG) is a rare autosomal dominant inherited disease, characterized by aplasia, atresia, or hypoplasia of the lacrimal and salivary systems with variable expressivity. The purpose of this study was to identify genetic etiology of an ALSG family.Materials and methodsWe recruited a Turkish family with ALSG and performed a mutational analysis, based on the candidate gene approach, to clarify the molecular genetic etiology.ResultsThe candidate gene sequencing of the FGF10 gene identified a novel heterozygous nonsense mutation (c.237G > A, p.Trp79*) in the exon 1.ConclusionThe identified novel mutation would result in a haploinsufficiency of the FGF10, because of nonsense-mediated mRNA decay caused by a premature stop codon. This report further confirms that ALSG is caused by the haploinsufficiency of functional FGF10.Clinical relevanceIdentification of the genetic etiology of the ALSG will help both the family members and dentist understand the nature of the disorder. Therefore, it will positively motivate oral health care to avoid further destruction of the tooth due to the lack of salivary production.

Catecholamine levels in a Ramadan fasting model in rats: a case control study

ABSTRACT Eating habits as well as physical exercise are very important for a healthy lifespan. Ramadan-type fasting, which is food and water avoidance during the daylight period for four weeks, has drawn attention due to its positive impacts on metabolism and health. The aim of this study was to compare the blood and urine catecholamine (CA) levels in fasting and non-fasting rats, in terms of stress response. A total of 20 male rats were randomly divided into a fasting group and a control group. Four weeks later, blood and urine samples were taken after decapitation. Analysis of CAs was done using high-performance liquid chromatography with florescence detection (HPLC-FLD). The dopamine (DA), adrenaline (ADR) and noradrenaline (NA) blood and urine concentrations were found to be higher in the fasting group compared to the control group, but the difference was statistically significant only for the blood DA levels (p < 0.05). In the fasting group, the blood values of ADR and NA correlated with each other but not with the DA levels, whereas there was correlation among the urine levels of DA, ADR and NA. In the control group, the blood and urine values of DA, ADR and NA correlated with each other. The differences observed in the blood and urine CAs indicate a specific regulation of CAs in Ramadan-type fasting, which needs to be investigated thoroughly in future studies.

The Characteristics of Families who rejected Vaccination during the Mop-up Oral Polio Vaccination Campaign and Their Reasons of Rejection: A Family Health Center Experience

OZET Amac: Mop-Up Polio asi kampanyasi, cocuk felcinin ortadan kaldirilmasi icin uygulanan asilama calismalaridir. Bu calismada amacimiz, Diyarbakir’da bir aile sagligi merkezi bolgesinde Mop-up oral polio asi (OPA) kampanyasi kapsaminda cocuklarina OPA yapilmasini reddeden ailelerin ozelliklerinin ve asiyi reddetme nedenlerinin belirlenmesidir. Yontem: Veriler Mop-up OPA kampanyasinda, cocuklarina asi yapilmasini reddeden ailelerle yuz yuze gorusulerek elde edildi. Anket formu; aile ve cocuklarin sosyo-demografik ozellikleri ve OPA’nin reddedilmesi ile ilgili 14 sorudan olusturuldu. Bulgular: Kampanya boyunca 1250 cocuk asi programina alindi. Bunlardan 38 tanesi (%3.04) asiyi reddetti. Asisi yaptirilmayan bu cocuklarin yas ortalamasi 23.2±17.7 ay idi. En kucugu 3 gunluk ve en buyugu ise 59 aylik idi. Ankete cevap veren aile bireyleri incelendiginde; %71’ini cocugun annesi (n=27), %8’ini cocugun babasi (n=3) ve %21’ini ise cocugun diger yakinlari (n=8) olusturmaktaydi. Asiyi reddeden ailelerin cocuklarinin %94.7’sinin diger asilari; Saglik Bakanligi Ulusal asi takvimine gore tam iken (n=36), %5.3’unun ise diger asilarinin eksik oldugu (n=2) tespit edilmistir. Cocuklarin %81.6’sina daha once OPA yapilmis iken (n=31) %18.4’une ise hic yapilmamistir (n=7). Sonuc: Asilama faaliyetlerinin yurutulmesinde ebeveynlerin egitim seviyeleri ve farkindalik duzeyleri hedeflenen asilama oranlarina ulasilmasi icin onemlidir. Yapilacak asi kampanyalarinda ailelerin asilama konusunda egitilmesi ve farkindaliklarinin arttirilmasinin asi retlerini azaltacagi kanaatindeyiz.

Delayed diagnosis of selective immunoglobulin deficiency: A case report

Selective immunoglobulin A deficiency (SIgAD) is the most common of all primary immunodeficiency diseases; however, the pathogenesis has not been fully understood. Although most people with SIgAD are asymptomatic, some may present with recurrent infections; such as respiratory disorders, gastrointestinal tracts disorders, and allergic disorders. Herein, we report a case with SIgAD who presented with the complaints of pruritus, dental caries and chronic sinopulmonary infections.

Sigara kullanıcılarda nikotin bağımlılık düzeyinin yaş ve cinsiyetle ilişkisi: Diyarbakır örneklemi

Objective: It was aimed to find out whether there is a relation between nicotine addiction level to be found with Fagerström Nicotine Addiction Test (FNAT) and age-gender among the smokers chosen from the patients who have applied to Dicle University Medicine Faculty Clinic of Family Medicine. Methods: Our study was made prospectively by using FNAT to measure nicotine addiction level and sociodemographic data forms, which were filled face to face among the smokers above 20 years old who applied to the clinic of Family Medicine between 15.08.2014 and 15.10.2014 for any other reasons. Results: 9 out of 151 patients who smokers were excluded because of missing data form filled. 108 (76.1%) of the remaining 142 participants were male and 34 (23.9%) were females. While the general age average was 41.54 ± 9.80, the age average was 41.50 ± 10.04 for males and 41.67 ± 9.30 for females. There was no meaningful difference between ages considering the gender (p>0.05). Average cigarette consumption for males was 32.18 ± 21.64 packet/year and 22.55 ± 16.29 packet/year for females, which shows a meaningful difference (p<0.05). Among all the participants, there could not be found a meaningful difference between age groups and nicotine addiction levels. Considering the genders, there was no meaningful difference between nicotine dependence levels (p>0.05). Conclusion: In our nicotine addiction level, study made with FNAT, there was no meaningful difference addiction level and age-gender. Smoking among males was three times as high as smoking among females. The amount of cigarette consumption was found higher in elder ages since smoking had been started at early ages.
