Lupus vulgaris is a chronic paucibacillary form of cutaneous tuberculosis occurring in a person with a moderate to high degree of immunity. It commonly occurs over the buttocks and trunk in India. Involvement of the genitalia is uncommon, and lesions involving the penis, extremely rare with few cases reported worldwide. There also exists a confusion regarding nosology of tuberculosis of the genitalia. A brief discussion and review of literature are being discussed along with the report of a case of genital tuberculosis involving the glans penis.

A middle-aged male patient of about 34 years, from a village in a district of southern Bengal in India, presented to the dermatology outpatient department (Burdwan Medical College, Burdwan, West Bengal, India) with complaints of multiple painless raised lesions with slight oozing for the last seven days. The man belonged to a tribe dealing with the manufacturing of goods from the hides of carcasses of dead cattle and livestock. He had been involved in the handling of a cow that had recently died from an unknown cause, but there were signs of bleeding from the natural orifices before its death. On examination, the patient had three to four vesicles with serosanguinous discharge situated over the arm and fingers (Fig. 1a–c). Some of the lesions were surrounded by smaller vesicles, and there was intense perilesional edema (Fig. 2). Few lesions had a central depression with onset of eschar formation. Systemic examination was within normal limits. The discharge from the lesions was sent for culture and staining. As the patient had already received prophylactic antibiotic therapy, the culture from the serosanguinous discharge turned out to be inconclusive. Based on the history and typical clinical findings and lack of any systemic involvement, a diagnosis of cutaneous anthrax was established. The patient was started on oral ciprofloxacin, 500 mg twice daily for 60 days, and asked to come for follow-up, and the lesions resolved with the prescribed treatment. Discussion

Sir, Rhinosporidiosis is a chronic granulomatous infection caused by Rhinosporidium seeberi, a protistan parasite in the class Mesomycetozoea as per its recent taxonomic destination.1 Nose and eyes being the most common sites, penis and urethra manage to secure much lower rank among the sites of occurrence. Being a disease of world‐wide distribution, this menace has managed to intrude into several states of India quite effectively with Tamil Nadu and Kerala being at the top of the list. An increasing number of reports from different parts of West Bengal should sensitize us to know more about the modes of transmission and changing trends of the ecosystem in our state, if any.

Nevus depigmentosus is a congenital disorder of pigmentation which occurs in all sexes and races. It commonly presents in childhood and is mostly nonprogressive. It is a form of cutaneous mosaicism with functionally defective melanocytes and abnormal melanosomes. Localized solitary lesions need to be differentiated from conditions like nevus anemicus, ash leaf macule, vitiligo, etc., and generalized lesions from hypomelanosis of Ito. Treatment is usually not required although cosmetic camouflage, phototherapy, grafting, and other modalities may be tried.
