BACKGROUND AND PURPOSE: Aneurysm treatment by intrasaccular packing has been associated with a relatively high rate of recurrence. The use of mesh tubes has recently gained traction as an alternative therapy. This article summarizes the midterm results of using an endoluminal sleeve, the PED, in the treatment of aneurysms. MATERIALS AND METHODS: A total of 19 wide-neck aneurysms were treated in 18 patients: 10 by implantation of PEDs alone and 9 by a combination of PED and coils. Angiographic and clinical results were recorded immediately and at 6 months following treatment. RESULTS: Immediate angiographic occlusion was achieved in 4 and flow reduction, in another 15 aneurysms. Angiography at 6 months demonstrated complete occlusion in 17 and partial filling in 1 of 18 patients. There was no difference between coil-packed and unpacked aneurysms. Of 28 side branches covered by ≥1 device, the ophthalmic artery was absent immediately in 1 and at 6 months in another 2 cases. One patient experienced abrupt in-stent thrombosis resulting in a transient neurologic deficit, and 1 patient died due to rupture of a coexisting aneurysm. All giant aneurysms treated with PED alone were demonstrated by follow-up cross-sectional imaging to have involuted by 6 months. CONCLUSIONS: Treatment of large, wide-neck, or otherwise untreatable aneurysms with functional reconstruction of the parent artery may be achieved with relative safety using dedicated flow-modifying devices with or without adjunctive use of intrasaccular coil packing.

Background and Purpose— The introduction of flow diverters (FDs) has expanded the possibilities for reconstructive treatment of difficult intracranial aneurysms. Concern remains as to the long-term patency of the perforating arteries and side branches covered during stent placement. Our purpose was to evaluate the performance of and early effect on covered branches after implantation of the Silk FD in the treatment of basilar artery aneurysms. Methods— Twelve patients with an aneurysm of the basilar artery that was treated by implantation of the Silk FD were included in our retrospective study. Both unruptured and previously ruptured, formerly untreated, and recurrent aneurysms were treated. During follow-up, patients were monitored for clinical evolution, patency of the covered vessels, and aneurysmal obliteration. Results— Of the 2 ruptured aneurysms, 1 was initially treated by FD implantation. The FD covered the basilar bifurcation and the origin of a P1 segment of the posterior cerebral artery in 9 cases, the origin of the superior cerebellar artery in 9, and of the anterior inferior cerebellar artery in 3. There was 1 acute basilar artery occlusion a few hours after FD implantation. During a mean follow-up of 16 weeks, 3 patients experienced a symptomatic neurologic event. Conclusions— Implantation of the Silk FD in the basilar artery was feasible and well tolerated in most cases to date. However, late ischemic events affecting perforating arteries may occur after FD implantation, suggesting that the indication should be restricted to otherwise untreatable aneurysms in this location.

BACKGROUND AND PURPOSE: Cerebral aneurysms are preferentially located at arterial curvatures and bifurcations that are exposed to major hemodynamic forces, increasingly implicated in the life cycle of aneurysms. By observing the natural history of aneurysm formation from its preaneurysm state, we aimed to examine the hemodynamic microenvironment related to aneurysm initiation at certain arterial segments later developing an aneurysm. MATERIALS AND METHODS: The 3 patients included in the study underwent cerebral angiography with 3D reconstruction before a true aneurysm developed. The arterial geometries obtained from the 3D-DSA models were used for flow simulation by using finite-volume modeling. The WSS and SWSSG at the site of the future aneurysm and the flow characteristics of the developed aneurysms were analyzed. RESULTS: The analyzed regions of interest demonstrated significantly increased WSS, accompanied by an increased positive SWSSG in the adjacent proximal region. The WSS reached values of >5 times the temporal average values of the parent vessel, whereas the SWSSG approximated or exceeded peaks of 40 Pa/mm in all 3 cases. All patients developed an aneurysm within 2 years, 1 of which ruptured. CONCLUSIONS: The results of this hemodynamic study, in accordance with the clinical follow-up, suggest that the combination of high WSS and high positive SWSSG focused on a small segment of the arterial wall may have a role in the initiation process of aneurysm formation.

