Prematurity and associated factors in Santa Catarina State, Brazil, in 2005: an analysis based on data from the Information System on Live Births

O presente estudo objetivou estimar a prevalencia da prematuridade no Estado de Santa Catarina, Brasil, e identificar seus fatores associados. Os dados foram obtidos no banco de dados do Sistema de Informacoes sobre Nascidos Vivos (SINASC) de Santa Catarina. Foram aplicados os testes qui-quadrado e de tendencia linear de Wald entre a variavel dependente (prematuridade) e as independentes. Para verificar a associacao dos fatores estudados a prematuridade realizou-se a analise de regressao logistica multipla. Em 2005, a prevalencia de prematuridade foi igual a 6,1% (IC95%: 5,9-6,2). A chance de prematuridade foi maior quanto menor o numero de consultas pre-natal, em maes com idade superior a 40 e inferior a 20 anos e em maes submetidas ao parto cesariano. A prevalencia de prematuridade parece estar aumentando em Santa Catarina. Dado o atual panorama no estado, e possivel sugerir que o incentivo a assistencia pre-natal e o desestimulo a cesariana eletiva poderiam contribuir com a reducao da prematuridade. Mais estudos utilizando os bancos de dados existentes e apontando suas fragilidades podem contribuir para a melhoria da qualidade dessas informacoes.The aim of this study was to evaluate the prevalence of preterm birth in the State of Santa Catarina, Brazil, and identify associated factors. Data were obtained from the SINASC database (Information System on Live Births). The chi-squared test and Wald linear trend test between the dependent variable (prematurity) and the covariates were applied. Logistic regression was performed to evaluate the association between factors. In 2005, the preterm birth rate was 6.1% (95% CI: 5.9-6.2). Odds of prematurity were higher among mothers under 20 and over 40 years old, those with fewer prenatal consultations, and those undergoing cesarean sections. Prevalence of prematurity in Santa Catarina appears to be increasing. This scenario suggests the need to encourage prenatal care and discourage cesarean deliveries in order to decrease the prematurity rate. More studies using the existing databases could help identify their shortcomings and improve the quality of the data.

Validity of Partial Protocols to Assess the Prevalence of Periodontal Outcomes and Associated Sociodemographic and Behavior Factors in Adolescents and Young Adults

BACKGROUND Most studies comparing prevalence of periodontal disease and risk factors by using partial protocols were performed in adult populations, with several studies being conducted in clinical settings. The aim of this study is to assess the accuracy of partial protocols in estimating the prevalence of periodontal outcomes in adolescents and young adults from two population-based birth cohorts from Pelotas, Brazil, and to assess differences in the estimation and strength of the effect measures when partial protocols are adopted compared to full-mouth examination. METHODS Gingival bleeding at probing among adolescents (n = 339) and young adults (n = 720) and dental calculus and periodontal probing depth among young adults were assessed using full-mouth examinations and four partial protocols: Ramfjord teeth (RT), community periodontal index (CPI), and two random diagonal quadrants (1 and 3, 2 and 4). Socioeconomic, demographic, and periodontal health-related variables were also collected. Sensitivity, absolute and relative bias, and inflation factors were calculated. Prevalence ratio for each periodontal outcome for the risk factors was estimated. RESULTS Two diagonal quadrants showed better accuracy; RT had the worst, whereas CPI presented an intermediate pattern when compared to full-mouth examination. For bleeding assessment in adolescence, RT and CPI underestimated by 18.4% and 16.2%, respectively, the true outcome prevalence, whereas among young adults, all partial protocols underestimated the prevalence. All partial protocols presented similar magnitude of association measures for all investigated periodontal potential risk factors. CONCLUSION Two diagonal quadrants protocol may be effective in identifying the risk factors for the most relevant periodontal outcomes in adolescence and in young adulthood.

