Histopathological Effect of Preoperative Chemotherapy Using Oral 5-Fluorouracil in Patients with Operable Gastric Cancer

Recent studies of modulated 5-fluorouracil (5-FU) have reported a high response rate in patients with metastatic and advanced gastric and colorectal cancer [1]. 5-FU tablets (Kyowa Hakko, Japan) have been commercially available in Japan since the 1970s and are widely used for the treatment of gastric, colorectal, and breast cancer. The drug is mainly employed in an adjuvant setting with long-term oral administration in mild toxic dose schedules [2]. However, there has been no marked benefit of postoperative adjuvant chemotherapy for 5-year survival, except in the stage II or III subgroups of gastric carcinoma [3,4].

Prognostic study of gastric cancer patients with liver metastases treated by arterial infusion chemotherapy.

肝転移 (同時性) を伴う胃癌111例を対象として臨床病理学的事項1) ならびに動注療法について検討した.H因子別ではH1が30.6%, H2が27.0%, H342.4%であった. 胃癌の肉眼型では2型38.0%, 3型46.6%であり, 組織型では分化型64. 8%, 低分化型29.6%であった. 胃切除群の予後は胃非切除群より有意に良好であった (p<0.001). H1, H2, H3の予後はH3が最も悪かったが, 3群間に有意差を認めなかった. 胃切除・動注 (+) の予後は胃切除・動注 (-) より軽度良好であった. 胃非切除・動注 (+) の予後は胃非切除・動注 (-) より有意に良好であった (p<0.01). したがって, 胃原発巣とH1, H2の可及的切除, それと投与方法を工夫した肝動注および全身療法が必要と考えられた.

Preoperative endoscopic activated carbon particle CH44 stainig as a indicator of the lower choracic lymphadenectomy for cardiac cancer.

下部胸腔内リンパ節の局在, 個数, 転移状況を知り, 郭清精度を高める目的で, 食道進展を伴う噴門癌20例に対し, 微粒子活性炭CH44を腫瘍口側端に近い食道粘膜下に内視鏡的に注入したのち切除手術を行い, 郭清したリンパ節を検索した.黒染リンパ節出現率はそれぞれ110番: 85%, 111番: 90%, 112番: 40%であった.組織学的に確定したリンパ節個数はそれぞれ110番: 2.0個, 111番: 3.5個, 112番: 0.9個であった. リンパ節転移陽性症例は2例 (10%) であった.微粒子活性炭CH44によるリンパ節染色法はリンパ節郭清精度を高めるのに有用である.

Clinicopathological study on the recurrent type and effects of multidisciplinary therapy for gastric cancer.

再発胃癌121例の再発形式は, 腹膜52.1%, 肝19.8%, リンパ節9.9%, 局所6.6%, その他11.6%であった. 腹膜再発ではporとsigが多く, 肝では分化型が多かった (p<0.01). 腹膜型は深達度でより深く, リンパ節転移も高度であった (p<0.05). 肉眼型で, 1, 2型に肝転移が多く, 3, 4型に腹膜型が多かった (p<0.001). 再発の特長として, 局所再発が少なかった. 次にps (+) 症例での, 再発群と非再発群の比較では, lyとn因子が再発に関与していた (p<0.05). 再発に対する化学療法あるいは放射線療法の有効群は無効群よりそれぞれ有意に予後が良好であった (p<0.05).