Background and purpose The purpose of this study was to assess the initial post-market experience of the device and how it is compared with the Penumbra Pivotal trial used to support the 510k application. Methods A retrospective case review of 157 consecutive patients treated with the Penumbra system at seven international centers was performed. Primary endpoints were revascularization of the target vessel (TIMI score of 2 or 3), good functional outcome as defined by a modified Rankin scale (mRS) score of ≤2 and incidence of procedural serious adverse events. Results were compared with those of the Penumbra pivotal trial. Results A total of 157 vessels were treated. Mean baseline values at enrollment were: age 65 years, NIHSS score 16. After use of the Penumbra system, 87% of the treated vessels were revascularized to TIMI 2 (54%) or 3 (33%) as compared with 82% reported in the Pivotal trial. Nine procedural serious adverse events were reported in 157 patients (5.7%). All-cause mortality was 20% (32/157), and 41% had a mRS of ≤2 at 90-day follow-up as compared with only 25% in the Pivotal trial. Patients who were successfully revascularized by the Penumbra system had significantly better outcomes than those who were not. Conclusion Initial post-market experience of the Penumbra system revealed that the revascularization rate and safety profile of the device are comparable to those reported in the Pivotal trial. However, the proportion of patients who had good functional outcome was higher than expected.

AbstractBackground and Purpose:Growth and rupture, the two events that dominate the evolution of an intracranial aneurysm, are both dependent on intraaneurysmal flow. Decrease of intraaneurysmal flow is considered an attractive alternative for treating intracranial aneurysms by minimally invasive techniques. Such modification can be achieved by inserting stents or flow diverters alone. In the present paper, the effect of different commercial and innovative flow diverters’ porosity was studied in intracranial aneurysm models.Material and Methods:Single and stent-in-stent combination of Neuroform II as well as single and stent-in-stent combination of a new innovative, low-porosity, intracranial stent device (D1, D2, D1 + D2) were inserted in models of intracranial aneurysms under shear-driven flow and inertia-driven flow configurations. Steady and pulsating flow rates were applied using a blood-like fluid. Particle image velocimetry was used to measure velocity vector fields in the aneurysm midplane along the vessel axis. Flow and vorticity patterns, velocity and vorticity magnitudes were quantified and their value compared with the same flows in absence of the flow diverter.Results:In absence of flow diverters, a solid-like rotation could be observed in both shear-driven and inertia-driven models under steady and pulsatile flow conditions. The flow effects due to the insertion of low-porous devices such as D1 or D2 provoked a complete alteration of the flow patterns and massive reduction of velocity or vorticity magnitudes, whereas the introduction of clinically adopted high-porous devices provoked less effect in the aneurysm cavity. As expected, results showed that the lower the porosity the larger the reduction in velocity and vorticity within the aneurysm cavity. The lowest-porosity device combination (D1 and D2) reached an averaged reduction of flow parameters of 80% and 88% under steady and pulsatile flow conditions, respectively. The reduction in mean velocity and vorticity was much more significant in the shear-driven flows as compared to the inertia-driven flows.Conclusion:Although device porosity is the main parameter influencing flow reduction, other parameters such as device design and local flow conditions may influence the level of flow reduction within intracranial aneurysms.ZusammenfassungHintergrund und Ziel:Die zwei wichtigsten Faktoren für die Entwicklung intrazerebraler Aneurysmen, nämlich Wachstum und Ruptur, hängen vom intraaneurysmatischen Blutfluss ab. Eine Verminderung des intraaneurysmatischen Blutflusses durch minimalinvasive Techniken wird als attraktive Behandlungsmethode erachtet. Eine solche Modifikation des Blutflusses kann durch das Einbringen eines Stents oder „flow diverter“ allein erzielt werden. In der vorliegenden Arbeit untersuchten die Autoren den Effekt der Porosität verschiedener handelsüblicher und innovativer „flow diverters“ an Modellen intrakranieller Aneurysmen.Material und Methodik:Sowohl einzelne oder Stent-in-Stent-Kombinationen des Neuroform II (NF) als auch einzelne oder Stent-in-Stent-Kombinationen von neuen innovativen, niedrigporösen intrakraniellen Stents (D1, D2, D1 + D2) wurden in Modellen intrakranieller Aneurysmen mit Eigenschaften von „shear-driven“ und „inertia-driven“ Fluss platziert. Flächen mit Geschwindigkeitsvektoren in der mittleren Ebene des Aneurysmas parallel zur Achse des Gefäßes wurden mit Hilfe der „particle image velocimetry“ (PIV) ermittelt. Eigenschaften von Fluss und Verwirbelungen, Geschwindigkeit und Ausmaß von Verwirbelungen wurden gemessen und mit Messwerten des gleichen Modells ohne „flow diverter“ verglichen.Ergebnisse:Ohne „flow diverter“ konnte eine beständige Rotation in beiden – „shear-driven“ und „inertia-driven“ – Flussmodellen beobachtet werden. Die Auswirkungen nach Platzierung eines niedrigporösen Modells wie D1 oder D2 riefen eine komplette Änderung der Flusseigenschaften und eine massive Verringerung der Geschwindigkeit und des Ausmaßes von Verwirbelungen hervor, wohingegen die Platzierung klinisch angewendeter hochporöser Modelle geringere Auswirkungen auf die Kavität des Aneurysmas hatte. Erwartungsgemäß haben die Ergebnisse gezeigt: Je kleiner die Porosität ist, desto größer sind die Auswirkungen auf Blutflussgeschwindigkeit und Verwirbelungen im Aneurysma. Die Kombination mit der geringsten Porosität (D1 und D2) erzielte eine durchschnittliche Reduktion der Flussparameter um 80% bzw. 88% bei konstanten und pulsatilen Flüssen. Die Verminderung von mittlerer Geschwindigkeit und von Verwirbelungen war beim „shear-driven“ Fluss deutlich signifikanter als beim „inertia-driven“ Fluss.Schlussfolgerung:Obwohl die Porosität der wichtigste Parameter zur Senkung des Flusses ist, können andere Parameter wie das Design des jeweiligen Modells oder lokale Flusseigenschaften die Wirksamkeit der Flussreduktion in intrakraniellen Aneurysmen beeinflussen.

