Pé diabético: orientações e conhecimento sobre cuidados preventivos

INTRODUCAO: O pe diabetico e considerado uma complicacao do Diabete mellitus e a maior causa de amputacoes de membros inferiores. Para evitar seu aparecimento sao necessarias orientacoes de medidas preventivas e autocuidado do portador. OBJETIVOS: Verificar o conhecimento dos usuarios do programa de diabetes acerca de cuidados preventivos ao pe diabetico, identificar as orientacoes que o paciente recebe quanto a prevencao, e observar a aderencia aos procedimentos de autocuidado preventivos. MATERIAIS E METODOS: Pesquisa exploratoria de campo, envolvendo 40 diabeticos do tipo 2 e cinco enfermeiros. Foi realizado exame fisico dos pes dos diabeticos e uma entrevista estruturada com os enfermeiros. Os dados foram organizados e analisados por estatistica descritiva. RESULTADOS E DISCUSSAO: No exame fisico verificaram-se grau de mobilidade comprometido em 52,5% dos participantes; uso inadequado de calcados em 85%; retirada de cuticulas em 62,5%. Nao houve diferencas significativas na perfusao entre os pes direito e esquerdo, entretanto houve presenca de micoses e rachaduras. Pontos de alta pressao apresentaram menores sensibilidades. As orientacoes fornecidas pelos enfermeiros sao variaveis, todos afirmam orientar sobre o uso de calcados e corte de unhas; entretanto, nao se verifica adesao a esses itens e faltam orientacoes importantes como o exame diario dos pes. CONCLUSAO: Os itens com menor adesao sao os mais simples e passiveis de correcao. Fazem-se necessarios adequada avaliacao e acompanhamento individual levando em consideracao o grau de conhecimento e a facilidade para processar as informacoes. Uma acao multiprofissional poderia potencializar as orientacoes e aumentar a aderencia as mesmas.

Abordagem dos métodos avaliativos em fibromialgia e dor crônica aplicada à tecnologia da informação: revisão da literatura em periódicos, entre 1998 e 2008

The objective of the present study was to perform a literature review, in an exploratory and analytical way, to determine the research approach in fibromyalgia and chronic pain descriptors related to the matter of information technology. In order to achieve this goal, a bibliographic survey in the electronic databases SciELO, LILACS and PUBMED was carried out. The search for articles was made exclusively in journals found in these databases, and only those published from 1998 to 2008 were considered for the study. The results showed a great variety of assessment questionnaires for fibromyalgia syndrome, among which the following questionnaires stood out in number of citations in articles: Fibromyalgia Impact Questionnaire (FIQ), Quality of Life Assessment - SF-36 Questionnaire, Beck Depression Inventory, Visual Analog Pain Scale (VAS), American College of Rheumatology Criteria (ACR), State-Trait Anxiety Inventory (STAI), and McGill Pain Questionnaire. The Fibromyalgia Impact Questionnaire (FIQ) was the most cited in both fibromyalgia (57%) and chronic pain (56.3%) descriptors. The association of fibromyalgia and chronic pain descriptors with the subject of information technology resulted in 26 articles, representing 9.7% of all articles found in the search. There was also a considerable increase in the number of research carried through during the years 1998-2007, with an annual growth rate of 26%.

Mineração de Dados aplicada à fisioterapia

INTRODUCTION: With the increasing amount of data stored in the practice of physiotherapy and health area in general, expands the possibility of obtaining important information to decision support of health professionals. However, many times the volume of generated data is so great that their use is difficult, requiring more sophisticated procedures for data manipulation. OBJECTIVE: This article aims to present and discuss the potential use of the KDD process on a set of monitoring data for physical therapy patients, as well as its usefulness in decision-making therapeutic or prophylactic. METHODS: We selected a subset of data, referring to records available in a physical therapy clinic, from which were extracted three major groups of data mining tasks: association, classification and clustering. RESULTS: Knowledge was extracted from the data in such a way that allows the reader to understand step-by-step process, broadening their understanding of the results. Knowledge was discovered in various formats, which showed the possible relationships among the variables available. Not only the knowledge was discussed, but also the importance of quality of data collected. CONCLUSIONS: The tasks of classification, association rules and clustering allowed a better understanding of the patients characteristics seen by the clinic in question, thus expanding the knowledge of professionals in the identification of actions to be adopted.

