Development of Diabetes in Chinese With the Metabolic Syndrome A 6-year prospective study

OBJECTIVE—We investigated the association of the metabolic syndrome with new-onset diabetes in the Hong Kong Cardiovascular Risk Factor Prevalence Study cohort. RESEARCH DESIGN AND METHODS—We followed up on 1,679 subjects without diabetes at baseline. Those with a previous diagnosis of diabetes or those who were receiving drug treatment were considered to be diabetic. The remaining subjects underwent a 75-g oral glucose tolerance test (OGTT). Diabetes was defined by plasma glucose ≥7.0 mmol/l with fasting and/or ≥11.1 mmol/l at 2 h. RESULTS—The prevalences of the metabolic syndrome at baseline were 14.5 and 11.4%, respectively, according to U.S. National Cholesterol Education Program (NCEP) and International Diabetes Federation (IDF) criteria. After a median of 6.4 years, there were 66 and 54 new cases of diabetes in men and women, respectively. The metabolic syndrome at baseline predicted incident diabetes. Hazard ratios (HRs) for the NCEP and IDF definitions of the syndrome were 4.1 [95% CI 2.8–6.0] and 3.5 [2.3–5.2], respectively. HRs for fasting plasma glucose (FPG) ≥6.1 or 5.6 mmol/l were 6.9 [4.1–11.5] and 4.1 [2.8–6.0], respectively. The NCEP and IDF criteria had 41.9 and 31.7% sensitivity and 87.5 and 90.2% specificity, respectively. Their positive predictive values were low, ∼20%, but their negative predictive values were ∼95%. CONCLUSIONS—The metabolic syndrome, particularly its component, elevated FPG, predicts diabetes in Chinese. An individual without the metabolic syndrome is unlikely to develop diabetes, but one who has it should practice therapeutic lifestyle changes and have periodic FPG measurements to detect new-onset diabetes.

Cancer risk assessments of Hong Kong soils contaminated by polycyclic aromatic hydrocarbons

The aim of this study was to evaluate soils from 12 different land use types on human cancer risks, with the main focus being on human cancer risks related to polycyclic aromatic hydrocarbons (PAHs). Fifty-five locations were selected to represent 12 different types of land use (electronic waste dismantling workshop (EW (DW)); open burning site (OBS); car dismantling workshop (CDW) etc.). The total concentrations of 16 PAHs in terms of total burden and their bioaccessibility were analysed using GC/MS. The PAHs concentrations were subsequently used to establish cancer risks in humans via three exposure pathways, namely, accident ingestion of soil, dermal contact soil and inhalation of soil particles. When the 95th centile values of total PAH concentrations were used to derive ingestion and dermal cancer risk probabilities on humans, the CDW land use type indicated a moderate potential for cancerous development (244 × 10(-6) and 209 × 10(-6), respectively). Bioaccessible PAHs content in soil samples from CDW (3.60 × 10(-6)) were also classified as low cancer risk. CDW soil possessed a higher carcinogenic risk based on PAH concentrations. Bioremediation is recommended to treat the contaminated soil.

Risk assessment of human exposure to bioaccessible phthalate esters via indoor dust around the Pearl River Delta.

There is limited information on the bioaccessible fractions of phthalate esters in indoor dust in order to estimate human exposure. In the present study, workplace dust and settled house dust samples from Hong Kong, Shenzhen, and Guangzhou, the three major cities scattered around the Pearl River Delta (PRD) were collected. Chemical analyses showed that the phthalates in workplace dust ranged from 144 to 1810 μg/g, with dust from shopping malls containing the highest level, and in home dust ranged from 181 to 9240 μg/g. The most abundant phthalate ester found was bis(2-ethylhexyl) phthalate (DEHP) in both workplace dust and home dust, followed by di-n-butyl phthalate (DBP) and di-iso-butyl phthalate (DIBP). Principal Components Analysis (PCA) indicated that indoor dust around PRD showed similar phthalate esters patterns of composition. A significant correlation was observed between total phthalate esters concentrations in home dust and the number of year of house construction (p < 0.05). The oral bioaccessibility of phthalate esters in indoor dust ranged from 10.2% (DEHP) to 32% (DMP). Risk assessment indicated that the dominant exposure routes varied in different phthalate esters exposure profiles and the dermal contact exposure pathway was identified as an important route for indoor DEHP exposure.

Trophic relationships and health risk assessments of trace metals in the aquaculture pond ecosystem of Pearl River Delta, China.