BACKGROUND AND PURPOSE: Large IC artery occlusion is often resistant to recanalization. We present our initial experience with the PS. MATERIALS AND METHODS: Presenting with a severe acute ischemic stroke, the first 27 consecutive patients were considered for thromboaspiration therapy and retrospective data base analysis. All patients received standard thrombectomy treatment as monotherapy or in combination with thrombolysis or IC stent placement. The primary end point was revascularization of the target vessel to grade 2 or 3 on the TICI scale. Secondary end points were improvement of >4 points on the NIHSS score at discharge and favorable outcome, and improvement in overall mortality at 3 months and in sICH- and procedure-related adverse events. RESULTS: At baseline, the mean age was 66 ± 14 years and the mean NIHSS score was 14 ± 7. The anterior circulation was affected in 23 patients, and there were 4 basilar artery occlusions. Intracranial stent placement was performed in 4 patients. A recanalization to TICI 2 or 3 was achieved in 25 patients (93%). None of the patients developed sICH. At hospital discharge, 15 patients (56%) had an NIHSS improvement of >4 and 13 patients (48%) had an mRS score of <2 at 3 months. There was a significant correlation between complete vessel recanalization and favorable outcome. The all-cause mortality at 3 months was 11%. CONCLUSIONS: The PS showed a high potential for recanalization of acute thromboembolic occlusions of the large cerebral arteries. Complete recanalization was strongly correlated with good clinical outcome.

BACKGROUNDRuptured aneurysms of < 2 mm are not amenable to endovascular coiling and therefore pose a significant treatment challenge. OBJECTIVETo test recently introduced flow diverters that allow endovascular reconstruction via another method and may represent a new treatment option for such lesions. PATIENTS AND METHODSThree female patients presented with acute subarachnoid hemorrhage. An aneurysm of < 2 mm was identified in all patients as the cause of bleeding. The aneurysms were located at the C2 segment of the internal carotid in 2 patients and on the basilar bifurcation in the other. All patients had failed early endovascular treatment attempts. Flow diversion with the SILK flow diverter was offered as an alternative in each patient. RESULTSSILK deployment successfully eliminated the aneurysms in all 3 instances. One of the aneurysms was excluded from contrast material visualization immediately after stent deployment. Transient thrombotic complication was observed in the case of the basilar artery aneurysm. It resolved with the administration of intraarterial tirofiban. There was no treatment-related morbidity, and none of the aneurysms reruptured after SILK implantation during a clinical follow-up of at least 4 months (range, 4–10 months). Imaging follow-up showed complete vessel remodeling in all cases. CONCLUSIONFlow diversion treatment prevented rebleeding during the follow-up period. Reverse remodeling of the concerned vascular segment with delayed disappearance of the aneurysm was observed in each case.

