TRANSFERÊNCIA DE POLÍTICA: PERSPECTIVA DO TRATAMENTO DIRETAMENTE OBSERVADO DA TUBERCULOSE

Objective: to investigate how the policy transfer of the Directly Observed Treatment of Tuberculosis was developed from the perspective of the community health professionals of the Conceicao Hospital Group. Method: it is a quantitative research of the epidemiological inquiry type, which was based on the application of a validated and self-directed instrument to the health professionals belonging to the 12 Primary Health Care Units to the Community Health Service of the Conceicao Hospital Group. For the analysis, the beta regression, the description of the frequencies of the variables, the distribution of the mean, median and the standard deviation were used. In addition, the standardized mean and standardized scores were created. Results: 109 health professionals participated in the study, most of them female (77.06%), aged between 30 and 59 years old (87.16%) and belonging to the medical category (37.61%). Regarding the three analyzed dimensions of the policy transfer process, it was observed that the Knowledge dimension was the one that had the highest score (77.8%), followed by the dimensions Information (72.7%) and Innovation (67.7%). The need for involvement and participation of the population in the discussions of Directly Observed Treatment, improvement of the infrastructure of the health services and the complexity of the process of accomplishment of this practice were some, among others, important aspects pointed out. Conclusion: it is necessary to reformulate and improve the actions related to the operationalization and transfer of the Directly Observed Treatment of Tuberculosis

Obstacles in the detection and reporting of tuberculosis cases: a discursive analysis

OBJECTIVE To analyze the discourses of health professionals about the obstacles in the process of detection and reporting of tuberculosis cases in Mozambique. METHOD Qualitative exploratory study with a theoretical-methodological approach of Discourse Analysis of French matrix. The study was conducted in Mozambique in 2014 at three levels: central, provincial and district. The study included 15 health professionals, 4 physicians, 6 technicians and 5 nursing professionals, who worked in the National Tuberculosis Control Program, with more than 1 year of experience. RESULT The following discursive blocks emerged: Detection of tuberculosis cases in laboratories; Underreporting of tuberculosis cases; Obstacles to detect cases of tuberculosis: long distances and lack of transport; and Reporting of cases for decision making. FINAL CONSIDERATIONS The discourses analyzed point to the ideological affiliation that includes the lack of investment policies in the health sector and the political commitment as basic obstacles in the detection and reporting of tuberculosis cases.

Discursos sobre a terapia de curta duração para o controle da tuberculose

Objetivo: analisar os sentidos produzidos por meio das experiencias e percepcoes dos profissionais de saude sobre a implementacao da estrategia do tratamento diretamente observado de curta duracao para o controle da tuberculose. Metodo: estudo qualitativo realizado em Mocambique com 15 profissionais de saude. Criterios de inclusao: atuacao no Programa de Controle da Tuberculose ha mais de um ano e estar em atividade profissional no periodo da entrevista. Para a coleta de dados foi usado um roteiro semiestruturado e as entrevistas realizadas individualmente foram gravadas. Resultados: emergiram tres unidades discursivas: os procedimentos no tratamento do doente; o papel de Agente Comunitario da Saude e da familia; as dificuldades na aplicacao do tratamento diretamente observado de curta duracao. Conclusao: o tratamento da tuberculose requer um envolvimento constante da familia, da comunidade e principalmente do Estado na criacao e aplicacao de politicas de formacao dos recursos humanos e melhoria de infraestruturas.Objective: to analyze the meanings produced through the experiences and perceptions of healthcare professionals on the implementation of the strategy of directly observed treatment short course for tuberculosis control. Intervención educativa para recolección de esputo de la tuberculosis: un estudio casi experimental

Objetivo: avaliar a qualidade da amostra de escarro antes e apos as orientacoes de Enfermagem ao paciente. Metodos: estudo com delineamento de pesquisa quase experimental, do tipo grupo unico, antes e depois, nao randomizado. Participaram do estudo pacientes com suspeita de tuberculose pulmonar, sintomaticos respiratorios por mais de tres semanas, maiores de 18 anos, de ambos os sexos e sem antecedente de tuberculose nos ultimos dois anos. A intervencao educativa consistiu em orientacoes individualizadas sobre a coleta da amostra de escarro, fundamentadas nas diretrizes do Ministerio da Saude do Brasil e na entrega de folder explicativo. Resultados: participaram 138 pacientes com suspeita de tuberculose pulmonar. Os resultados evidenciaram importante acrescimo das amostras com particulas purulentas, volume maior que 5mL e aumento na taxa de pacientes diagnosticados com tuberculose, apos a intervencao educativa. Conclusao: comprovou-se que, apos a intervencao educativa, obtiveram-se amostras de escarro com maior qualidade, com aspecto e volume satisfatorios para efetividade do exame baciloscopico.Abstract Objective: to evaluate the quality of the sputum sample before and after the Nursing guidance to patients. Methods: this is a quasi-experimental research design, single group type, before and after, non-randomized study. The study enrolled patients with suspected pulmonary tuberculosis, respiratory symptomatic patients for over three weeks, aged over 18 years, of both genders and without tuberculosis history in the last two years. The educational intervention consisted of individualized guidance on the collection of sputum sample, which was based on the guidelines of the Ministry of Health of Brazil and on the explanatory folder delivery. Results: in this study participated 138 patients with suspected pulmonary tuberculosis. The results showed significant increase of the samples with purulent particles, volume greater than 5 mL and increased rate of patients diagnosed with tuberculosis, after the educational intervention. Conclusion: it was shown that after the educational intervention, it was observed sputum samples with better quality, with satisfactory aspect and volume for the effectiveness of the bacilloscopic examination.
