cm. By shielding the detector units with enough slant mass to absorb the electromagnetic component in the air showers, a clean measurement of the muon content is allowed, a concept to be implemented in a next generation of UHECR experiments. The operation of a ground array detector poses challenging demands, as the RPC must operate remotely under extreme environmental conditions, with limited budgets for power and minimal maintenance. The RPC, DAQ, High Voltage and monitoring systems are enclosed in an aluminium-sealed case, providing a compact and robust unit suited for outdoor environments, which can be easily deployed and connected. The RPCs developed at LIP-Coimbra are able to operate using a very low gas flux, which allows running them for few years with a small gas reservoir. Several prototypes have already been built and tested both in the laboratory and outdoors. We report on the most recent tests done in the field that show that the developed RPCs have operated in a stable way for more than 2 years in field conditions.

Rationale: In many countries sip feeding (oral nutritional supplements, ONS) at home is not covered by the compulsory health insurance, contrary to the service of tube feeding. The economic consequences of this statutory rule were analyzed and reimbursement of sip feeding was requested by the Swiss Federal Office of Public Health with success. Methods: Average treatment costs of sip and tube feeding were calculated. Additional costs and direct cost savings caused by the statutory rule were calculated. Indirect costs caused by malnutrition in Switzerland were estimated based on the commissioned report of the Swiss Federal Office of Public Health. Results: Tube feeding is more than twice as expensive as sip feeding (EUR 325 and 140, respectively), when adjusted for calories and period of treatment. 40% of sip feeding at home was not covered by the compulsory health insurance, leading to cost savings of EUR 250,000. However, in 10% of these cases tube feeding was initiated after refusal of the reimbursement of costs. The extra costs of these unnecessarily tube feedings amounted to approximately EUR 50,000 per year. Furthermore, malnutrition negatively affects rate of complications, mortality and effort for medical care. In Switzerland, this is responsible for additional costs of EUR 430,000,000 per year. Conclusion: The analysis shows that reimbursement of medically indicated ONS is cost-effective: Costs arising from reimbursement are largely covered by direct cost savings within 5 years. In the long term additional cost savings are expected by improving the therapy of malnourished patients and thereby reducing the costs generated by malnutrition.

OBJECTIVE Early dental caries diagnosis within the changing global pattern of dental caries development requires the use of auxiliary resources and adjuncts to diagnosis. The aim of this study was to introduce an alternative resource for drying teeth to enable early diagnosis of dental caries in epidemiological surveys. METHODS Polyurethane tips and three-in-one syringes were fitted to nebulizers and non-professional portable compressors for dental surface drying. The output air pressure of these sets was compared with the output pressure from the three-in-one syringe in a dental office. RESULTS Although the output pressure from the alternative resources was lower than the output pressure from the dental office equipment, the dental surfaces were dried satisfactorily, allowing the early diagnosis of the dental caries. CONCLUSION When a dental setting is not available, these alternative resources for drying teeth can be used satisfactorily.
