Transferring an Artifact Handling System into a Mobile Long-term ECG Device with Dry Electrodes

This paper presents a methodology for the transfer of an artifact handling system into a mobile ECG monitoring device. The realization of a well working artifact handling system is achievable even where strong constraints like limited processing power and the use of a textile wearable system with dry ECG electrodes occur. The presented solution can increase the appraisal quality significantly in 24/7 monitoring, where fully automated ECG analysis is necessary. This system is equipped with algorithms for ECG channel selection, baseline wander removal and artifact detection. The outcome is a significant improvement in the automatic QRS detection.

Automatic Detection of Atrial Fibrillation for Mobile Devices

Two versions of a new detector for automatic real-time detection of atrial fibrillation in non-invasive ECG signals are introduced. The methods are based on beat to beat variability, tachogram analysis and simple signal filtering. The implementation on mobile devices is made possible due to the low demand on computing power of the employed analysis procedures. The proposed algorithms correctly identified 436 of 440 five minute episodes of atrial fibrillation or flutter and also correctly identified up to 302 of 342 episodes of no atrial fibrillation, including normal sinus rhythm as well as other cardiac arrhythmias. These numbers correspond to a sensitivity of 99.1 % and a specificity of 88.3%.

Der Grundsatz der Pflegeversicherung „ambulant vor stationar“ druckt dabei einerseits die Intention der Gesundheitspolitik aus, die familiale Pflegebereitschaft zu fordern und anderseits die Vorstellung, dass man faktisch immer noch auf die Stabilitat eines Familiennetzwerkes informeller Helfer zuruckgreifen kann (Meyer und Dohner 2006).
