Hemodynamic oxygenation effects during the bathing of hospitalized adult patients critically ill: systematic review

Objectives: To identify scientific evidence on the impact of hemodynamic oxygenation of the bathing in the adult patient hospitalized in critical condition; to verify the possibility of establishing criteria for the indication of the bathing in that patient, based on hemodynamic effects of oxygenation in different clinical situations. Methods: Systematic review of primary and secondary literature, without restriction of time or language. PIO strategy used: P (problem) = “Intensive Care Units” and variations, I (intervention) = bathing and variations; O (result) = “Hemodynamic Phenomena” / “Oxygen Consumption” and variations. Sources: CINAHL databases, Dedalus, EMBASE, COCHRANE, LILACS, PubMed / MEDLINE; libraries of Nursing Schools, Fluminense Federal University and Federal University of Rio de Janeiro; cross references, and, articles from PubMed and ISI. Results: Of 44,597 references six quasi-experiments remained. During the bathing, mixed venous oxygen saturation declined significantly from baseline, being restored 30 minutes later. WOUND INFECTION - SISTEMATIC LITERATURE REVIEW

Objectives: To review, identify and analyze the main aspects of the scientific production of healthcare professionals about wound infection, providing an update of relevant technical information for improving their capabilities for a better injury care. Methods: This is a review of the literature contained at bireme and pubmed databases published from 2002 to 2008. Results: The results revealed nineteen publications and all the information data were subjected into thematic analysis, divided into three distinct categories: the best evidence for the prevention and control of the infections, the association between infections and the places where the injuries are present and the indication of the best healing implementation techniques. Conclusion: There is a consensus that professional nurses must always update themselves for the promotion of the correct epidemiological studies about the patient’s social and demographic profiles in order to improve adequate identification of the microorganisms probably concerned to these infections. The development of new standardized protocols leading to uniform practices into reliable results are necessary for delineating a pattern of a specialized nursing care in the search for the total quality being the nurse the prime agent in this question.

Adtendite a falsis prophetis qui veniunt ad vos

Each segment of a society is guided by parameters established by regulatory bodies instituted by the society itself. In this sense, laws, codes, norms and monitoring indicators of such a society are established. It is legitimate to say that laws should follow the will of the people and not the opposite. It is also legitimate to question certain indicators in terms of their real benefits, as well as the context in which they were conceived and, above all, operationalized. Roughly speaking, it would be reasonable to assume that most indicators are created with good intentions, although often, in reality, the theory is different. In the field of publishing and editing within the health sector, the logic of indicators is no different. Ultimately, the inclusion of a journal in an internationally recognized database, implies recognizing that it has incorporated certain quality parameters that include the preservation of ethics in research and publications, the intrinsic quality of the construct and the external validity of its results.

Instrumento para a realização de exame físico: contribuindo para o ensino em enfermagem

Objetivou-se elaborar uma proposta de instrumento para a realizacao de exame fisico de adultos e idosos por academicos de enfermagem. O instrumento proposto derivou da analise obtida pela coleta de dados sobre exame fisico por meio de revisao de literatura e de mapeamento cruzado com a Classificacao Internacional para a Pratica de Enfermagem CIPE® para a adicao, subtracao ou adequacao com terminologia preexistente. O cotejamento entre as versoes evidenciou que, de 296 termos descritos na literatura, apenas 66 (22,1%) eram previstos na CIPE®. A versao final do instrumento proposto consta dos itens: nome, idade, leito, numero de prontuario, diagnostico medico, data de realizacao do exame, setor de internacao, e itens sobre: nivel de consciencia e dependencia, estado geral, cabeca e pescoco, torax, orgaos genitais, membros e dispositivos invasivos, incluindo legendas norteadoras e uma legenda k relacionada as alteracoes de pele no final do instrumento. Conclui-se que a versao final esta pronta para ser testada com os academicos.Resumen The objective was to develop a proposal for an instrument to carry out physical examination of adults and elderly by nursing students. The proposed instrument was derived from analysis obtained by collecting data on physical examination by means of literature review and cross-mapping with the International Classification for Nursing Practice (CIPE) for addition, subtraction or fit with pre-existing terminology. The comparison between the versions showed that from 296 as described in the literature, only 66 (22,1%) were provided in the CIPE. The final version of the proposed instrument consists of items: name, age, bed, number of records, medical diagnosis, date of examination, hospital sector, items: level of consciousness and dependency, general condition, head and neck, chest , genitals, limbs and invasive devices , including subtitles and a guiding legend k related to skin changes at the end of the instrument. It is concluded that the final version is ready to be tested along with academics.

Evaluation of bedbath in critically ill patients: impact of water temperature on the pulse oximetry variation

Estudio participativo, cuasi-experimental, tipo antes y despues, que utilizo abordaje cualitativo de medidas biofisiologicas, representadas por la saturacion de oxigeno medida por el oximetro de pulso (SpO2), registrada en tres momentos: antes, durante y despues de un bano en el lecho del enfermo critico internado en UTI de un hospital universitario en Brasil. Objetivo: comparar la SpO2 en diferentes momentos del bano, con y sin control de temperatura de agua. Muestra de conveniencia recolectada entre diciembre de 2007 y abril de 2008, compuesta por 30 pacientes con edad superior a 18 anos con clasificacion en TISS-28 a partir de nivel II. Los resultados muestran que la temperatura de agua controlada determina una menor variacion en la SpO2 (p<0,05). No fueron establecidas nitidas diferencias en la variacion de saturacion entre hombres y mujeres o entre los grupos etarios. Se concluyo en que la temperatura del agua precalentada y constante durante el bano en el lecho es capaz de minimizar la caida de la SpO2 que ocurre durante la manipulacion del paciente para la realizacion del procedimiento.This is a participant study, quasi-experimental, of a before and after type. A quantitative approach of biophysiological measures was used, represented by the saturation of oxygen measured by pulse oximeter (SpO2), and recorded on three occasions: before, during and after the bedbath in critically ill patients hospitalized at the ICU of a University Hospital in Brazil. Objective: to compare the SpO2 in various stages of the bath, with and without control of water temperature. Data collection was performed between December 2007 and April 2008 on a convenience sample consisting of 30 patients aged over 18 who had classification in TISS-28 from level II. Results show that water temperature control means a lower variation of SpO2 (p<0.05). No marked differences in variation of saturation between men and women or between age groups were established. In conclusion, heated and constant water temperature during the bedbath is able to minimize the fall of SpO2 that occurs while handling patients during procedures.

El uso del preservativo entre los participantes del Carnaval - perspectiva de género

Objective: This study has the purpose of identifying the use of condoms among men and women who participate in Carnival. Methods: Descriptive research with a quantitative nature using a sample according to convenience. The subjects were spectators, partygoers and workers present in the carnival parade at Sambadrome, in Rio de Janeiro, Brazil. Data were collected in February of 2013, with 1067 respondents, by using a form. The analysis was performed using EpiInfo program. Results: Men use and have in-hand condoms in their day by day more often than women, they agree with the statements that the condom interferes in the sexual relation, they are more impulsive and tend to be expose themselves to risk situations. Conclusion: Inequality between the sexes and the hegemonic normative of male gender prevails in our society and contributes to the vulnerability of men and women.
