P-982 - Secondary traumatic stress among wives of war veterans with PTSD

Rezultati dosadasnjih istraživanja ukazuju na to da supruge ratnih veterana imaju niz psihickih i tjelesnih simptoma ukljucujuci i simptome sekundarnog traumatskog stresa. S obzirom na broj veterana Domovinskog rata oboljelih od PTSP-a postavlja se pitanje psiholoskog zdravlja njihovih supruga. Cilj ovog istraživanja bio je ispitati razinu simptoma sekundarnog traumatskog stresa (STS), trenutnih psiholoskih simptoma i percipirane kvalitete života i usporedbu između skupina ispitanica ciji su muževi oboljeli od PTSP-a (KS1, N=50), ispitanica ciji muževi nemaju PTSP (KS2, N=50) i ispitanica ciji muževi nisu sudjelovali u Domovinskom ratu (US, N=50). Također, cilj je bio i ispitati povezanost simptoma STS-a, psiholoskih simptoma i percipirane kvalitete života. Istraživanje je provedeno u Centru za psihotraumu Rijeka gdje je sa svim ispitanicama proveden je strukturirani intervju iz kojeg su dobiveni sociodemografski podaci. Sve ispitanice su ispunile upitnik Kratki inventar simptoma i MANSA upitnik za procjenu kvalitete života. Modificirani upitnik za posrednu traumatizaciju ispunjavale su samo supruge ratnih veterana. Dobiveni rezultati govore da 36% supruga veterana oboljelih od PTSP-a zadovoljava kriterij za sekundarni traumatski stresni poremecaj. Ispitanice iz skupine KS1 postižu i znacajno vecu razinu simptoma STS-a, svih ispitivanih simptoma emocionalnog funkcioniranja te znacajno manju percipiranu kvalitetu života u odnosu na ispitanice iz skupina KS1 i US. Rezultati ispitivanja povezanosti pokazuju da supruge veterana koje imaju vecu razinu simptoma STS imaju i vecu razinu svih ispitivanih psiholoskih simptoma te manju razinu percipirane kvalitete žiivota. Klinicari koji se bave psihotraumom trebali bi imati na umu da život s traumatiziranom ratnim veteranom može dovesti do sekundarnih stresnih reakcija kod njihovih supruga. Sistemski psihoterapijski pristup u radu s veteranima Domovinskom rata mogao bi ne samo prevenirati simptome sekundarnog traumatskog stresa vec i poboljsati funkcioniranje primarne žrtve traume i partnersko funkcioniranje.

2723 – Risk and protective factors for suicidality among croatian war veterans with posttraumatic stress disorder

There are limited data about suicidality among Croatian war veterans. The present study is aimed to assess correlation between suicidality and possible risk and protective factors in PTSD patients. Method The sample consisted of 123 war veterans who completed Suicide Behaviors Questionnaire (SBQ-R), Reasons for Living Inventory (RFL-48), General Health Questionnaire - subscale for suicidal behavior (GHQ), Hopkins Symptom Checklist Depression Scale (HSCL-25) and Harvard Trauma Questionnaire (HTQ). Results indicated that suicidality is correlated with higher PTSD and depression scores, and negatively correlated with reported reasons for living. We discuss our results in terms of their use and applicability in everyday work with patients who are in suicidal risk.

1917 – Childhood trauma and developmental processes

Introduction Children can experience traumas in family, school and society, as well as participants in natural disasters, terrorism, war and civilian casualties. Their reactions depend on age, sex, individual ego strength, coping strategies, but also on the reactions of childs surrounding. Objectives Clinical entities associated with the traumatic experience in children are common in psychiatric practice. Frequent manifestations of these experiences are classical clinical picture of PTSD, as well as depression, anxiety, somatization, behavior disorder, dissociative reactions, etc. Aims The childs perception of the traumatic event, and subsequent developing psychiatric symptoms were the aims of this case report. Methods We present difficulties in growing a 9 years old girl, whose father, after being stricken at the front and had been treated for PTSD, attempted suicide. Results Faced with the traumatic situation, the patient used a self-protective mechanism suppressing negative emotions and memories to combat the negative effects of traumatic experience. Ways of coping were associated with the low degree of reliance with mother, brother and siblings. During adolescence she manifested symptoms intensified anxiety and depresion, guilt to encourage a sense of protection. Conclusions Secure attachment is the first line of defense against psychopathology launched traumatic experience. In terms of traumatic events in the child unless objective factors related to the traumatic event affects a childs perception of danger / threat, and perceptions of care required and provided by others. In that space shape the emotional experience of the child, which includes a sense of helplessness, irretrievable loss, anger and betrayal.

P-294 - Oculogyric crisis - unexpected side effect when using low doses of risperidone in adolescent patient

Introduction The use of antipsychotics in treatment of children and adolescents requires good knowledge of psychopathology, psychofarmacotherapy, developmental processes and family relations. The combination of clinical experience of those working with psychotic adolescents and a good collaboration with parents, creates a therapeutic space where good results in treatment can be achieved. Objectives The number of antipsychotics registered for use in children and adolescents is quite limited. It is necessary to have parental consent for the use of a medication in this age, with previous explanation of therapeutic goals, limitations and possible side effects of antipsychotics. Methods We present a 14-year old girl adolescent with psychotic symptoms, in which case the course of treatment and discontinuance of therapy was caused by a side effect - an oculogyric crisis. Results In the treatment we used a low daily dose of risperidone, a generic medication (1xa0mg) per os, in the evening. We used generic, not an original medication, respecting recommendations of health sector economy. The effect of the terapy was measured using Clinical Global Impression Scale (CGI) and Brief Psychiatric Rating Scale (BPRS). The daily dose of risperidone wasn‘t elevated. The girl was discharged six days after risperidone was first prescribed. Side effects were not expected. On the way home, the patient experiences oculogyric crisis. Conclusions Our clinical presentation indicates that, in spite of low doses of risperidone in generic form, side effects are not excluded, and that wasn‘t noticed when original antipsychotics were used in the same doses.
