Uso regular de serviços odontológicos entre adultos: padrões de utilização e tipos de serviços

The aim of this study was to estimate the prevalence of regular use of dental services by adults and identify groups where this behavior is more frequent. A cross-sectional population-based study was carried out in Pelotas, southern Brazil, including 2,961 individuals who answered a standardized questionnaire. Overall prevalence of regular use of dental services was 32.8%. The following variables were positively associated with regular use: female gender, age >or= 60 years, no partner, high educational level, high economic status, private service user, good/excellent self-rated oral health, and no perceived need for dental treatment. Those who had received orientation on prevention and expressed a favorable view towards the dentist had higher odds of being regular users. Especially among lower-income individuals, regular use was infrequent (15%). When restricting the analysis to users of public dental services, schooling was still positively associated with the outcome. Dental services, especially in the public sector, should develop strategies to increase regular and preventive use.O objetivo deste estudo foi avaliar o uso regular de servicos odontologicos entre adultos, identificando grupos nos quais esse comportamento e mais frequente. O estudo foi realizado em Pelotas, Rio Grande do Sul, Brasil, com desenho transversal de base populacional, envolvendo 2.961 individuos, que responderam um questionario estruturado. A prevalencia de uso regular encontrada foi de 32,8%. Estiveram positivamente associadas ao uso de forma regular as seguintes caracteristicas: ser do sexo feminino, ter idade < 60 anos, nao ter companheiro, ter alta escolaridade, ter maior nivel economico, usar servicos privados, ter autopercepcao de saude bucal boa/otima, nao ter autopercepcao de necessidade de tratamento, ter recebido orientacao sobre prevencao e ter manifestado opiniao favoravel ao profissional. O uso regular foi pouco frequente, especialmente entre os menos favorecidos (15%). Restringindo a analise apenas para usuarios do servico publico, observou-se maior utilizacao regular entre os mais escolarizados. Preditores da realização de consultas odontológicas de rotina e por problema em pré-escolares

OBJECTIVE: To estimate the prevalence of dental visits among preschool children and determine the factors associated with using dental services. METHODS: A cross-sectional study was conducted with 1,129 five-year-old children from the Pelotas Birth Cohort Study in Pelotas (Southern Brazil) 2004, from September 2009 to January 2010. Use of dental services at least once in the childs life and the reason for the childs first dental visit were recorded. The categories assigned for the first dental visit were: routine check-up, resolution of a problem, or never saw a dentist. The oral examinations and interviews were performed in the childrens homes. Socioeconomic aspects and independent variables related to the mother and child were analyzed using multivariable logistic regression. RESULTS: The prevalence of dental visits (both categories combined) was 37.0%. The main predictors for a routine visit were higher economic status, mothers with more schooling, and mothers who had received guidance about prevention. Major predictors for a visit because of a problem were having felt pain in the previous six months, mothers with higher education level, and mothers who had received guidance about prevention. Approximately 45.0% of mothers received information about how to prevent cavities, usually from the dentist. Children of mothers who adhered to health programs were more likely to have had a routine dental visit. CONCLUSIONS: The rate of preschool visits to dental services was lower than the rate for medical appointments (childcare). In addition to income and education, maternal behavior plays an important role in routine visits. Pain reported in the last six months and a high number of teeth affected by tooth decay, independent of other factors, were associated with visits for a specific problem. Uso regular de serviços odontológicos entre adultos e idosos em região vulnerável no sul do Brasil

OBJECTIVE To estimate the prevalence of regular use of dental care services by adults and older adults residing in vulnerable community and to identify associated factors. METHODS A population-based cross-sectional study was carried out with 3,391 adults and older adults residing in areas of social vulnerability in Porto Alegre, Southern Brazil, from July to December of 2009. A systematic sampling method was used the selection probability proportional to the population of each of the the 121 census sectors. The outcome for regular use of dental care services was defined as regular use of dental services, regardless of the presence of dental problems. A standardized questionnaire was administered, which included demographic, socioeconomic, type of dental care services, self-perception of dental health and self-perceived needs variables. A chi-square test for heterogeneity was used for bivariate analyses, and a Poisson regression with a robust variance and Wald tests were performed for the adjusted analysis. RESULTS The prevalence of regular use of dental services was 25.7%. The prevalence was higher among people with >12 years schooling (PR 2.48 [95%CI:1.96;3.15]), higher income (PR 1.95[95%CI: 1.03;1.53]), use of private health services (PR 1.43 [95%CI: 1.20;1.71]),excellent self-perceived oral health (PR 4.44 [95%CI: 3.07;6.42]) and a self-perceived need for consultation related to routine checkup (RP 2.13 [95%CI: 1.54;2.96]). CONCLUSIONS Inequalities were found in the regular use of dental services. Integrated approaches that raise awareness of oral health, improve self-care and expand access to dental services, may contribute to increase the use of dental services on a regular basis.