Tabagismo no Brasil: desigualdades regionais e prevalência segundo características ocupacionais

This study describes the prevalence of daily tobacco smoking according to sex, age, per capita household income and occupation of residents aged 15 years or more in Brazil and regions using data from the 2008 National Household Sample Survey (PNAD/IBGE). The analysis was adjusted for the sampling design and included 252.768 individuals. Daily smoking prevalence in Brazil was 15.1%, varying from 12.8% in the North region to 17.4% in the South region, and it was 62% higher in men compared to women. Smoking prevalence was inversely proportional to household income, 18.6% among the poorest 20% and 11.5% among the wealthiest 20%. The same trends for gender, age and income were observed in the different regions of Brazil. Daily smoking was 3% higher among workers compared to non-workers. White collar workers presented a smoking prevalence below 10%, while blue collar workers had rates above 20%. The association between smoking and occupation persisted after the adjustment for sex, age and household income. The inequalities found should be considered when developing effective strategies for smoking reduction. The more exposed occupational groups should have priority in the interventions.

Aspectos sociodemográficos e clínicos da qualidade de vida relacionada à saúde bucal em adolescentes

OBJETIVO: Estimar a prevalencia e identificar fatores sociodemograficos e parâmetros bucais associados ao impacto negativo da condicao bucal na qualidade de vida de adolescentes. METODOS: Foram analisados dados de 5.445 adolescentes entre 15 e 19 anos que participaram do inquerito nacional de saude bucal (SBBrasil 2010), considerando a complexidade do desenho amostral. O desfecho foi a qualidade de vida relacionada a saude bucal, avaliada por meio do questionario Oral Impacts on Daily Performance e analisada de forma discreta. As variaveis de exposicao foram sexo, cor da pele, escolaridade, renda familiar, idade, carie nao tratada, perda dentaria, dor de dente, oclusopatias, sangramento gengival, calculo dentario e bolsa periodontal. Foram conduzidas analises de regressao de Poisson e apresentadas as razoes de medias (RM), com respectivos intervalos de 95% de confianca (IC95%). RESULTADOS: Dos pesquisados, 39,4% relataram pelo menos um impacto negativo na qualidade de vida. Apos o ajuste, a media do impacto negativo foi de 1,52 (IC95%1,16;2,00) vez maior no sexo feminino e 1,42 (IC95% 1,01;1,99), 2,66 (IC95% 1,40;5,07) e 3,32 (IC95% 1,68;6,56) vezes maior nos pardos, amarelos e indigenas, respectivamente, em relacao aos brancos. RESULTS Of the total, 39.4% reported at least one negative impact on their quality of life. After adjustment, the mean negative impact was 1.52 (95%CI 1.16;2.00) times higher in females and 1.42 (95%CI 1.01;1.99), 2.66 (95%CI 1.40;5.07) and 3.32 (95%CI 1.68;6.56) higher in those with brown, yellow, and indigenous skin color, respectively, when compared to those with white skin. The lower the level of schooling, the greater the negative impact (MR 2.11, 95%CI 1.30;3.41), likewise for individuals with household income below R

The greatest impact on quality of life was found among adolescents with four or more teeth with untreated dental caries (MR 1.53, 95%CI 1.12;2.10), one or more missing teeth (MR 1.44. 95%CI 1.16;1.80). those with dental pain (RM 3.62, 95%CI 2.93;4.46) and with severe (MR 1.52, 95%CI 1.04;2.23) and very severe malocclusion (MR 1.32, 95%CI 1.01;1.72). CONCLUSIONS Brazilian adolescents reported a high negative impact of oral health on their quality of life. Inequalities in distribution should be taken into account when planning preventive, monitoring and treatment strategies for oral health problems in groups with the highest impact on their quality of life.

Time trend of asthma in children and adolescents in Brazil, 1998-2008

The aim of this study was to estimate the prevalence of dental and gingival pain and associated factors among Brazilian adolescents (15-19 years of age). Data from 16,126 adolescents who participated in the Brazilian Oral Health Survey SB-Brazil 2002-2003 were used. The outcome measured was dental and gingival pain in the last six months. Independent variables were per capita income, schooling, school enrollment, gender, skin color, age, area of residence, time since last dental appointment, type of dental service, DMFT index and its components, dental calculus, and Dental Aesthetic Index. Simple and multiple Poisson regression analyses were performed. Prevalence of dental and gingival pain was 35.6% (95%CI: 34.8-36.4). Increased prevalence of pain was associated with: female gender, low income, non-students, students enrolled in public schools, and grade-for-age lag. In addition, adolescents with high levels of dental caries and dental calculus also reported higher prevalence of dental pain. Dental and gingival pain can be considered a relevant public health problem, suggesting the need for preventive measures.