Treino locomotor com suporte parcial de peso corporal na reabilitação da lesão medular: revisão da literatura

Introduction: The locomotor training with bodyweight support (LTBWS) has been used for approximately twenty years in the field of rehabilitation in patients who suffer from neurological pathologies. The LTBWS fa- vors these improvements muscle, cardiovascular and osteo-psychological, because maximum residual potential develops the body, providing reintegration into the familial, social and professional. Objective: Identify the main methods of assessment and their parameters LTBWS with the purpose of contributing to the establishment of reliable evidence for the rehabilitation practice of people with spinal cord. Materials and methods: Original ar- ticles were analyzed, published between 2000 and 2011, involving gait training after spinal cord, with or without partial body weight support, and training assistance technologies such as functional electrical stimulation and biofeedback among others. Results: The majority of the participants of the studies was male; injury levels ranged from C3 to L3, ASIA had scores from A to D; injury times ranged from 0.3 months 33 years. Also it was noted that there is no consensus regarding LTBWS Protocol. Conclusion: The locomotor training with bodyweight support shows up, viable in the rehabilitation of patients who suffer from a neurological pathology such as the spinal

LASERTERAPIA EM ÚLCERAS POR PRESSÃO: LIMITAÇÕES PARA AVALIAÇÃO DE RESPOSTA EM PESSOAS COM LESÃO MEDULAR

Laser therapy is effective in accelerating the scarring of wounds, but there is a shortage of evidence in relation to its use with pressure ulcers and to studies on treatment in people with spinal cord injuries (SCI) undergoing outpatient treatment. The study aimed to report the limitations found in the implementation of a laser therapy program for treating pressure ulcers, supporting a reflection on approaches of this nature. This is exploratory-descriptive research. Six subjects with spinal cord injuries from two rehabilitation centers were followed up at twice-weekly attendances over an eight-week period, receiving laser irradiation on one lesion, the other lesion being the control. The main limitations observed were difficulty in following the guidance on push-up and appearing for the consultations. It is believed that such limitations are related to flaws in the guidance relating to the prevention of these lesions and to changes in sensitivity in the areas affected. DESCRIPTORS: Laser therapy. Effects of cold water immersion on variables of balance in healthy subjects with open and closed eyes

Introduction Cryotherapy is the use of cold as therapeutic approach. Although often used, its impact on the postural balance is not well-known.Objectives To analyze the effects of cold water immersion in the postural balance on single-leg balance in healthy subjects through the variables of center-of-pressure, velocity anteroposterior and medial-lateral oscillations, comparing conditions open and closed eyes in the moments before, immediately, 20 and 40 minutes after cold water immersion.Material and methods Cross-sectional study with 30 male subjects, cold water immersion at 5 °C during 15 minutes assessed in a force platform, protocol of 3 trials of 30 seconds each with 10 seconds of rest, the average of the 3 trials was used for analysis. The software GraphPad Prisma 5.0 was used for statistical analysis, with the ANOVA test for repeated measures and comparisons with test of Friedman and post-test of Dunn.Results It was observed improvement of the balance by the results of the following variables: in the condition Open Eyes (OE) it was observed significant difference only in the velocity anteroposterior (AP) in the moment before immersion when compared to the 40-minute moment. Significant differences were observed in the condition Closed Eyes (CE) in the following comparisons: in the Center-of-Pressure (COP) only at the immediate moment when compared to the 40-minute moment; in the velocity AP in the moment before when compared to the 40-minute moment, in the immediate moment when compared to the 20-minute and 40-minute moments; and in the medial-lateral velocity (Vel-ML) in two comparisons: in the moment before immersion when compared to the 20-minute and 40-minute moments after cryotherapy. Results show safety in performing activities after cryotherapy.Conclusion Improvement of the postural balance after cold water immersion on the predetermined conditions of this study.