Peso e altura autorreferidos para determinação do estado nutricional de adultos e idosos: validade e implicações em análises de dados

This study evaluated the validity of self-reported weight and height for determining nutritional status and the implications of their use for analyzing associations with health outcomes. A population-based cross-sectional study in Pelotas, Rio Grande do Sul State, Brazil, in 2007 (n = 2,986) drew a sub-sample of 276 individuals aged ≥ 20 years. Mean self-reported weight was similar to measured weight; height was overestimated in men (1.4 cm) and women (2.5 cm); real body mass index (BMI) was underestimated by about 1 kg/m(2). Even with small mean differences, data variability was great. The results were influenced by gender, age, and schooling. The use of self-reported measures underestimated prevalence of overweight and obesity, and unpredictable errors were found in the analysis of association with health outcomes (underestimation, overestimation, and reversal of real effect measures). Correction equations reduced the mean differences but did not resolve variability of the differences, classification errors, or biases in the associations.

Experiência de cárie e utilização do serviço público odontológico por escolares: estudo descritivo em Arroio do Padre, Rio Grande do Sul, 2013

OBJECTIVE: to describe the use of public dental services and to assess deft/DMFT indexes in the city of Arroio do Padre, state of Rio Grande do Sul, Brazil. METHODS: this was a cross-sectional study of all children aged 5 and 12 living in the city in 2013 (N=50); the children underwent oral health exams and the adults responsible for them answered questionnaires. RESULTS: mean deft and DMFT were 3.9 and 1.3. In deciduous teeth (deft) there was predominance of the decay component (82%) while in the permanent dentition (DMFT) the filled component was predominant (65%); 19 children students had decayed teeth at the time of the examination and 13 of these had had a dental visit at the Basic Health Unit. CONCLUSION: most children presented caries history, which may be attributed in part to low exposure to fluoride, low parental education and little appreciation of deciduous teeth.OBJETIVO: descrever a utilizacao do servico publico odontologico e os indices de ceo-d e CPO-D no municipio de Arroio do Padre-RS, Brasil. METODOS: estudo descritivo censitario com todos os residentes nas idades de cinco e 12 anos, em 2013; foram realizados exames bucais e aplicado questionario junto aos responsaveis. RESULTADOS: participaram do estudo 50 escolares, 22 com cinco e 28 com 12 anos de idade; as medias de ceo-d e CPOD foram de 3,9 e 1,3 respectivamente; na denticao decidua (ceo-d), houve predominio do componente cariado (82%), e na permanente (CPO-D), o predominio foi do componente obturado (65%); do total, 19 escolares apresentaram dentes cariados no momento do exame e destes, 13 haviam consultado com dentista da unidade basica de saude. CONCLUSAO: a maioria dos escolares apresentava historia de carie, possivelmente atribuida - em parte - a pouca exposicao ao fluor, baixa escolaridade dos pais e pouca valorizacao da denticao decidua.

Prevalência e fatores associados à prática de esportes individuais e coletivos em adolescentes pertencentes a uma coorte de nascimentos

O presente estudo investigou a pratica de esportes individuais e coletivos e fatores associados em jovens com idade media de 11 anos, pertencentes a uma coorte de nascimentos. Informacoes dos jovens e de suas maes foram coletadas por meio de questionarios. O desfecho foi dividido em pratica de esportes individuais e coletivos. Uma analise por meio de regressao de Poisson foi conduzida para estabelecer os fatores associados a pratica esportiva, obedecendo a um modelo conceitual de analise com as variaveis independentes hierarquizadas. Um total de 4350 jovens foi estudado. A pratica de esportes coletivos foi menor nas meninas - 68,1% (IC95% 66,2 - 70,0) em comparacao aos meninos - 82,1% (IC95% 80,5 - 83,7). Nos esportes individuais, a prevalencia foi de 12,9% (IC95% 11,6 - 14,4) e 18,9% (IC95% 17,3 - 20,6) em meninas e meninos, respectivamente. Entre as meninas, a pratica de esportes individuais relacionou-se diretamente com nivel economico e inversamente com assistir televisao. Nos meninos este desfecho associou-se com estudar em escolas privadas e ter maes fisicamente ativas. Em esportes coletivos, a pratica foi menor naqueles que despendem maior tempo assistindo TV. Em ambos os sexos houve uma relacao direta entre o uso regular do video-game e a pratica de esportes. Politicas publicas devem considerar os diferentes aspectos relacionados a pratica esportiva e atender a disparidades socioeconomicas no acesso a diferentes modalidades esportivas. Alem disso, o maior acesso a diferentes locais de praticas esportivas, bem como o incentivos de pais, amigos e ambiente escolar devem ser fortalecidos.
