Fibrosarcoma in bizarre parosteal osteochondromatous proliferation.

Abstract Bizarre parosteal osteochondromatous proliferation (BPOP) is a rare benign lesion predominantly involving the small bones of the hands and feet. Malignant transformation in BPOP has not been documented in the English literature. This report presents the coexistence of fibrosarcoma with BPOP in the right distal fibula of an 18-year-old woman.

Primary clear cell sarcoma of bone

Clear cell sarcoma is a rare soft tissue sarcoma of young adults with melanocytic differentiation. It occurs predominantly in the soft tissue of extremities, typically involving tendons and aponeuroses. Primary clear cell sarcoma of bone is extremely rare. We report a case of primary clear cell sarcoma of the right first metatarsal in a 48-year-old woman and provide a literature review of the entity.

A case of primary myxoid leiomyosarcoma of the liver.

Primary hepatic leiomyosarcoma is very rare and no primary myxoid leiomyosarcoma in the liver has been reported yet. Most cystic space-occupying lesions in the liver are benign in nature. But, rarely, malignancy could appear as a cystic lesion by ultrasonographic examination. A 64-year-old woman with a huge cystic mass detected by hepatic ultrasonography was diagnosed as primary hepatic myxoid leiomyosarcoma by immunohistochemical and ultrastructural studies after various image studies and fine needle aspiration biopsy of the liver mass.

Difference of recovery course of motor weakness according to state of corticospinal tract in putaminal hemorrhage

OBJECTIVES We investigated differences in recovery course of motor weakness according to the state of the corticospinal tract (CST) in putaminal hemorrhage, using diffusion tensor tractography (DTT). METHODS We recruited 36 patients with complete weakness of the affected extremities at onset. The patients were classified into two groups according to the findings of DTT for the CST at chronic stage: group A- preserved integrity of the CST around the lesion, and group B- discontinued integrity of the CST. Motor function of the affected extremities was measured over a six month period using the Motricity Index (MI). RESULTS The MI scores differed significantly each month, except at the onset, between group A and group B (p<0.05). In both groups, we observed significant increases between onset and one month, between one month and two months, between two month and three months, and between three months and four months (p<0.05). However, there were no significant increases after four months (p>0.05). The degree of difference between months was as follows: onset ∼1 month, 1 month ∼2months, 2 months ∼3months, and 3 months ∼4months. CONCLUSIONS Patients with preserved integrity of the CST showed better motor function than patients with discontinued integrity of the CST. In both groups, significant motor recovery was achieved during the first four months after onset. In addition, the most rapid motor recovery occurred during the first month and then decreased gradually with the passage of time.

Prediction of motor outcome using remaining corticospinal tract in patients with pontine infarct: Diffusion tensor imaging study

Abstract The aim of this study was to investigate the relationship between the remaining corticospinal tract (CST) as determined by diffusion tensor imaging (DTI) and 6-month motor outcome in patients with pontine infarct. Ratios of fractional anisotropy (FA), fiber number (FN), and CST area were calculated, and the FN ratio and CST area ratio showed significant correlation with all 6-month motor outcome. Thus, the remaining CST in the pons measured using DTI at early stage of stroke could predict motor outcome in patients with pontine infarct.

Expression of Oncogene Product in the Colorectal Carcinoma

The expression of and can be seen in many solid tumor, but the pattern and incidence of expression were different according to organ, countries, and examiners, thus it is not definitely defined. Total 67 colorectal carcinoma in paraffin sections are analysed by immunohistochemically for evaluation of the and expression according to the age, sex, chief complaints, location, differentiation, modified Dukes stage, using the specific monoclonal antibodies. The results were summarized as follows : The age of patients ranged from 32 years to 82 years. The mean age was 57.6 years. The expression of and was not correlated with age. Male was 29 cases(43.3%) and female was 38 cases(56.7%). The male to female ratio was 1:1.31. The expression of was increased in female(p was increased in the rectum(p in the nucleus, cytoplasmic membrane, and cytoplasm. The 62 cases(92.5%) out of 67 cases showed positive reaction of in the cytoplasmic membrane and cytoplasm. The positive rate of and was 97.0% and 91.4% in well differentiated adenocarcinoma, 100.0% and 95.0% in moderately differentiated adenocarcinoma, 90.0% and 90.0% in poorly differentiated adenocarcinoma, 100.0% and 100.0% in mucinous carcinoma. The positive rate of and was 94.1% and 88.2% in Dukes stage , 96.0% and 96.0% in Dukes stage , 100.0% and 100.0% in Dukes stage , 100.0% and 88.9% in Dukes stage , and 100.0% and 100.0% in Dukes stage D. The expression of in metastatic colorectal carcinoma showed diffuse and strongly positive reaction than primary colorectal carcinoma. The expression of in primary colorectal carcinoma showed diffuse and strongly positive reaction than metastatic colorectal carcinoma.

