Readmission from orthopedic surgical site infections: an integrative review

OBJECTIVE Characterizing readmissions from orthopedic surgical site infections. METHOD An integrative review of literature in the LILACS, IBECS, MEDLINE, Cochrane, SciELO and PUBMED databases, using the descriptors Patient readmission, Wound infection, Cross infection, Orthopedic procedures, Orthopedics. RESULTS 78 studies were identified and 10 publications were selected. Surgical site infections are the most common cause of unplanned orthopedic readmissions, representing long periods of hospitalization, new surgical procedures and high costs, and greater possibility of subsequent hospitalizations. Most significant predictors have indicated average length of hospitalization, need for intensive care, emergency status at admission, risk of death, age > 65 years, males and higher body mass index. CONCLUSION Readmission rates have increasingly become measures of quality and concerns about costs. New studies could involve issues related to indirect costs, specifically social and psychological costs.OBJETIVO Caracterizar los reingresos por infeccion del sitio quirurgico ortopedico. RESULTADOS Fueron identificados 78 estudios y seleccionadas 10 publicaciones. La infeccion del sitio quirurgico es la causa mas frecuente entre los reingresos ortopedicos no planificados, que representan largas estancias hospitalarias, nuevas intervenciones quirurgicas y costos elevados, ademas de mayor posibilidad de ingresos subsiguientes. Los factores predictivos mas significativos senalaron tiempo de estancia media, necesidad de cuidados intensivos, status de urgencia en el ingreso, riesgo de muerte, edad >; 65 anos, sexo masculino y mayor indice de masa corporea. CONCLUSION Las tasas de reingreso se hacen cada vez mas medidas de cualidad y preocupacion con relacion a los costos. Nuevos estudios podrian involucrar cuestiones relacionadas con costos indirectos, especialmente los sociales y psicologicos.

Sexual activity of people with spinal cord injury: development and validation of an educational booklet

Objetivo: Descrever o processo de construcao e validacao de uma cartilha educativa sobre atividade sexual de pessoas com lesao medular. Metodos: Estudo metodologico em quatro etapas: pesquisa documental em blogs, revisao integrativa, construcao da cartilha educativa, validacao de conteudo com 11 juizes, validacao de aparencia tecnica com 3 juizes e validacao de aparencia com 37 pessoas do publico-alvo. Considerouse o Indice de Validade de Conteudo (IVC) minimo de 0,80. Resultados: A cartilha apresentou IVC global de 0,91 pelos juizes profi ssionais de saude; 0,87 pelos juizes designers grafi cos; e 0,94 na validacao de aparencia pelo publico-alvo. Entretanto, os juizes e publico-alvo propuseram sugestoes de melhorias da cartilha, que foram acatadas e modifi cadas para versao fi nal do material. Abstract Objective: To describe the process of development and validation of an educational booklet about the sexual activity of people with spinal cord injury. Methods: Methodological study performed in four steps: documentary search in blogs, integrative review, development of the educational booklet, and face and content validation, namely content validation with 11 referees, technical face validation with three referees, and face validation with 37 people from the target audience. The minimum value established for the content validity index (CVI) was 0.80. Results: The booklet presented a global CVI of 0.91 after evaluation by healthcare professionals, 0.87 after inspection by graphic designers, and 0.94 after examination by members of the target audience. Despite the signifi cantly high indexes, referees and people from the target audience proposed suggestions to improve the booklet, which were accepted and incorporated into the fi nal version of the material. Conclusion: The booklet showed content and face adequate to promote healthy and satisfactory sexual activity of people with spinal cord injury.

Priority nursing foci and interventions for older people in acute care settings

Background: Priority nursing foci in acute care delivery to older patients are influenced by multiple factors, which may compromise care quality. Objectives: To describe nurses’ perception of the priority foci in the most frequent nursing interventions aimed at older inpatients in acute care settings, as well as to identify the association between priority nursing foci and other variables. Methodology: Exploratory, descriptive, correlational study using a non-random convenience sample of 124 nurses (♀=100, 80.6%; M=29.8 years; SD=6.75). A questionnaire was used to assess nurses’ perception of self-care priorities, and the frequency and importance of nursing interventions. Results: The most important self-care priorities were self-grooming, self-dressing/undressing, and self-transferring. Significant differences were found between frequency and importance in the nursing interventions of attending (Z=-3.272; p=0.001) and informing (Z=-4.824; p=0.000). Significant negative correlations were found between self-grooming and nurses’ age (rs=-0,203; p= 0.032), length of service (rs=-0,194; p=0.041), and the number of nursing care hours per older patient week (rs=-0.257; p=0.007). Conclusion: Nurses’ initial and lifelong training, together with institutional strategies, should be based on a new paradigm.
