Prevalência de fissuras labiopalatais no município de Campos dos Goytacazes-RJ, 1999-2004

O objetivo deste estudo foi determinar a prevalencia de fissuras labiopalatais no municipio de Campos dos Goytacazes-RJ, Brasil, em criancas nascidas entre 01/01/1999 e 31/12/2004. Foram considerados a faixa etaria, tipo de fissura, sexo e raca da crianca, alem do estado civil, grau de escolaridade e procedencia da mae do portador na epoca do parto. Deste modo, foram verificados os prontuarios de pacientes inscritos em servicos de referencia em tratamento de anomalias craniofaciais, com o objetivo de selecionar os nascidos e/ou domiciliados no municipio de Campos dos Goytacazes. Apos selecao inicial, foi realizada busca no Sistema de Informacoes sobre Nascidos Vivos (SINASC), definindo o universo do estudo, a saber: nascidos vivos entre 1999 e 2004, cujas maes eram residentes em Campos dos Goytacazes. Neste periodo, o numero de nascidos no municipio, por residencia da mae, foi de 46.707, dos quais 63 possuiam algum tipo de fissura labiopalatal, representando uma prevalencia de 1,35 casos por 1000 nascidos vivos. A analise dos dados revelou que 55,6% dos pacientes eram do sexo masculino. Os tipos de fissuras mais encontrados foram o de labio e palato e o de palato isolado (34,9% cada). Embora a raca branca tenha representado 61,3% dos individuos estudados, o percentual de casos por raca nao diferiu estatisticamente em relacao a distribuicao de nascidos na populacao. Com relacao a mae do portador, a maior parte das maes era solteira ou casada, possuindo como caracteristica o baixo grau de escolaridade.

Periapical status and prevalence of endodontic treatment in institutionalized elderly.

The aim of this study was to determine the prevalence and frequency of apical periodontitis and root fillings in 450 institutionalized Brazilian elderly. The teeth with apical periodontitis were assessed using the Periapical Index (PAI) score. A total of 942 teeth were evaluated in 98 subjects (57 females and 41 males) with mean age of 74 years. The observed frequency of total edentulous subjects was high (76%) in this population. A total of 126 teeth showed root fillings, of these only 46 (36.5%) were scored as having adequate quality. Apical periodontitis was found in 114 teeth (12.1%) in 42 (42.9%) subjects. 80 (70.2%) inadequate root-filled teeth showed apical periodontitis. There was a significant correlation between the presence of periapical pathology and inadequate root-filled teeth. Inadequate root-filled teeth were associated with an increased prevalence of apical periodontitis in these subjects. This fact may result in increased endodontic retreatment needs for this population.

Fissuras orais e sua notificação no sistema de informação: análise da Declaração de Nascido Vivo (DNV) em Campos dos Goytacazes, Rio de Janeiro, 1999-2004

O objetivo deste estudo foi investigar a notificacao de ocorrencia de fissuras orais na Declaracao de Nascido Vivo (DNV), atraves dos tres itens do campo 34 (notificacao de malformacao congenita). Para tal, foram verificados os prontuarios de pacientes inscritos em servicos de referencia em tratamento de anomalias craniofaciais, incluindo os portadores de fissuras labiopalatinas e nascidos no municipio de Campos dos Goytacazes (RJ) entre 01/01/1999 e 31/12/2004. A seguir, foi feito levantamento junto ao Setor de Dados Vitais da SMS, onde todos os sujeitos da amostra (63) tiveram suas DNV localizadas. Constatou-se que apenas 53,3% das DNV apresentavam o registro de malformacao no primeiro item e a subnotificacao ocorreu em todos os tipos de fissura, sendo maior na fissura palatina (70%). Quanto a descricao (2o item), a fissura palatina apresentou o maior numero de erros, sendo descrita corretamente em 25% dos casos. Todos os documentos estudados apresentaram o codigo da malformacao (3o item) em branco. Nao houve diferenca estatistica entre os diferentes profissionais quanto ao preenchimento dos formularios. Deste modo, concluiu-se que a DNV se mostrou ineficiente no registro das fissuras labiopalatais, em decorrencia de falhas no preenchimento, principalmente pela ausencia do preenchimento do codigo da CID-10.

Ethical and legal considerations on professional liability of the orthodontist

INTRODUCTION: After the enactment of Law 8078, of September 11, 1990, the Consumers Defense Code implied important alterations in Brazils legal scenario, providing a greater balance in the relationship between consumers and service providers. From this law, dental surgeons came to establish a consumer relationship with their clients. OBJECTIVE: Due to the ethical and legal issues against the dental professionals, this work makes general considerations about the nature of the dentist´s obligation in services, specifically in Orthodontics. CONCLUSION: The responsibility of the professional shall be restricted to predictable risks and undertaken obligations. When the professional warns the client in a correct, clear and express manner, it will lower the chances of being later charged.

Dental age assessment: which is the most applicable method?

To evaluate which method is the most applicable for estimating estimate dental age in Brazilian children, Nolla or Demirjian. For this purpose, both methods were reviewed and the differences between them brought to light. A total of 403 healthy children between the ages of 7 and 13 years with satisfactory panoramic radiographs, similar socio-economic background and ethnic origin were included. Panoramic radiographs were examined by the Nolla and Demirjian methods. Dental age was calculated for both methods, and the differences between the dental age and the chronological age were compared with analysis of variance with post hoc Dunnett. The Nolla method showed no significant difference in relation to chronological age in the majority of age groups for boys and girls, except for 12-year-old boys (over-estimation of 1.00) and between 11- and 12-year-old girls (over-estimation of 0.51 and 0.59, respectively). An over-estimation of the dental age was observed by using the Demirjian method (for boys 0.89-1.84 and for girls 0.69-1.97) for all age groups. The Nolla method is suitable for Brazilian children when it comes to age estimate with care to growth spurt beginning (around 11 and 12 years). However, the Dermijian method should not be used, because it over-estimated the age in both sexes.