Cadmium, lead, zinc, Chromium, copper, nickel and manganese in sediments and in aquatic organisms were collected from the aquaculture pond ecosystem of the Pearl River Delta (PRD), China and analyzed to evaluate bioaccumulation and trophic transfer in food chains, as well as the potential health risk of exposure to the Hong Kong residents via dietary intake of these aquatic products. The results revealed that based on the biota-sediment accumulation factor, omnivorous fish and zooplankton accumulated more trace metals from sediment than carnivorous fish. Concentrations of seven trace metals in aquaculture pond of PRD significantly decreased with increasing trophic levels, showing that these trace metals were trophically diluted in predatory and omnivorous food chains. The hazard index values of all fish species were smaller than 1 for adults and children, indicating there was no health risk from the multiple metals via ingestion of the freshwater fish for the inhabitants.

Cancer risk assessment of polybrominated diphenyl ethers (PBDEs) and polychlorinated biphenyls (PCBs) in former agricultural soils of Hong Kong.

The major objective of this study was to evaluate the carcinogenic risk posed to humans through PBDEs and PCBs of changing agricultural land use for recycling of e-waste and open burning of municipal waste. Nine locations were selected to represent 6 different types of land use such as e-waste dismantling workshop (EW (DW)) and e-waste open burning site (EW (OBS)). The total concentrations for PBDEs and PCBs, and the bioaccessibility of PCBs were determined using Soxhlet extraction and in vitro simulated gastric solution, respectively. Both total and bioaccessible concentrations were subsequently used to establish the cancer risk probabilities in humans via ingestion, dermal contact and inhalation of soil particles. It was found that very low cancer risk in all 6 types of different land use was caused by BDE-209. Nevertheless, at the 95th centile, the concentration of PCBs in EW (DW) and EW (OBS) indicate a low cancer risk to humans of 40 and 2.1 in a million, respectively, while the same was also observed for the bioaccessible PCBs in EW (DW) of 1.71 ± 2.96 in a million.

Application of food waste based diets in polyculture of low trophic level fish: effects on fish growth, water quality and plankton density.

Food waste was collected from local hotels and fish feed pellets were produced for a 6 months long field feeding trial. Three types of fish feed pellets (control diet: Jinfeng® 613 formulated feed, contains mainly fish meal, plant product and fish oil; Diet A: food waste based diet without meat and 53% cereal; Diet B: food waste based diet with 25% meat and 28% cereal) were used in polyculture fish ponds to investigate the growth of fish (grass carp, bighead and mud carp), changes in water quality and plankton density. No significant differences in the levels of nitrogen and phosphorous compounds of water body were observed between 3 fish ponds after the half-year feeding trial, while pond receiving Diet A had the highest density of plankton. The food waste combination of Diet B seems to be a better formulation in terms of the overall performance on fish growth.

Profiles and removal efficiency of polybrominated diphenyl ethers by two different types of sewage treatment work in Hong Kong

This study was to investigate removal efficiencies and profiles of 14 polybrominated diphenyl ether (PBDE) congeners by two different types of sewage treatment work (STW) in Hong Kong: Stonecutters Island STW (SCISTW) which uses chemically enhanced primary treatment (CEPT) process and Sha Tin STW (STSTW) which adopts biological treatment. The results indicated that both SCISTW and STSTW had a high total removal efficiency for BDE-47, BDE-99 BDE-209 and total PBDEs (SCISTW: 71.6 ± 15.8, 84.7 ± 12.3, 96.0 ± 2.62 and 87.4 ± 8.02%, respectively; STSTW: 74.8 ± 9.5, 90.7 ± 9.14, 96.2 ± 2.41 and 89.3 ± 2.62%, respectively) and PBDEs were chiefly removed by sorption. However, the profile of PBDEs demonstrated that the relative proportions of BDE-28 and BDE-47 in total PBDEs markedly increased, while that of BDE-209 decreased in the effluent samples of the two sewage treatment works, especially in STSTW. The percentage of BDE-209 in total PBDEs in effluent (49.3%) of SCISTW was 21.2% lower than that in influent (70.5%), and the percentage of BDE-209 in total PBDEs in effluent (13.8%) of STSTW was 34.1% reduced from influent (47.9%). Despite overall removal, the percentage of BDE-47 in total PBDEs in effluent (17.6%) of SCISTW was 6.85% higher than that in influent (10.7%), and the percentage of BDE-47 in total PBDEs in effluent (33.5%) of STSTW was 18.1% increased from influent (16.8%). The increase in proportion of BDE-47 in the effluent might raise environmental and public health concerns. Our study is a first attempt in reporting the PBDE congener profiles in different phases of sewage treatment.

