Long-Term Outcome of Anatomical Reconstruction Versus Tenodesis for the Treatment of Chronic Anterolateral Instability of the Ankle Joint: A Multicenter Study

The long-term clinical outcome after anatomical reconstruction and tenodesis in the treatment of chronic anterolateral ankle instability was assessed in a retrospective multicentre study. The first group (AR) consisted of 25 patients (mean age at operation 22 yrs ± 5.7) who underwent anatomical reconstruction and the second group (TE) of 29 patients (mean age 23 yrs ± 6.6) who underwent tenodesis. For both groups, the mean follow-up period was 12.3 yrs (AR ± 2.5 yrs, TE ± 2.7 yrs). At physical examination, there were significantly more patients in the TE group (n = 18) with a positive anterior drawer sign as compared with the AR group (n = 7) (p = 0.02). Medially located degenerative changes in the ankle joint as seen on standard radiographs were seen more often in the TE group (n = 7) than in the AR group (n = 1) (p = 0.03). The mean talar tilt, 4.7° in the AR group vs 6.9° in the TE group, (p = 0.02) and anterior talar translation, 2.9 mm in the AR group vs 4.3 mm in the TE group, (p = 0.04) were significantly higher in the TE group at radiographic stress examination. According to the rating system developed by Good et al. (1975), significantly fewer patients in the TE group (n = 8) had an excellent result as compared with the AR group (n = 15) (p = 0.03) and more patients in the TE group (n = 9) had a fair or poor result (p = 0.04) as compared with the AR group (n = 2). We conclude that a tenodesis procedure does not restore the normal anatomy of the lateral ankle ligaments. When compared with anatomical reconstruction, a tenodesis leads to inferior results in terms of functional and mechanical stability, as well as overall satisfaction at long-term follow-up.

Anatomical reconstruction versus tenodesis for the treatment of chronic anterolateral instability of the ankle joint: a 2- to 10-year follow-up, multicenter study.

Abstract The clinical outcome of anatomical reconstruction or tenodesis in the treatment of chronic anterolateral ankle instability was assessed in a retrospective multicenter study. The anatomical reconstruction group (group A) consisted of 106 patients (mean age at operation 24 ± 8.4 years) and the tenodesis group (group B) of 110 patients (mean age at operation 26 ± 11.4 years). Patients were evaluated at a mean follow-up of 5.5 ± 2.8 years in group A and 5.2 ± 2.9 years in group B. The review protocol included patient characteristics, physical examination, two ankle scoring scales to evaluate the functional results, and standard anteroposterior and lateral radiographs to evaluate degenerative changes. Mechanical stability was evaluated using standardized stress radiographs. A larger number of reoperations was performed in group B (P = 0.008). At physical examination, more patients in group B had a smaller range of ankle motion than those in group A (P = 0.009). ¶A larger proportion of patients in group B had medially located osteophytes, as seen on standard radiographs (P = 0.04). On stress radiographic examination, the mean talar tilt (P = 0.001) and mean anterior talar translation (P < 0.001) were seen to be significantly greater in group B than in group A. There were no differences in mean Karlsson score between the groups, but more patients in group A had an excellent result on the Good score (P = 0.011). Unlike anatomical reconstructions, tenodeses do not restore the normal anatomy of the lateral ankle ligaments. This results in restricted range of ankle motion, reduced long-term stability, an increased risk of medially located degenerative changes, a larger number of reoperations, and less satisfactory overall results.

The role of the medial ligamentous structures on patellar tracking during knee flexion

PurposeThe influence of the medial patellar ligamentous structures on patellar tracking has rarely been studied. Thus the main purpose of this cadaveric biomechanical study was to determine the influence of the medial patellofemoral (MPFL), medial patellomeniscal (MPML) and medial patellotibial (MPTL) ligaments on the three-dimensional patellar tracking during knee flexion. This study was conducted using a validated cadaveric optoelectronic protocol for analysis of patellar kinematics.MethodsFor each cadaveric knee study, four successive acquisitions were performed; first was studied patellar tracking in healthy knees, then the junction between MPFL and vastus medialis obliquus (VMO) was sectioned, the MPFL was released at its patellar attachment and finally was released the insertion of the MPML and MPTL.ResultsIn this study, the MPFL accounts for 50–60% of the medial stabilization forces of the lateral patellar shift during patellar engagement in the femoral trochlea. This work confirm and clarify the role of the MPFL as the primary stabilizer of the patella during the initial 30° of knee flexion. Moreover, this study shows no significant results regarding the stabilizing action of the VMO on the patella during knee flexion.ConclusionThis in vitro study, conducted with an experimental protocol previously validated in the literature, helps quantify the actions of the MPFL, the VMO, and the MPML/MPTL respectively, and identify areas of joint motion where these structures have the most significant influence. This confirms the importance of reconstruction in the treatment of chronic patellar instability. During its reconstruction, care should be taken to adjust the MPFL balance during the initial 20°–30° of flexion.