Background and Purpose: In acute stroke it is no longer sufficient to detect simply ischemia, but also to try to evaluate reperfusion/recanalization status and predict eventual hemorrhagic transformation. Arterial spin labeling (ASL) perfusion may have advantages over contrast-enhanced perfusion-weighted imaging (cePWI), and susceptibility weighted imaging (SWI) has an intrinsic sensitivity to paramagnetic effects in addition to its ability to detect small areas of bleeding and hemorrhage. We want to determine here if their combined use in acute stroke and stroke follow-up at 3T could bring new insight into the diagnosis and prognosis of stroke leading to eventual improved patient management. Methods: We prospectively examined 41 patients admitted for acute stroke (NIHSS >1). Early imaging was performed between 1 h and 2 weeks. The imaging protocol included ASL, cePWI, SWI, T2 and diffusion tensor imaging (DTI), in addition to standard stroke protocol. Results: We saw four kinds of imaging patterns based on ASL and SWI: patients with either hypoperfusion and hyperperfusion on ASL with or without changes on SWI. Hyperperfusion was observed on ASL in 12/41 cases, with hyperperfusion status that was not evident on conventional cePWI images. Signs of hemorrhage or blood-brain barrier breakdown were visible on SWI in 15/41 cases, not always resulting in poor outcome (2/15 were scored mRS = 0–6). Early SWI changes, together with hypoperfusion, were associated with the occurrence of hemorrhage. Hyperperfusion on ASL, even when associated with hemorrhage detected on SWI, resulted in good outcome. Hyperperfusion predicted a better outcome than hypoperfusion (p = 0.0148). Conclusions: ASL is able to detect acute-stage hyperperfusion corresponding to luxury perfusion previously reported by PET studies. The presence of hyperperfusion on ASL-type perfusion seems indicative of reperfusion/collateral flow that is protective of hemorrhagic transformation and a marker of favorable tissue outcome. The combination of hypoperfusion and changes on SWI seems on the other hand to predict hemorrhage and/or poor outcome.

IntroductionThis study was performed to assess the effect of aneurysm geometry on parameters that may have an impact on the natural history of intracranial aneurysms, such as intraaneurysmal flow pressure and shear stress.MethodsFlow was simulated in 21 randomly selected aneurysms using finite volume modeling. Ten aneurysms were classified as side-wall aneurysms, with either single-sided or circumferential involvement of the parent artery wall, and 11 as bifurcation aneurysms (symmetric or asymmetric), with an axis either perpendicular or parallel to the parent artery. The flow patterns were classified as either jet or vortex types (with regular or irregular vortex flow). Pressures and shear stresses were characterized as evenly or unevenly distributed over the aneurysm wall and neck.ResultsAll side-wall and four of the bifurcation aneurysms with a perpendicular axis had a vortex type flow pattern and seven bifurcation aneurysms with a parallel axis (four symmetric and two asymmetric) had a jet flow pattern. Jet type flow was associated with an uneven pressure distribution in seven out of seven aneurysms. Vortex type flow resulted in an even pressure distribution in five out of six aneurysms with an irregular flow pattern and six out of eight with a regular flow pattern. No firm relationship could be established between any geometrical type and shear stress distribution. Only 1 of 14 aneurysms with a perpendicular axis, but 4 of 7 aneurysms with a parallel axis, had ruptured.ConclusionAneurysm geometry does have an impact on flow conditions. Aneurysms with a main axis parallel to the parent artery have a tendency to have a jet flow pattern and uneven distribution of unsteady pressure. These aneurysms may have a higher rate of rupture as than those with a main axis perpendicular to the parent artery.

SummaryBackground and purpose. To assess the feasibility and results of parent vessel stent reconstruction with balloon expandable and self-expandable stents in the treatment of intracranial aneurysms. Methods. In a total of 18 aneurysms balloon expandable (group A) and self expandable (group B) stents were used in 9 cases each. Stent implantation alone was used in 3 cases, and additional coil packing in the other 15. Results. Stents were successfully deployed in 8 out of 9 in group A and in 9 out of 9 cases in group B. Nearly complete occlusion was achieved in all but one case. At 3 or 6 months stable occlusion was found in 4 group A and 2 group B patients, progressive thrombosis in 3 cases in both groups, and recanalisation in 1 case in group B. Late follow up at 1–4 years demonstrated one progressive thrombosis one recanalisation and 1 stable occlusion in 3 group A, and 2 stable occlusions in 2 group B. patients. Complications included one aneurysm perforation in group A, one in-stent thrombosis and a distal arterial perforation in group B and one groin hematoma in both groups. Conclusion. Stent reconstruction of intracranial arteries harbouring aneurysms is feasible and may result in aneurysm thrombosis without coil packing in some cases. Self expanding stents seem to provide a higher rate of success. Aggressive antiplatelet treatment increases the risk of hemorrhagic complications.
