Membranous glomerulonephritis mediated by renal tubular epithelial antigen-antibody complex

Abstract Autologous renal tubular epithelial antigen was demonstrated together with immunoglobulins and β 1 c along the glomerular capillary walls in 4 out of 9 patients with idiopathic membranous glomerulonephritis, and the existence of the clinical entity of glomerulonephritis mediated by tubular epithelial antigen-antibody complexes was suggested. Although no differences in laboratory data were noted between tubular antigen-positive and -negative groups, clinical remission occurred only in the negative group. The fact that no remission was observed in the tubular antigen-positive group may be explained by the continuous supply of the antigen from endogenous sources that maintained the production of immune complexes and subsequent glomerular injury.

意識障害, 急性腎不全にて発症した悪性リンパ腫の1例を経験した.症例は48歳, 男性, 主訴は意識障害. 平成元年3月10日, 近医より, 急性腎不全にて紹介され, 入院した. 入院時, 意識は, 呼名に反応する程度で, 瞳孔不同を認めたが, 項部強直, 病的反射等はみられなかった. 血圧は, 122/86mmHg, 脈拍は72/min, 後頸部両側に3-4個, 母指頭大の表在リンパ節を触知した. 入院時検査成績では, Hb13g/dl, RBC 401×104/cm2, WBC 5,300/cm2, Plt 4.2×104/cm2, GOT 78 KU, GPT 10 KU, LDH 2,926 WU, BUN 131mg/dl, Cr 14.3mg/dl, 尿量は1lで潜血 (+), 蛋白 (+), 血液ガスでは, 著明な代謝性アシドーシスが見られた. 第5病日にリンパ節生検, 骨髄穿刺を施行し悪性リンパ腫 (LSG分類では, diffuse large cell, stage IV) と診断した. VP療法を行ったところ, 意識障害および頸部リンパ節腫脹は急速に改善し, LDHも低下し, BUN, クレアチニンも正常化した. 以後, ACMPなどの化学療法を行うも, 次第に治療に反応しにくくなり, 8月14日, 突然強直性の痙攣をきたし, 死亡した. 肝臓, 腎臓のautopsyにて, リンパ腫細胞の浸潤が確認された.

A patient who had lost taste acuity during captopril treatment recovered it after zinc supplement

アンギオテンシンI変換酵素阻害剤であるcaptopril治療中の血液透析患者に顕著な味覚障害が出現した.その味覚低下はcaptopril投与中止と硫酸亜鉛投与で改善した.症例は33歳, 男性の血液透析患者, 1日量37.5mgのcaptoprilを投与されていた.投与開始7週後より高度味覚障害が出現した.電気味覚計検査では, 右大錐体神経領域, 舌咽神経領域, 鼓索神経領域, 左大錐体神経領域のすべてに反応は消失し, 左舌咽神経領域の味覚閾値は26dBで左鼓索神経領域では30dBだった. すべての領域で基本の4つの味覚, 酸味, 甘味, 辛味, 苦味が障害されていた.血清亜鉛濃度は65μ9/dlでcaptoprilを中止した. 発中止7日目, 味覚は自覚的および電気味覚計検査で軽度改善した. captopril中止65日目の全血亜鉛濃度は361μg/dlで, 硫酸亜鉛1日量150mg経口投与した. 投与6日目の電気味覚計検査ではさらに改善がみられ, 自覚的改善は顕著だった.亜鉛投与33日目の電気味覚計検査では, 味覚脱失部位はなくなった.この症例においては, captopril治療で惹起された亜鉛欠乏が味覚障害の原因と考えられた.
