Energetic recovery from hypothermic preservation in the rat liver

Changes in energy metabolism induced by norepinephrine were observed in liver perfused immediately after dissection and in liver stored in Euro-Collins solution at 0 degrees C for 24 hr. Oxygen consumption, glucose output, ATP content, and tissue pH were measured in isolated perfused rat liver at 37 degrees C. In fresh liver, a two-fold increase in glucose output and oxygen consumption was induced by norepinephrine (1 microM), while changes in ATP content and tissue pH were minimal. In stored liver, the cumulative increments in glucose output and oxygen consumption induced by norepinephrine were 30 and 27% those of fresh liver, respectively. ATP content of the unstimulated liver was 76% that of the fresh liver, then decreased to 50% during stimulation. A transient decrease in tissue pH (0.14 pH unit) was significant compared with that of fresh liver. These results suggest that norepinephrine stimulation is useful for assessing energetic and functional recovery of reperfused liver following hypothermic storage.

Clinicopathological Studies of Central Type of Cholangiocarcinoma Compared with Hilar Bile Duct Cancer.

胆管細胞癌中枢型肝切除例10例と肝門部胆管癌肝切除例13例を比較検討した.腫瘍の肉眼型は, 胆管細胞癌10例中9例では肝内に明らかな腫瘤を形成し周囲の肝実質に浸潤性に発育していたのに対し, 肝門部胆管癌では腫瘤形成よりも胆管壁に沿う浸潤が主体であった.腫瘍の体積は, 胆管細胞癌では肝門部胆管癌に比較して有意に大きかった.胆管細胞癌では, 肝門部胆管癌に比較して大血管浸潤のほかに肝胆道の周囲臓器への浸潤を高率に認めた.胆管細胞癌でも12, 13, 8番へのリンパ節転移, ly, v, pn (+) を肝門部胆管癌と同じく高率に認めた.ew (+) は両者とも80%以上の高率であり, 主にly, v, pnの存在によった.hw (+) は肝門部胆管癌で有意に高率であった.肝門部胆管癌の5年生存率は62.5%と良好であったが, 胆管細胞癌では3年生存率17.9%と不良であった.両者は同一の規約で検討されるべき多くの類似点を有しているが, 進展様式, 予後などに違いが認められた.

閉塞性黄疸肝肝切除17例を対象として, 術後経過に影響をおよぼすと考えられる術前因子 (肝切除率, 術前血清総ビリルビン値, 減黄率b値, 血清アルブミン値, プロトロンビン時間, ヘパプラスチンテスト, ICGR15, K, Rmax, 50g経口糖負荷試験) と術後経過との関連についてretrospectiveに検討を加えた.減黄率b値, 血清アルブミン値, プロトロンビン時間の良好な症例では, 術後順調に経過することが多かった.術前総ビリルビン値, ICGR15が高値を示した症例では手術死亡が多くみられた.50g経口糖負荷試験において, 正常型では全例術後順調に経過したが, linear型では手術死亡例が多く, これらの因子は肝切除術の術後経過を反映していると考えられた.

は じめ に 原発 性肝 肉腫 は非 常 に まれ で,診 断 ・治療 に苦 慮 す る こ とが 多い.今 回われ われ は術前 肝生 検 にて原 発性 肝 平滑 筋 肉腫 と診 断 され,肝 右 葉切 除 を施行 した症例 を経験 した ので,若 干 の文 献的考 察 を加 えて報 告す る. 症 例 患 者:58歳,男 性,運 転手. 主 訴:背 部 痛. 家 族歴:特 記す べ き もの な し, 既 往歴:41歳 時,胃 平滑 筋腫(噴 門直 下 の8×5×4cm の粘 膜下腫 瘍)に て噴門 側 胃切除 お よび脾 摘. 現病 歴:昭 和58年10月 背部痛 出現,11月 内科 にて逆 流 性食 道炎 ・食 道潰瘍 を指 摘 され た.昭 和59年1月 腹 部 超音 波検査 で肝 内腫 瘤 を発見 され,精 査 の 目的 で入 院 した, 入院 時現症:体 格 中等,栄 養 良好,血 圧 ・脈 拍 ・体 温 は正常 で,皮 膚 お よび可 視粘膜 に貧血 ・黄疸 は な く, 表在 リンパ節 は触知 せず.手 掌紅 斑 ・クモ状血 管腫 は 見 られ なか った,胸 部 には異 常 な く,腹 部 は平 担で 軟 らか く,肝 は触 知せ ず,波 動 も認 め なか った. 入 院時検 査所 見(表1):便 潜 血反 応 陽性 ,尿糖 陽性, 表1 入院時検査所見