Clinical and Histopathological Studies on Carcinoma of the Uterine Cervix in Taegu

Clinical and histopathological studies were made on 202 cases of malignancy of cervix, that were visited to the Department of Obstertrics and Gynecology, Yeungnam University Hospital, during 5 years from 1983 to 1987. The results were summarised as follows. 1. Malignancies of the uterine cervix were 10% of total female malignancies. 2. Among 202 cases, 195 cases(96.5%) were squamous cell carcinoma, in which 60 cases(30.0%) of carcinoma in situ, 9 cases(4%) of microinvasive, and 126 cases(62.5%) of invasive carcinoma were included. 3. The average age of the patients with squamous cell carcinoma was 49.4 years old. ; In cases of carcinoma in situ, it was 43.8, microinvasive, 40.0, invasive 52.1 years old. 4. Clinical symptoms of the patient with squamous cell carcinoma in order of frequency were as follows. ; vaginal bleeding(47.5%), abnormal cytology(15.4%), and abnormal vaginal discharge(9.4%). 5. Duration of the chief complaints was most commanly less than 6 months(73.2%), and the average duration was 3.8 months. 6. The most common age of marriage was between 19 to 22 years 0Id(46.5%). The average was 21.5 years old. 7. The gravity was 51.5% in 5~8 times, and average 6.2 times. The parity was 61.9% in 1~4 times, and average 3.9 times. 8. The subdivision of 126 cases of invasive carcinoma was made according to FIGO stage classification, stage I, 40 cases(31.8%), stage II, 54 cases(42.9%), stage III, 11 cases(8.7%), and stage IV, 8 cases(6.3%). 9. The histologic subtypes of invasive squamous cell carcinoma were distributed as follows. ; large cell keratinizing type, 25 cases(19.8%), large cell nonkeratinizing type, 101 cases(80.2%). 10. In the cytologic diagnosis, class I was 2 cases(l.9%), class II was 16 cases(15.1%), class III was 33 cases(31.1%), class IV was 31 cases(29.3%), class V was 24 cases(24.6%). 11. The frequency of lymph node metastasis was 7.5% in stage I, and 11.1% in stage II.

Three Cases of Primary Adenosquamous Carcinoma of Stomach

Adenosquamous carcinoma of stomach is a mixed glandular-epidermoid tumor where both components are neoplastic. Its incidence is extremely rare. The five theories on the origin of squamous components are 1) island of ectopic squamous epithelium in the gastric mucosa, 2) squamous metaplasia of gastric epithelium, 3) squamous differentiation in a preexisting adenocarcinoma, 4) endothelial cell differentiated toward squamous elements, and 5) totipotential undifferentiated cells of the gastric mucosa. We experienced three cases of adenosquamous carcinoma. Case 1 was a 71-year-old female patient. ; an ulcerative lesion was present in the pylorus, measuring 5cm in diameter. Case 2 was a 57-year-old male patient. ; an ulcerative lesion is present in the pylorus, measuring 6 em in diameter. Case 3 was a 58-year-old female patient. ; an ulcerative lesion was present in the body and fundus, measuring 10cm in diameter. Microscopic examination revealed a mixed malignant squamous and adenomatous component.

A Case of Pseudomembranous Colitis

Many reports have been made concerning underlying and associated conditions causing pseudomembranous colitis and it has been documented that occurrence of pseudomembranous colitis is related with antibiotics administration. Recent study showed that Clostridium difficile produced enterotoxin by colonization in intestinal wall and leading into pseudomembranous colitis. Diagnosis is based on positive culture of Clostridium difficile, positive test of Clostridium difficile toxin and specific histological findings after observation of whitish plaque on colonoscopic or sigmoidoscopic examination. Authors have experienced one case of pseudomembranous colitis developing after long term ampicillin administration in a case with colon cancer associated with diarrhea and diagnosis was confirmed by typical pseudomembrane on biopsy following classical whitish plaque observation on sigmoidoscopic examination. Symptoms have been ameliorated by discontinuation of antibiotics and administration of metronidazole in four days and disappearance of whitish plaque on repeated sigmoidoscopic examination and improvement of clinical symptoms after 9 days of medication.
