Anticoagulant-induced intramural intestinal hematoma

Intramural hematoma of the intestine is a rare complication of anticoagulant therapy. We evaluated 7 nontrauma patients with intramural hematoma of the intestine diagnosed at our institution between May 1998 and June 2001. All of the patients were receiving long-term anticoagulant therapy for previous diseases. All 7 patients had abdominal pain, 6 had additional symptoms of nausea and vomiting, and 4 had melanotic stools at admission. Six of the patients had abnormal coagulation parameters. Both abdominal ultrasonography (US) and computerized tomography (CT) showed the exact pathology in all patients. Five of the 7 patients were treated, nonoperatively, and the other patients underwent surgery. All of the patients were followed with abdominal US and CT (mean 12 = months), with complete resolution of their intramural hematomas. Abdominal US and CT evaluation performed together will help the accuracy of diagnosis of intramural hematoma, but nonoperative therapy is the treatment of choice, with surgery indicated if generalized peritonitis or intestinal obstruction develops.

Results of alcohol and albendazole injections in hepatic hydatidosis: Experimental study

Background:  Percutaneous drainage with alcohol injection for hydatid cysts has been commonly used in the last two decades. Albendazole is the drug of choice in the medical treatment of hydatidosis, and has also been used as an intraoperative scolicidal solution. The side‐effects of its local application are not well known and have not been investigated. The purpose of the present study was to investigate the effects of the intracystic injections of alcohol and albendazole solutions on the hydatid cysts and hepatobiliary system of rabbits.

Dual treatment of albendazole in hepatic hydatidosis: New therapeutic modality in 52 cases

Background:  Hepatic hydatidosis still remains as a serious problem in general surgery. Recurrence and/or secondary hydatidosis rates are up to 25% in the cases treated with surgery alone. Albendazole is the most commonly used drug in the medical treatment of echinococcosis. The aim of the present study was to evaluate the effect of albendazole using the intraoperative and perioperative periods as dual therapy.

T.:+90 326 2111900 +90 532 5060009 E-mail:[email protected] Özet Amaç: Genel cerrahların; acil dışındaki ameliyatlar arasında, en fazla yaptıkları ikinci sırada yer alan ameliyat fıtık onarımıdır. Shouldice herniorafi klasik ameliyatlardan biri olup gerilimli olarak adlandırılan ameliyat sınıfında yer almaktadır. Bu ameliyat her cerrahın repertuarında bulunmalıdır. Cerraha bir alternatif sağladığı gibi, gerilimsiz tekniklerin uygulanamayacağı vakalarda kullanımı zorunlu hale gelebilir. Bu çalışmada Lichtenstein herniorafi ve Shouldice herniorafi arasındaki avantaj ,dezavantaj ve komplikasyonlarını literatür eşliğinde inceledik. Gereç ve Yöntem: Hastanemizde yapılan Lichtenstein herniorafi ve seçilmiş hastalardaki Shouldice herniorafi vakalarını karşılaştırdık. Nisan 2007-mayıs 2008 tarihleri arasında inguinal herni tanısıyla Samsun Bafra Devlet Hastanesi Genel Cerrahi Servisi’nde Lichtenstein herniorafi yapılan 75 hasta ile Shouldice herniorafi yapılan 33 hasta takibe alınmıştır. Hastaların yaş-cins dağılımı, herni tipi, anestezi şekli, ortalama yatış süresi, erken ve geç komplikasyonları incelenmiştir. Bulgular: Postoperatif dönemdeki erken komplikasyonlar; idrar retansiyonu, yara enfeksiyonu ve hematom gelişmesi olarak gözlenmiştir. İdrar retansiyonu gelişen hastaların hepsine spinal anestezi uygulanmış olup, geçici idrar sondası takılması gerekti. Yüzeyel yara enfeksiyonu drenaj ve antibiyoterapi ile tedavi edildi. Hastaların geç dönem komplikasyonları incelendiğinde Schouldice herniorafi yapılan 1 hastada uylukta parestezi, lichtenstein herniorafi yapılan 1 hastada postoperatif 5.ayda nüks gelişti. Sonuç: Her cerrahın bilmesi gerektiğini düşündüğümüz bu tekniğin cerrahi eğitimi veren kurumlarda yeni yetişen cerrahlara alternatif bir yöntem olarak verilmesi gerektiğini düşünüyoruz. Cerrahiye bir alternatif sağladığı gibi, gerilimsiz tekniklerin uygulanamayacağı vakalarda doğrudan tercih edilebilir.

Mesh repair of a pelvic bone defect caused by a migrated acetabular cup

Abstract. Conventional transacetabular removal of the migrated acetabular cup can be hazardous due to intraoperative injury to iliac vessels. We present a case of a migrated acetabular cup, in which we used a combined preperitoneal and acetabular approach for its removal. With a bimanual approach, the procedure was safer and easier and allowed mesh repair of the pelvic bone defect. The preperitoneal mesh repair is a well-known method for inguinofemoral hernias. However, it has not been used before in the repair of an acetabular defect after removal of a migrated cup.
