Lymphocyte transformation in mycosis fungoides

SummaryLymphocyte transformation tests with PHA were carried out in 26 patients with mycosis fungoides (MF) in different stages of the disease.In premycotic and infiltrative stages of MF there were no significant disturbances in the lymphocyte response to PHA.Inhibition of DNA synthesis and blastic transformation was observed only in the tumorous stage. The results are in favour of the nosological distinctiveness of mycosis fungoides within the lymphoma group.ZusammenfassungBei 26 Mycosis fungoides-Kranken wurde in den verschiedenen Krankheitsstadien ein Lymphocyten-Transformationstest mit PHA durchgeführt.Im prämykotischen und infiltrativen Stadium der MF gab es keine bedeutenden Störungen in der Lymphocyten-Reaktion auf PHA.Eine Inhibition der DNS-Synthese und blastische Transformation war nur in der Tumorperiode zu beobachten. Die Resultate sprechen für die nosologische Verschiedenheit der Mycosis fungoides in der Lymphomgruppe.

Heart rate turbulence assessment in systemic sclerosis: the role for the detection of cardiac autonomic nervous system dysfunction

OBJECTIVE To assess the heart rate turbulence (HRT) in patients with SSc as a method of assessment for cardiac autonomic nervous function. METHODS We prospectively studied 68 consecutive patients with SSc before inclusion in the study. After a detailed clinical evaluation, including echocardiography, 45 subjects [aged 54.6 (14.7) years; 40 women] underwent 24-h Holter monitoring for HRT and time- and frequency-domain heart rate variability (HRV) assessment. Results were compared with those in 30 age- and sex-matched healthy controls. RESULTS As compared with controls, HRT was impaired in SSc patients: the median turbulence onset (TO) was higher (P = 0.0001) and the median turbulence slope (TS) was lower (P = 0.0003). Abnormal HRT (TO > or =0.0% and/or TS < or =2.5 ms/RR) was found in 42% of SSc patients. Moreover, SSc duration correlated negatively with values of TS (r = -0.3; P = 0.045). HRT did not differ between diffuse and limited SSc groups. All estimated time- and the majority of frequency-domain values of HRV parameters in SSc were significantly lower than in controls. Significant correlations were also demonstrated between HRT and HRV parameters. CONCLUSIONS HRT, like HRV assessment, indicates a frequent impairment of the cardiac autonomic nervous system in SSc patients, irrespective of the SSc type.

Identification of HLA Antigens in Familial and Non-Familial Epidermodysplasia verruciformis

The distribution of HLA specificities was studied in 7 non-familial cases of epidermodysplasia verruciformis (EV) and in 5 cases from one family. In the non-familial cases, six antigens of locus A (2, 3, 9, 10, 24, 26) and eight antigens of locus B (7, 15, 27, 35, 37, 38, 40, 41) were found. All 5 cases of familial EV possessed 26,5 haplotype, however, 6 of the remaining 12 healthy family members inherited the same haplotype. There is no association of EV with HLA-A and B antigens. However, the defect of cell-mediated immunity, present in all EV cases, may indicate the association of the disease with other major histocompatibility products (i.e. loci on immune response genes).

Pulmonary hypertension in systemic sclerosis determines cardiac autonomic dysfunction assessed by heart rate turbulence

There is limited data on heart rate turbulence (HRT) in systemic sclerosis (SSc) patients, potentially threatened with cardiac autonomic dysfunction. We performed 24-hour Holter monitoring for HRT assessment in 45 patients with SSc and 30 healthy controls. Abnormal HRT defined as turbulence onset (TO) >or=0.0% and/or turbulence slope (TS)<or=2.5 ms/RR was found in 19 (42%) of SSc patients while not in controls. HRT was significantly impaired in SSc group, as compared to controls: a median of TO was higher (-0.13 vs. -2.45%, p=0.0001), and a median of TS was lower (6.90 vs. 13.21 ms/RR, p=0.0003), respectively. Multivariate analysis showed that abnormal HRT in SSc was significantly influenced by pulmonary hypertension (PH) (OR 6.1, 1.1-48.6; p=0.049) and older age (OR 1.8, 95%CI 1.1-3.5; p=0.03). In conclusion, HRT assessment indicates a frequent impairment of cardiac autonomic nervous system in SSc patients. Moreover, older age in these patients and especially PH are independent significant predictors of cardiac autonomic dysfunction.
