Giant unilateral hydrocele "en-bisac" with right hydronephrosis in an adult: A rare entity.

Dharamveer Singh; Pallavi Aga; Apul Goel

Abdominoscrotal hydrocele consists of two large sacs; the abdominal and scrotal, both are connected via the inguinal canal. The diagnosis is made by clinical examination and can be confirmed with ultrasound scan. Surgical treatment is considered mandatory since spontaneous resolution is extremely rare. Herein, we report a case of giant unilateral hydrocele en-bisac who presented with spontaneous rupture of the sac.

Egg shell in bladder: A calculus around neglected Foley balloon catheter.

Dharamveer Singh; Pawan Vasudeva; Apul Goel

Suprapubic catheterization of the bladder is used as a short- or long-term alternative to urethral catheterization. As with any indwelling urinary catheter, proper care is vitally important to minimize complications.

OBJECTIVES To share our experience with nondismembered ureteroplasty (NDU), a novel application of an old technique in congenital midureteral stenosis (CMS). METHODS A 35-year-old man was diagnosed as a case of CMS, a rare benign condition on the basis of extensive evaluation, including ultrasonography, intravenous urography, ethylenedicystine renal scan, antegrade and retrograde contrast study of the left kidney and ureter, and a 64-slice computed tomographic angiography of the abdomen with 3-dimensional reconstruction. He was managed by open NDU, where a longitudinal incision over the narrowed segment was closed transversely using the Heineke-Mikulicz principle, thereby preserving the midureteral blood supply, which is considered tenuous. RESULTS The patient had a successful outcome and is asymptomatic at 1-year follow-up, with significant resolution of hydroureteronephrosis on ultrasonography, and a nonobstructed left kidney with 37% differential renal function on renal scan. CONCLUSIONS Nondismembered ureteroplasty is a meticulous surgical technique emphasizing blood supply preservation; it can be a useful option in relatively long-segment CMS and adds another method in the surgeons armamentarium for repairing this rare condition. This technique can be especially useful during laparoscopic or robotic repair of such stenosis.

Forgotten stent leading to complex panurinary stone: single-sitting endourologic management

Dharamveer Singh; Apul Goel; Nisar Ahmed; Bhupendra Pal Singh

A case of forgotten and fragmented JJ ureteral stent with significant complex, panurinary stone bulk (stone in kidney, ureter and bladder) was managed in a single sitting using mechanical lithotrite for bladder and ureteral components and percutaneous nephrolithotripsy for the renal component.

Asymptomatic posthaemodialysis femoral arteriovenous fistula: multidetector computed tomography (MDCT) evaluation and management

Priyank Yadav; Sandeep Nunia; Dharamveer Singh; Hira Lal

A 33-year-old man with end-stage renal disease underwent right femoral venous puncture by palpatory method for haemodialysis. Following puncture, there was profuse bleeding which was controlled with thrombin injection. Thereafter, he was asymptomatic. Four months later, during renal transplant workup, a prominent thrill was found at the site of femoral access. Doppler ultrasound demonstrated a fistula between the femoral artery and femoral vein. Multidetector computed tomography (MDCT) demonstrated the location of fistula (figure 1) for which he underwent surgical repair. Postoperative MDCT showed successful resolution of fistula (figure 2). The patient underwent renal transplantation in the right iliac fossa and …