Continuous therapeutic ultrasound in the healing process in rat skin

Introduction : The therapeutic ultrasound is one of the main resources utilized on physical therapy to stimulate the healing due its thermal and mechanic effects. However, your application still not properly standardized. Objective :To analyze the presence of the collagens types I and III stimulated by the ultrasonic wave in continuous mode at the cutaneous injury local for first intention. Material and methods : It was utilized 90 Wistar rats, young adults, with average weight of 230g, divided in 3 groups of 30 animals: control group, treated with ultrasound off; group 1, treated with ultrasound 0.5 W/cm2 and group 2, treated with ultrasound 2.0 W/cm2. Each group was subdivided in 3 subgroups according with the healing phases, 3, 7 and 21 days of consecutive therapeutic sessions. The ultrasound therapy began 24 hours after the termination of the surgical act, at the dorsal region, applying frequency of 3 MHz and intensities 0.5 W/cm2 and 2.0 W/cm2 during 5 minutes under sliding technique. Later, the animals were sacrificed according to the healing phases for removal of incisional area and histological analyses. Conclusions : The results allowed conclude that the continuous mode of therapeutic ultrasound in the intensities 0.5 W/cm2 and 2.0 W/cm2promoted stimulus to formation of collagens types I and III in the lesion area for first intention.

Acute effects of short and long duration dynamic stretching protocols on muscle strength

Objective Compare the acute effects of dynamic stretching protocols on the isokinetic performance of the quadriceps and hamstring muscles at two velocities in adult males.Methodology Included the participation of 14 males (21 ± 2.6 years; 178 ± 0.4 cm; 73.2 ± 20.9 kg) were assessed using an isokinetic dynamometer before and after following a short or long-duration dynamic stretching protocol or a control protocol. The results were assessed by a two-way ANOVA and a Scheffe’s post hoc test at a 5% significance level.Results No difference was found in the variables assessed at 180°/s after LDDS. At 60°/s, LDDS reduced the power of the knee flexors. The control protocol reduced the power of the knee flexors and increased the power of the extensors. At 60°/s, the work of the knee flexors exhibited a reduction after LDDS. The control protocol resulted in a reduction in the work of the flexors. The peak torque angle exhibited a reduction in the extensors and flexors after LDDS and SDDS.Conclusion Dynamic stretching did not cause any change in the peak torque, which points to its possible use in activities involving velocity and muscle strength. The executing dynamic stretching before physical activities such as running and high-intensity sports might be beneficial by promoting increases in heart rate and in body temperature.

Isokinetic performance, functionality, and pain level before and after lumbar and pelvic estabilization exercise in individuals with chronic low back pain

Introduction Chronic low back pain with a mechanical and degenerative origin is among the most common symptoms experienced by people all over the world. Objective To analyze the effects of a protocol for lumbar and pelvic segmental stabilization with regard to isokinetic variables, functionality, and pain in patients with low back pain. Materials and methods The sample consisted of 21 subjects, 8 female and 13 male, with a mean age of 42.6 ± 12.5 years. We used tests for mobility (modified Schober index), flexibility (finger-floor distance), functionality (Roland-Morris questionnaire), pain (Visual Analog Scale of Pain), and the isokinetic variables torque peak, work, flexors/extensors ratio before and after application of a protocol for segmental stabilization in subjects with chronic low back pain with a mechanical and degenerative origin. This protocol was used for two months, being applied twice a week. The data were statistically tested by means of the software Statistica, version 8.0. Results There were significant improvements for the variables flexibility (p = 0.014), functionality (p < 0.001), pain level (p < 0.001), torque peak flexion (p = 0.008), torque peak range (p < 0.001), torque flexion (p = 0.001), torque range (p < 0.001), and flexors/extensors ratio (p = 0.001). Conclusion Even with the improved levels of trunk muscles strength, both flexors and extensors, it was not possible to rebalance the segment according to data available in the literature. It is believed that there is a need for a protocol with a longer application period, maintaining the recruitment of extensor muscles. Regarding mobility of the lumbar segment, the protocol showed no significant differences in post-treatment (p = 0.520). This may be attributed to improved muscle control, providing the increased passive stiffness required to promote stability.
