Objective: to evaluate the cognitive state of elderly people under ambulatory care and investigating the connection between such cognitive state and sociodemographic variables, health conditions, number of and adhesion to medicine. Methods: transversal, exploratory, and descriptive study, with a quantitative approach, realized with 107 elderly people under ambulatory care in a university hospital in southern Brazil, in november, 2013. The following variables were used: gender, age, civil status, income, schooling, occupation, preexisting noncommunicable diseases, number and type of prescribed medications, adhesion, mini-mental state examination score, and cognitive status. Data was analyzed through inferential and descriptive statistics. Results: the prevalence of cognitive deficit was of 42.1% and had a statistically significant connection to schooling, income, civil status, hypertension, and cardiopathy. Conclusion: nurses can intervene to avoid the increase of cognitive deficit through an assessment of the elderly person, directed to facilitative strategies to soften this deficit.

Objective: to determine the prevalence of adherence to medication therapy in elderly outpatients; to verify whether there is an association between medication therapy adherence and demographic and socioeconomic factors, health conditions. Method: a quantitative, exploratory, descriptive, cross-sectional study, performed in outpatient clinics of a university hospital in Rio Grande do Sul, Brazil. Three data collection instruments were answered by a convenience sample of 107 elders. A descriptive and inferential statistical analysis was conducted. Results: the prevalence of adherence was 86.9%. A statistically signifi cant association was identifi ed between the adherence variable and receiving medical guidelines on how to take medications, and having adverse reactions. Conclusion: Nurses must know the prevalence of medication adherence and related factors in order to plan interventions that promote therapeutic success.

Objective: To reflect on the harm reduction among older people who use drugs, from the perspective of Morins complexity. Methods: A philosophical theoretical study that used the seven guiding principles of a binding thought that deals with uncertainty and are constantly building process: systemic or organizational, holographic, retroactivity, recursion, self-reliance, dialogical and reintroduction of the knowing subject in all knowledge. Results: There is a relationship between some guiding principles of harm reduction and behaviour of elderly people who use drugs, reflecting from questions and provocations made by antagonistic and complementary characteristics. Conclusions: The framework of complexity enabled dialogue on the plurality of ideas that permeate the lives of elderly people who use drugs and who show antagonistic and discordant practices, which the Harm Reduction Program seeks to understand and respect, without proposing behaviours judged suitable and abstinence. Objective: To reflect on the harm reduction among older people who use drugs, from the perspective of Morins complexity.