Sports Activity Level after Surgical Treatment for Chronic Anterolateral Ankle Instability A Multicenter Study

The objective of this retrospective multicenter study was to determine whether anatomic reconstruction or tenodesis produces better results in athletic patients with lateral ankle instability. Forty-one patients who underwent anatomic reconstruction and 36 patients who underwent tenodesis were followed up. The median preinjury Tegner score for both groups was 9 (range, 7 to 10). At follow-up, 2 to 10 years after surgical treatment, the median Tegner score for both groups was 8 (range, 4 to 10). In the tenodesis group, 17 patients had a lower Tegner score than before the operation, but in the anatomic reconstruction group only 4 patients had lower scores. Significantly more patients in the tenodesis group (15) had limited ankle dorsiflexion than in the anatomic reconstruction group (3). Plain radiographs revealed that 11 patients in the tenodesis group had medially located osteophytes, compared with only 2 patients in the anatomic reconstruction group. Stress radiographs revealed that more patients in the anatomic reconstruction group had normal laxity values than in the tenodesis group (38 and 28, respectively). According to the rating system developed by Good et al., 36 patients in the anatomic reconstruction group had a good or excellent result, versus 21 patients in the tenodesis group. Anatomic reconstruction was found to be superior to tenodesis in all of the investigated outcome measures.

Repeat Revision of Anterior Cruciate Ligament Reconstruction A Retrospective Review of Management and Outcome of 10 Patients With an Average 3-Year Follow-Up

Background To the authors’ knowledge, no previous published study has focused on management and outcome of repeat revision of anterior cruciate ligament reconstruction in terms of functional result and meniscus and articular cartilage status. Hypothesis Repeat revision of anterior cruciate ligament reconstruction improves knee stability, but with inferior results for functional outcome compared with primary anterior cruciate ligament reconstruction. Meniscal tears and subsequent articular cartilage degeneration are more prevalent with successive revisions due to recurrent laxity. Study Design Case series; Level of evidence, 4. Materials and Methods: Between February 2003 and November 2006, a consecutive series of 10 patients with an average age at 30 years (range, 17-48) were operated on for a repeat revision of anterior cruciate ligament reconstruction (2 revisions after a primary reconstruction) with arthroscopic procedures. A clinical and a radiographic evaluation were performed to assess anterior cruciate ligament reconstruction failures, outcome of revisions, and causes of failures. Meniscal tears and articular cartilage lesions were analyzed. Results The average follow-up of the second revision was 38 months (range, 12-61). At latest follow-up, final International Knee Documentation Committee assessment was excellent or good in 7 cases. Postoperatively, only 2 patients recovered to the same sports activity level they had before their first anterior cruciate ligament reconstruction. Four had a lower level, and 4 discontinued sports activity. The postoperative average side-to-side KT-1000 arthrometer maximum manual difference was 1.3 ± 1.9 mm. Nine patients had meniscal tears and 7 had articular cartilage lesions. Meniscal tears, meniscectomies, and articular cartilage degeneration increased after the second revision (P = .016, P = .0098, and P = .0197, respectively). Severe articular cartilage degeneration (International Cartilage Repair Society grade III and IV lesions) was found in patients with bad functional outcome (final International Knee Documentation Committee assessment C or D) (P = .0472). Incidence of articular cartilage degeneration was found to be more prevalent in cases of meniscal tears and partial meniscectomy at the same tibiofemoral compartment (P = .0157). Index anterior cruciate ligament reconstruction and first revision failures were caused by recurrent trauma (60% and 70%, respectively) or a surgical technical error with tunnel malpositioning (40% and 10%, respectively). Conclusion Outcome of repeat revision of anterior cruciate ligament reconstruction was excellent or good in 70% of the cases, although decreased after the second revision, in relation to the occurrence of meniscal tears and articular cartilage lesions. Meniscal and articular cartilage lesions were more frequent and more severe with recurrent laxity. The cause of failures was mainly recurrent trauma, followed by surgical technical errors.

Study of patellar kinematics after reconstruction of the medial patellofemoral ligament.

BACKGROUND Medial patellofemoral ligament reconstruction is currently the technique of choice for the treatment of patellar instability. But what should be the most appropriate graft tension for optimal restoration of patellofemoral kinematics? METHODS Six freshly frozen cadaveric knees were studied, the three bone segments were respectively equipped with opto-reflective markers. The acquisitions were made using the Motion Analysis System®. Six successive acquisitions were performed for each knee under different levels of graft tension. FINDINGS With an intact medial patellofemoral ligament, the medial patellar tilt increased up to a mean value of 2.02° (SD 3.1), the medial patellar translation gradually increased up to a mean value of 3.3mm (SD 2.25) with a slight lateral rotation over the first 30° of knee flexion with a maximum mean value of 1.22° (SD 0.8) at 20° of knee flexion. Reconstruction of the medial patellofemoral ligament was performed using different levels of tension applied to the graft. Only 10 N of graft tension could restore normal patellar tilt, lateral shift and rotation, with results approximating those measured on healthy knee. INTERPRETATION This study confirms the role of the medial patellofemoral ligament in providing adequate patellar stability during the first 30° of knee flexion. According to our findings, a 10 N tension applied to the graft appears sufficient to ensure proper control of patellar tracking whereas 20, 30 and 40 N of tension are excessive tension values inducing a major overcorrection in all studied parameters.

