Laser hair removal affects sebaceous glands and sebum excretion: a pilot study.

BACKGROUND During laser-assisted hair removal, sebaceous glands closely associated with hair follicles might also be affected. OBJECTIVE We investigated the effect of the long-pulsed ruby laser on sebaceous glands. METHODS Sebum excretion rates (SERs) of 16 subjects were measured quantitatively by means of sebum-absorbent tape and analyzed by means of image analysis techniques on laser-treated sites, compared with adjacent untreated areas. Evaluation was done at an average of 9 months (range, 4.5 to 12 months) after the last treatment. Histologic examinations were performed on 3 representative subjects before treatment, immediately after treatment, and 9 months after the last treatment. RESULTS Significant increases in SERs were observed in 11 of 16 subjects (68.75%). Three subjects (18.75%) showed lower SERs, whereas 2 subjects (12.5%) demonstrated no difference in SERs between treated and untreated areas. Biopsy specimens showed an apparent reduction in sebaceous gland size. Specimens taken immediately after laser irradiation revealed sporadic damage to sebaceous glands. CONCLUSION In some patients a variable but statistically significant increase in sebum excretion occurs 4 to 12 months after ruby-laser hair removal treatment at high fluences. A reduction in sebaceous gland sizes on laser-treated areas was observed. We hypothesize that decreased resistance to sebum outflow may explain this result, following miniaturization or absence of hair shaft after ruby laser treatment. Further study is needed to assess mechanisms for this interesting response.

Both microneedling and ablative fractional laser can be used for the treatment of scars. The present article reports the case of a female patient who had the first half of a perioral scar treated with ablative fractional CO2 laser, and the second half treated with microneedling, using a 2mm roller. Microneedling: a case series associated with drug delivery

Six patients underwent two sessions of microneedling associated to drug delivery on the face, hands or stretch marks, at three-week interval between sessions. Patients were evaluated using clinical analysis and objective measures. Compared to baseline, objective face analysis showed decrease in acne lesions and improvement in the skin texture. The evaluation of stretch marks showed great improvement, and face and hands presented excellent results compared to baseline. The association of microneedling to drug delivery for rejuvenation of the face and hands, and for the treatment of stretch marks presented promising results in our study.

Laser assisted drug delivery: a review

The topical route for the delivery of drugs is essential in dermatology: it optimizes the penetration of medicaments, and when performed with the assistance of lasers, it takes place in a uniform and controlled manner. The degree of evidence of drug delivery is being investigated for many drugs. Moreover, the technique combines outstandingly with lasers for rejuvenation and treatment of skin diseases. It is therefore possible to conclude that the use of lasers for drug delivery is promising. While there are clinical studies on some substances that allow their indication and use, others require further controlled analyses, with longer follow-up periods, aimed at allowing thorough evaluations.

Comparative study: Hands rejuvenation treatment using intense pulsed light isolatedly or associated with 1, 340nm non-ablative fractional laser

Introduction: The dorsum of the hands is a visible part of the body and needs special attention regarding rejuvenation treatments. The combination of different techniques in this area is important to increase the effectiveness of the treatments. Objective: This study was aimed to evaluate the clinical efficacy of a fractional non-ablative laser (1,340nm Nd:YAP) combined with intense pulsed light (IPL) for rejuvenating the hands. Methods: A prospective comparative study evaluated 11 patients complaining of aging on hands, for 90 days. Two sessions were carried out with a two-week interval using IPL and 1,340nm Nd:YAP laser. The left hand was treated with 1,340nm Nd:YAP laser associated with IPL and the right hand with isolated IPL. The improvement was rated using scores from 1 to 4, evaluating the parameters :wrinkles,pigmentation,brightness,keratosis and overallrejuvenation observing theelasticity and filling of visible structures such as bones, tendons and vessels. Results: By analyzing the frequencies of the categories of the variables, it was possible to observe that there was a higher frequency of improvement in the left hand. For the comparative analysis of the studied variables, the mean value of each variable computed for each hand, evidenciated that the variable overall rejuvenation and the left hand showed more marked improvements for the studied characteristics. Ten among 11 patients had a higher satisfaction degree with the combined treatment, with absence of significant adverse effects. Conclusion: The association of 1,340nm laser to the IPL was shown to be safe and more effective than isolated intense pulsed light in the rejuvenation of the hands Introducao: O dorso das maos representa parte visivel do corpo, e seu rejuvenescimento demanda atencao especial. A combinacao de diferentes tecnicas nessa area e importante para aumentar a eficacia dos tratamentos. Objetivo: Este estudo avaliou a eficacia clinica do laser fracionado nao ablativo (Nd:YAP 1340nm) combinado com luz intensa pulsada (LIP) para rejuvenescimento das maos. Metodos: Este estudo prospectivo e comparativo avaliou durante 90 dias 11 pacientes com envelhecimento nas maos. Foram realizadas duas sessoes com intervalo de duas semanas utilizando. IPL e Nd:YAP 1340nm laser na mao esquerda e apenas LIP na mao direita.Foram criados escores de 1 a 4 para pontuar a melhora clinica, segundo os seguintes parâmetros: rugas, pigmentacao, brilho, ceratoses e rejuvenescimento global em relacao a elasticidade e preenchimento de estruturas visiveis como ossos, tendoes e vasos. Resultados: Ao analisar as frequencias das categorias das variaveis estudadas observamos que houve melhora mais acentuada na mao esquerda. Na analise comparativa, o calculo da media de cada aspecto para cada mao mostrou que a variavel com melhor resposta foi rejuvenescimento global e que a mao esquerda apresentou melhora de maior representatividade nas caracteristicas estudadas. Dez dos 11 pacientes revelaram-se mais satisfeitos com o tratamento combinado, sem a presenca de efeitos adversos significativos. Conclusoes: A associacao do laser fracionado nao ablativo 1340nm a LIP demonstrou ser segura e mais efetiva do que a LIP isolada no rejuvenescimento de maos
