Myofascial pain in childhood.

Summary Myofascial pain with trigger area is a common clinical entity in childhood. Experience with 85 cases representing 23 myofascial pain syndromes is presented, and the syndromes are illustrated. This condition may be precipitated by a variety of stress factors, acute or chronic illness, sudden trauma or repetitive strain, chilling or fatigue. Treatment with ethyl chloride spray and procaine injection is almost uniformly successful, although results are best in the absence of underlying illness. The technique of treatment with ethyl chloride spray is presented and discussed in detail, and mention is made of the technique of injection with procaine.

The diagnosis of nonadrenal female pseudohermaphrodism should be based on the following criteria: (1) the external genitalia should show hypertrophy of the clitoris and some degree of labial fusion; (2) in infants, measurement of 17-ketosteroids or pregnanetriol should be performed to rule out the adrenogenital syndrome; in older patients this syndrome can be ruled out clinically by the lack of precocious puberty and progressive masculinization; (3) laparotomy should reveal a female internal genital apparatus, and (4) adequate biopsies of both gonads should show normal ovaries, excluding the possibility of ovotestes and true hermaphrodism.