Environmental mercury concentrations in cultured low-trophic-level fish using food waste-based diets

In this study, different types of food wastes were used as the major source of protein to replace the fish meal in fish feeds to produce quality fish (polyculture of different freshwater fish). During October 2011–April 2012, the concentrations of Hg in water, suspended particulate matter, and sediment of the three experimental fish ponds located in Sha Tau Kok Organic Farm were monitored, and the results were similar to or lower than those detected in commercial fish ponds around the Pearl River Delta (PRD) region (by comparing data of previous and present studies). Health risk assessments indicated that human consumption of grass carp (Ctenopharyngodon idellus), a herbivore which fed food waste feed pellets would be safer than other fish species: mud carp (Cirrhina molitorella), bighead carp (Hypophthalmichthys nobilis), and largemouth bass (Lepomis macrochirus). Due to the lower species diversity and substantially shorter food chains of the polyculture system consisting of only three fish species, the extent of Hg biomagnification was significantly lower than other polyculture ponds around PRD. Furthermore, the use of food waste instead of fish meal (mainly consisted of contaminated trash fish) further reduced the mercury accumulation in the cultured fish.

Characterizing the optimal operation of photocatalytic degradation of BDE-209 by nano-sized TiO2

Brominated flame retardants have been widely used in industry. There is a rapid growing public concern for their availabilities in the environment. Advanced oxidation process (AOP) is a promising and efficient technology which may be used to remove emerging chemicals such as brominated flame retardants. This study aims at investigating optimal operational conditions for the removal of BDE-209 using nano-scaled titanium(IV) oxide. The residual PBDE congeners after photocatalytical degradation of BDE-209 by TiO2 were analysed by gas chromatography-mass spectrometry (GC-MS). It was found that the degradability of BDE-209 by TiO2 was attributed to its photocatalytic activity but not the small size of the particles. The half-life of removing BDE-209 by TiO2 was 3.05 days under visible light. Tetra- and penta-BDEs were the major degraded products of BDE-209. Optimum conditions for photocatalytical degradation of BDE-209 was found to be at pH 12 (93% +/- 1%), 5, 10, 20 mg/L (93.0% +/- 1.70%, 91.6% +/- 3.21%, 91.9% +/- 0.952%, respectively), respectively of humic acid and in the form of anatase/rutile TiO2 (82% +/- 3%). Hence, the efficiency of removing BDE-209 can be maximized while being cost effective at the said operating conditions.

Association of essential hypertension with a microsatellite marker on chromosome 17

Hypertension is related to sodium intake, and many patients with essential hypertension are overweight and have the metabolic syndrome. We therefore studied microsatellite markers close to the thiazide-sensitive Na–Cl cotransporter on chromosome 16 and a quantitative trait locus for abdominal obesity-metabolic syndrome (AOMS2) on chromosome 17, which have been found to be linked to hypertension in a previous genome scan in Chinese. There were 84 hypertensive subjects (44 men, 40 women, age 53±13 years) and 88 normotensive controls (40 men, 48 women, age 54±13 years) recruited. Specific oligonucleotide primers were used to amplify genomic DNA spanning the microsatellite markers D16S3396 and D17S1303 that consist of ATA and GATA repeats, respectively. We did not find any association between D16S3396 and blood pressure. In contrast, the distribution of D17S1303 genotypes differed between hypertensive subjects and normal controls (P=0.014). The number of GATA repeats correlated inversely with diastolic blood pressure (r=−0.18, P=0.02) and body mass index (r=−0.12, P=0.01). Nine GATA repeats in D17S1303 were associated with hypertension (OR 2.19, 95% CI 1.08–4.44, P=0.027), while 14 GATA repeats were associated with normotension (OR 0.26, 95% CI 0.10–0.66, P=0.002). The diastolic blood pressure in those with or without the (GATA)9 allele was 85.9±13.6 and 79.2±13.6 mmHg respectively (P=0.01), and in those with or without the (GATA)14 allele it was 73.8±11.0 and 81.8±14.0 mmHg respectively (P=0.003). Our results provide further evidence that a gene predisposing to hypertension in Chinese is in the vicinity of the microsatellite D17S1303.