Patellar tendon autograft reconstruction of the anterior cruciate ligament with and without lateral plasty in advanced-stage chronic laxity. A clinical, prospective, randomized, single-blind study using passive dynamic X-rays

PURPOSE A prospective randomized study was performed to assess the influence of extra-articular ilio-tibial band tenodesis on the results of arthroscopic anterior cruciate ligament (ACL) reconstruction in patients with advanced-stage chronic anterior laxity of the knee. METHODS Preoperatively, the two constituent groups of our series of 120 patients: group 1 (Kenneth Jones) and group 2 (Kenneth Jones+extra-articular ilio-tibial band tenodesis) were strictly comparable with regard to demographic data, activity level, interval between the injury and the operation, and even objective laxity. Through radiological measurements made by passive dynamic X-rays, we studied the evolution of the objective laxity on the two compartments (medial and lateral) of the knee before the surgery and in review. RESULTS At 2 years follow-up, there was no significant difference between the two groups in terms of the subjective result, sports, and the overall international knee documentation committee score, however, In terms of objective laxity; Gain laxity obtained after surgery on the lateral compartment, was statistically higher in cases of extra-articular associated plasty (+29%), by cons in cases of intra-articular reconstruction alone, the laxity of the lateral compartment was poorly controlled and has continued to evolve despite the plasty of the ACL. CONCLUSION The indication of an associated extra-articular plasty remains very discussed but we plead for an objective criterion with knowing the importance of preoperative objective laxity especially that of the lateral compartment to decide if it necessary, or not, being associated. Throwing Performance is Associated with Muscular Power

The aim of the present study was to test the hypothesis that performance in throwing events is associated with muscular characteristics of both upper and lower limbs. Thirty-eight male throwers volunteered to participate. Bench press and half squat tests were conducted on a guided barbell. The barbell displacement signal was recorded using a kinematic system. Maximal power, corresponding optimal velocity and force (P(max)S, V(opt)S, F(opt)S and P(max)BP, V(opt)BP, F(opt)BP for half squat and bench press, respectively) were extrapolated from the power-velocity relationship. Lower limb stiffness (K) was determined during maximal hopping. The results demonstrated that P(max)S and P(max)BP were correlated with each throwers seasons best performance (SBP, R=0.54, P<0.01 and R=0.71, P<0.001, respectively). P(max)S expressed relative to body mass was not correlated with SBP. K was significantly correlated with SBP (R=0.66, P<0.001). The relationship between P (max)BP expressed relative to body mass and SBP remained significant ( R=0.54, P<0.001). The results of the study suggest that high strength and stiffness values for lower limbs and strength and velocity characteristics for upper limbs may be associated with athletic throwing performance.

Reproducibility of an optical measurement system for the clinical evaluation of active knee rotation in weight-bearing, healthy subjects

INTRODUCTION A knee is typically evaluated passively by a clinician during an office visit, without using dedicated measurement tools. When the knee is evaluated with the patient standing and actively participating in the movement, the results will differ than when the knee is passively moved through its range-of-motion by the surgeon. If a precise measurement system was available, it could provide additional information to the clinician during this evaluation. HYPOTHESIS The goal of this study was to verify the reproducibility of a fast, flexible optical measurement system to measure rotational knee laxity during weight-bearing. MATERIAL AND METHODS Two passive reflective targets were placed on the legs of 11 subjects to monitor femur and tibia displacements in three dimensions. Subjects performed internal and external rotation movements with the knee extended or flexed 30°. During each movement, seven variables were measured: internal rotation, external rotation and overall laxity in extension and 30° flexion, along with neutral rotation value in 30° flexion. Measurement accuracy was also assessed and the right and left knees were compared. Reproducibility was assessed over two measurements sessions. RESULTS The calculated intra-class correlation coefficient (ICC) for reproducibility was above 0.9 for five of the seven variables measured. The calculated ICC for the right/left comparison was above 0.75 for five of the seven variables measured. DISCUSSION These results confirmed that the proposed system provides reproducible measurements. Our right/left comparison results were consistent with the published literature. This system is fast, reproducible and flexible, which makes it suitable for assessing various weight-bearing movements during clinical evaluations. LEVEL OF EVIDENCE Level III, experimental study.
